POLICY ON MISSING APPOINTMENTS & RE-REFERRAL TO SPECIALIST CLINICS Please make every effort to attend the appointments that have been reserved for you. If you need to cancel or change the appointment time, please give us plenty of notice by contacting our Appointments Secretary on 020 7561 7237 or phone our appointments line on 020 7272 1585. We understand that patients sometimes cannot attend their appointment for reasons that may be beyond their control and we ask all you to cancel and rearrange appointments if you can’t make it. If you don’t let us know you’re not coming, we waste a valuable 30 minute appointment that could have been given to someone else. So we have a policy that if you miss one appointment you will be offered one more. If you then miss a second appointment without giving plenty of notice, you will be discharged back to your GP. We welcome re-referrals to the service, however if you continue to miss appointments and require a second discharge for this reason, we regret that we shall be unable to accept another re-referral. We provide support, advice and expert guidance to patients who need help and support with understanding and managing their diabetes from day to day and/or who are about to start insulin or one of the newer diabetes treatments. Your GP has referred you to our Specialist Diabetes clinic because s/he thinks you will benefit from the help and guidance that our Specialist Diabetes Nurse, Jill Lankester, can give you. You and your GP will already have discussed your referral and this leaflet explains what will happen next. 1. When we receive your referral form, we will register you as a “referred” patient at St John’s Way Medical Centre. 2. Your GP will send us some important medical history relevant to your diabetes and this will be stored on our computerised Patient Database. Your medical record is confidential. We are registered under the Data Protection Act and we will not share any of your personal information with anyone outside the surgery except your own GP. Our Appointments Secretary will telephone you to arrange a date and time for your first appointment at our clinics. Specialist Diabetes Clinic – once referred to this clinic you will be offered an initial 30-minute appointment with Jill Lankester. Jill is a very experienced Diabetes Specialist Nurse who has worked for many years at the Whittington Hospital. With Jill, you will have the opportunity to discuss your diabetes in greater dept and make an action plan to improve your diabetes control. She will arrange your future appointments at the clinic. If it becomes necessary Jill will arrange for you to be referred to the Diabetes Clinic at the Whittington Hospital. Our Practice – overleaf you will find our policy on missed appointments and a map giving details on how to find us. When you arrive at the surgery, please make sure that you let the Receptionist know you have arrived.
