Inclusion criteria




Newly diagnosed type 1 DM
Newly pregnant with type 1 or type
2 diabetes
Recent hyperglycaemia secondary
to acute intercurrent illness
Recent severe and/or frequent
hypoglycaemia
Telephone (07500027385) to discuss
patients and email referral to
diabeticspecialistnurse@nbt.nhs.uk
Exclusion criteria




Under care of paediatric team
Diabetic foot problems
Acute eye problems
Suspected DKA/HHS
Refer to more appropriate clinic or
arrange admission
Receipt of the email will be confirmed
with an appointment time.
Please ensure accurate patient contact details
Please be aware patients requiring insulin that day may
need to be seen in ED or the assessment unit
