Please indicate on the registration form if you have a showpiece or fun routine (individual, group or team) which you would like to perform on Sunday afternoon. A limited number of gala performances will be permitted due to time constraints. REGISTRATION Registration Deadline from Provinces is MAY 5, 2010 Provinces are requested to send the team information electronically. Reminder: All forms requiring a signature can be emailed to national team pool members and potential participants prior to Regional Championships and can be ready to be collected immediately following qualification at Regionals. Registration Fee Athletes: $125.00 Coaches: $85.00 Team Managers: $85.00 Judges: $85.00 Late Registration Fee of $50.00 per participant will be applied, and will be invoiced to the province. The waiver and medical forms (attached) must also be completed for every registered participant (athletes, coaches, team managers and JUDGES.) Please ensure that all parents, supporters and others traveling with the team understand that access to the practice area is not permitted without accreditation.
