Camp Information Sheet
Due on July1st
Forms must be completed in full or you will not be allowed to board the bus - please read them carefully.
Please mail all forms and checks to the YL office before July 1st.
“Young Life Properties Health, Consent and Release” form
Please fill out this form completely with current dates for immunizations - even if you have to estimate!
Please don’t forget the insurance information (with the policy number) and parents’ signatures.
“Notice of Privacy Practices” form
Please sign and return this form.
Final Payment for camp is also due before we go to camp. The total cost of camp is $650.
Most of you have already paid the deposit of $200, so your balance of $450 is due on July 1st.
Departure: Saturday, July 16th
We will meet in the main parking lot of Novi High School (10 Mile Road Parking Lot) at 11:00pm.
Return: Sunday, July 24th
We will be returning to the Novi High School Parking Lot at approximately 9:00 am. For an updated
arrival time, please call Jody Fidrus’ cell phone voice mail (734-604-1101) after 8:00am on Sunday morning.
Suggested things to bring (One suitcase and one carry-on)
Swimsuit & towel
Toiletries (toothbrush, soap, shampoo, etc.)
Shoes you can get wet
Casual clothes (jeans, shorts, T-shirts)
Pillow for the bus (& a small blanket)
Jacket, sweatshirt, bandanas, hats
Sweatshirt for the evenings
Extra beach towel (camp provides towels, sheets, pillows)
Snacks for the bus ride (no glass bottles)
Comfortable shoes
Camera
Frisbee golf discs, wiffleball / bat, lacrosse sticks, cards…
One nice outfit (that would be a shirt, tie & khakis for guys and a dress, skirt or slacks for girls)
Grubby clothes and a pair of old shoes (not sandals or Tevas) that you can throw away
 You may bring a walkman/diskman/mp3 player or cell phone for the bus ride, however you will
have to check them all in when you arrive at the camp. The camp will keep and then return them at the
end of the week.
 Calling card if you wish to use pay phones while we’re at camp
 Extra money for meal stops and for the snack shop and camp store
Please note that no alcohol or drugs are allowed on our trip. If a camper is found with these items, they
will be sent home at the parents’ expense.

Contact Information
Campers may receive mail at camp. However, mail received after our week there will not be forwarded
home. Campers will have access to pay phones while at camp to call home.
