JOB DESCRIPTION TITLE: Wound and Ostomy Care Nurse JOB CODE: 154 GRADE: N9 DEPARTMENT: Clinical Operations BENEFITS: EFFECTIVE DATE:02/01/08 REVISION: 10/12/11 REVISED: 04-20-12 FLSA STATUS: EEO STATUS: PAGE: FUNCTION: Responsible for Wound, Ostomy and Continence Program development, including ensuring fiscal, quality and customer service outcomes for the wound, ostomy and continence patient population. Duties include: Community and provider marketing, staff, provider and patient education as well as overseeing clinical operations day to day, ensuring standards of care and providing consultation and expertise in the delivery of patient care. Activities include consultation as requested, assistance in case conferencing, clinical supervision, and provision of patient care. REPORTS TO: Director, Clinical Operations HOURS: 40 hours/week RELATIONSHIPS: Managers, Nurse Practitioners, PHV Staff, Physicians and clinics, patients/family members, physicians, access and care coordination center, HHA team leaders, and other departments. RESPONSIBILITIES: 1. Works collaboratively with Clinical Team Manager to select wound core team members and to ensure smooth day-to-day operations within respective team, ensure the care provided to patients meets agency and regulatory standards, quality patient care, and effective and efficient resources utilization. 2. Provides leadership to visiting staff to ensure appropriate multidisciplinary communication, collaborative working relationships, and a customer-oriented environment responsive to the needs of external/internal customers. Is easily available to assigned visiting staff for consultation and problem solving. Provides input for Wound Core Team members annual performance evaluations. 3. Coordinates and manages the use of and review of the McKesson Wound Advisor program. Monitors new WOCN referrals and consultations of new or changing wounds through the electronic functionality. Provides agency wound data and outcomes based on Wound Advisor information. Supports staff in the use of the functionality. 4. Participates in orientation of new staff, acting as a resource, supporting the preceptors, and identifying opportunities for staff development for new oriented as well as all team members. 5. Works collaboratively with the manager to facilitate the provision of quality care and case management through: home visits, co-visiting, case conferencing, chart audits, and financial analysis. 6. Performs assessment through independent home visits with an expected productivity of 4 unweighted visits per pay period for wound patients and documents all visits according to VNS standards. 7. Is responsible to establish short and long-term objectives, identifying areas of improvement for promoting both quality and cost effective patient care, and team/service evaluation. 8. Performs co-visits with PHV as needed to support ongoing competency of staff and provides CTM’s with feedback of staff competence via co-visit documentation form. 9. Conducts ongoing WOC program development, collaborating with the Director, Clinical Operations, and partners throughout Strong Health. 10. Assists in identifying new marketing opportunities, conducts community education and marketing on an ongoing basis. 11. Mentors and encourages current and future WCC staff to enrich the expertise throughout the clinical operations to benefit patient outcomes. 12. Coordinates annual VNS wound care symposium. QUALIFICATIONS: 1. NYS licensed RN, Masters Degree preferred, and WOCN Certification and experience required. 2. Community or home health experience preferred. 3. Expert in interviewing, assessment, communication, and case management skills, with current and comprehensive knowledge of community health nursing preferred. 4. Ability to mentor professional staff in case management of a patient caseload, including complex and high-risk populations. 5. Demonstrates good interpersonal and communication skills, both oral and written. 6. Demonstrates excellent teaching and mentoring skills.
