We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups. When We Are Closed Our out of hours service number on 0207 388 5800 This service is for emergencies only and you may be offered telephone advice, asked to attend a consultation or offered a visit by a deputising doctor depending on the urgency of the case NHS Direct In the case of urgent need when the practice is closed you can call NHS Direct (www.nhsdirect.nhs.uk/) (0845 4647) to speak to a triage nurse. Your needs will be assessed and advice offered or arrangements made for you to see a doctor. Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery re-opens. Call 999 in an emergency. Chest pains and / or shortness of breath constitute an emergency. Appointments The appointment number is - 020 7272 4323. Please call between 8.45am – 1pm or 2pm-6pm (closed from 12pm Wednesday). We offer morning and afternoon surgeries and will do our best to offer you the most convenient time and the earliest available appointment. If you cannot keep an appointment please telephone us as this frees up the appointment for someone else, patients who repeatedly miss appointments without cancelling will be removed from our list. Preference of practitioner We will endeavour to arrange your appointment with a doctor/health practitioner of your choice. However, in the interests of expediency and safe medical care, it may be necessary for your appointment to be arranged with another doctor/health practitioner. You will have the choice. Nurse Clinics The highly skilled Practice Nurse provides chronic disease management, family planning, sexual health, vaccinations and immunisations, cervical smears and other services. If you are not certain whether the service you require can be performed by our nurses, please check with the reception staff. Please be prepared to give reception staff the details of the reason for your visit as the time taken for different tasks can vary. Without such information we cannot operate an efficient system and keep waiting times to a minimum. Reception The receptionists will assist you in making emergency and routine appointments and offer help and assistance at all times. It is a legal requirement that confidentiality of patients’ records is maintained at the highest level by all staff. Their job can be very demanding – please help them to help you. Home Visits If possible please try to telephone reception before 10am if you require a home visit. A doctor or nurse may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance. House visits are only available for patients who are housebound because of illness or disability. Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the Health Centre. Access and Parking There is no car parking space available. Disabled access Whilst we make every effort to provide easy access to all members of the community, due to the restrictions of our converted premises, we regret there is no wheelchair access to the building. Patient services are provided on the ground floor and a disabled WC is provided. Repeat Prescriptions Please write your name and address on your request for a repeat prescription and either post it to us or bring it into reception, use the tear off slip whenever possible. If you provide a stamped addressed envelope we will forward your prescription to you. Alternatively you may collect it from reception but please allow at least 48 hours. Please allow extra time for weekends and bank holidays. In the interests of safety, prescriptions cannot be requested by telephone. You can also submit your request online via the form on this page. Sometimes you may be asked to come in and see your GP before a prescription can be issued. Prescriptions Charges and Exemptions Extensive exemption and remission arrangements protect those likely to have difficulty in paying charges (NHS prescription and dental charges, optical and hospital travel costs). The NHS prescription charge is a flat-rate amount which successive Governments have thought it reasonable to charge for those who can afford to pay for their medicines. Prescription prepayment certificates (PPCs) offer real savings for people who need extensive medication. NHS charges These charges apply in England only. In Northern Ireland, Scotland and Wales prescriptions are free of charge. Prescription (per item): £7.40 12-month prepayment certificate (PPC): £104.00 3-month PPC: £29.10 If you will have to pay for four or more prescription items in three months, or more than 14 items in 12 months, you may find it cheaper to buy a PPC. This clinic is attended by the Doctor, Practice Nurse. Please telephone for dates Asthma & Diabetes Clinics (weekly) These clinics are held on separate days and those patients who are known to have these disorders are regularly followed up. They are usually staffed by the Practice Nurse. Travel Clinic (weekly) This clinic is provided for travellers who require vaccinations and advice. It is attended by the Practice Nurse. Please allow six weeks, if possible, in order for us to complete the full requirements. Some procedures and certificates may require a fee to be paid by you. Test results & Nurse telephone contact These can be obtained by telephoning the surgery between 12.15am and 12.45pm. Sexual Health and Emergency Contraceptive Please contact the Practice Nurse. Confidentiality is guaranteed. Minor operations Those patients requiring small operations or injections into joints are treated by prior appointment. The Doctor will usually arrange these directly with you. Ante-Natal (weekly) This clinic is provided for all pregnant women to check the health of mother and baby at regular intervals during the pregnancy. The Practice Nurse and the Doctor usually attend the clinic. Smoking cessation and weight control (weekly) This clinic is provided to assist patients to give up smoking and to achieve a healthier weight. The clinic is attended by the Practice Nurse. Cervical smears & Post-Natal checks (weekly) The cervical smear test is available for women patients to identify pre-cancer signs. Post-Natal checks are provided for mothers who have recently had a baby, to check on their health and to provide advice and guidance. Hypertension (weekly) This clinic is provided for those patients with high blood pressure who require regular blood pressure checks and is attended by the Practice Nurse. Non-NHS/Private Work Non-NHS/Private Examinations If you require a private medical examination for employment, insurance, holiday or any other purpose this can be arranged outside normal surgery hours. Registration New Patient Registration If you wish to register, please ask our receptionist for an application form. All new patients living in the practice area are welcome regardless of age, gender, religion, social class, disability, health, race, sexual orientation or appearance. Our policy is that all patients registering with the practice must attend a routine medical health check. This helps the clinicians to familiarise themselves with your medical history.
