JOB DESCRIPTION FORM Job Title: Pharmacist Location: FHP/Guam Job Summary: Reports directly to the Sr. Ancillary Services Manager and responsible for dispensing drugs prescribed by physicians and other health practitioners and provides information to patients about medications and their use. Also provides advice to physicians and other health practitioners on the selection, dosages, interactions and side effects of medications. Duties and Responsibilities: 1. Dispenses drugs prescribed by physicians and other health practitioners and provides information to patients about medications and their use. 2. Provides advice to physicians and other health practitioners on the selection, dosages, interactions, and side effects of medications. 3. Monitors the health and progress of patients in response to drug therapy to ensure the safe and effective use of medication. 4. Understands the use, clinical effects, and composition of drugs, including their chemical, biological, and physical properties and performs actual mixing of ingredients to form powders, tablets, capsules, ointments, and solutions as prescribed by physician. 5. Counsels patients and answers questions about prescription drugs, including questions regarding possible side effects or interactions and among other drugs. 6. Provides information about over-the-counter drugs and makes recommendations after talking with the patient. 7. Provides advice about the patient’s diet, exercise, or stress management or about durable medical equipment and home health care supplies and helps patients manage conditions such as diabetes, asthma, smoking cessation, or high blood pressure. 8. Orders and stores medicines and ensures that all pharmaceutical products are safe, pure and effective. 9. Adheres strictly to laws that regulate the manufacture and sale of pharmaceutical products. 10. Maintains records of all pharmaceutical products being handled as per compliance with legal requirements. 11. Performs other duties that may be assigned from time to time. Job Specifications: 1. Graduate of Bachelor’s Degree with specialization in Pharmacy; License is required. 2. Minimum of 3 years experience with proven track record in dispensing drugs prescribed by physicians and other health practitioners, providing information to patients about medications and their use and providing advice to physicians and other health practitioners on the selection, dosages, interactions, and side effects of medications. 3. Must have the appropriate level of behavioral maturity and patience in handling patients. 4. Must have the diplomacy and tact in handling difficult and irate patients. 5. Very good interpersonal relationship skills and can work and relate well with co-employees, patients and customers. 6. Outstanding communication skills and persuasiveness, specially the ability to communicate instructions and procedures for treatments, keeping the patients and giving them the desired level of comfort and confidence. 7. Must have strong sensitivity to the needs and anxieties of patients and the ability to motivate patients to undergo treatments and procedures. 8. Strong ethics and a high level of personal and professional integrity. 9. Knowledgeable of federal and state laws and requirements relating to healthcare management.
