Support Plan for Jane Williams My support plan was written with help from:- Sally Williams Wendy Williams Christine Jones Trudy Smith Amy Brown my daughter my daughter my daughter my social worker my OT My name is Jane and I’m 74yrs old. I live alone in Ipswich since the death of my husband Frank. I have three daughters, Wendy who lives nearby, Christine who lives in London and Sally who lives in North Wales. I have 7 grandchildren and 4 great grandchildren. I have insulin dependent diabetes, renal impairment and diabetic retinopathy and at the start of the year I developed an unknown auto immune illness, which caused my lungs to bleed and I ended up drowning in my own blood, causing two cardiac arrests. I am one of only two people in the area to have this condition. This condition has caused tracheomalacia (massive scar tissue down my throat). When my breathing becomes bad, for some unknown reason, it affects my ability to use my legs making it impossible to climb steps etc. I use a nebuliser and my oxygen levels need to be checked several times during the day and I do this myself. If my oxygen falls below a certain level we have to phone 999. I already have a Helpline installed if I need some help. Due to the damage to my oesophagus I have difficulties with swallowing so my food has to be liquidised. I’m on a special diet of low 1 cholesterol, low potassium, low phosphate and low salt. I can walk but only for short periods and I have fallen and hurt myself in the past. I am nervous about moving around but manage holding onto furniture where I can. When I was a child I had rheumatic fever which left me with a drop foot so when my health fails I stumble lot. I always wear my helpline alarm. I don’t go outside unless I am in my wheelchair and I always need to have someone with me to push me as my breathing gets bad if I try and push myself. At the moment my daughter Wendy provides all the help I need but she is a teacher and works hard. I don’t want to feel a burden to my children or feel I rely on them. The mornings are particularly bad as my breathing every night tends to get worse. I try and prop myself up on pillows when I go to bed but find that I slip down the bed when I’m asleep. I can’t then pull myself up to sit up. Amy has visited me and suggested I have a mattress lifter which means I can just press a button and it sits me up. I think this will be a huge help for me and will mean I can do this on my own. The equipment we have at home: Wheelchair, commode next to the beds, walking frame, toilet raiser, breathing equipment and a bed stick. Amy has also suggested a ramp at the front door. The ramp will help me get in and out of my home which at the moment it feels a bit like a prison. A personal budget means I will be in control. I am an adult, I don’t want to be put to bed like a three year old and have to stay in just to please the time table of a private company. If I have control then I am the boss. As I will have a personal budget I will be able to change to another agency if the one I chose is unsuitable. Until support planning we didn’t know that we could employ a PA direct. It would be ideal if we could find someone locally who could be trained. For the moment we will try using an agency and think about employing a PA in the future. 2 I’m a respected member of the community I live in. I love my neighbours most of whom I’ve known since childhood. Friends and Family would describe me as: Very positive person Very sociable Wendy says I’m a very happy person who likes to verbally interact and jolly her along. The things I used to enjoy doing once my children were independent: Visiting local restaurants and pubs for meals with my friends Visiting friends at their home Barn dancing (but could only go one way because of my dropped foot) Visiting and staying with friends in Yarmouth (I have started knitting recently but it’s taken me 3 years just to knit a rib of a cuff) My past: I raised 3 children and am very proud of them Jobs I have done are o Assistant in a cake shop o Care worker in a children’s home o Cleaner Idea PA Someone young and strong. Although in some ways I would like a Fireman I think it would be better for me to have a female PA. Non smoker who doesn’t wear perfume Kind and understanding Punctual 3 What is Important to me? To stay living in my own home To be able to see my friends and have a change of scenery. (My daughter Christine is concerned that I’m losing my social skills and confidence) Being a respected member of the community I live in. To eat tasty meals For those around me to be non smokers and not wear perfume so as not to aggravate my chest condition as I have many allergies For me to take my medicine regularly and on time in order to manage my diabetes/health For me to be on a strict diet of low cholesterol, low potassium, low phosphate and low salt in order to manage my diabetes/health To keep my home looking nice What do I want to change/achieve? I don’t want to feel I am a burden on my family I want my friends to come around to visit me because they want to and I would like to be able to visit them. I want to be able to visit the Bingo club fortnightly to see my friends there. Otherwise I will be isolated and I need to regain my confidence and social skills. I would like to be able to go around Sainsbury’s and make choices – be a part of society I would like to be able to visit the seafront in the summer. I think the sea air helps my breathing and makes me feel good. 4 Day of Week Mon to Fri Time AM 30 min Activity I would like to get up at 8am but would need one paid person to help get me out of bed in the mornings and help with washing and dressing. (Before I needed about an hour to get up, but now I have my mattress elevator I can get out of bed myself). Wendy or my granddaughter will help me get to bed if I need it. (I don’t always need help at night as it is the mornings which are worse) Visit to my friends Every day Eve 30 min AM 30 min Weekly 2 hours 2 hours Once a week Saturday Support hours: Paid support hrs: Weekends I like to lay in at weekends. Would like to get up at about 9am. One paid person to help get me in the mornings to assist me getting washed and dressed Once I get my ramp installed I will be able to get out more in my wheelchair, with the help of one paid person Wendy will prepare my meals and freeze them for me so I always have a meal handy Wendy will do my housework for me 5.5 hrs per week How will I be supported? How will I use my PB? Outgoings SCC ABC care agency Daily rate £12.54ph x 5.5hrs pw = £68.97 x 52.14 weeks. £3,596.09 I eventually wish to have my own PA and I expect that this will reduce the cost of providing this support Contingency. My family are my first port of call should I need help and they are happy to provide this. If this should change they will contact Trudy. Totals £3,596.09 My Personal budget band is between £2,739.50 and £5,479 per year. My family provide a great deal of support and are willing to continue to do so. However should my needs or circumstances change then I shall need a review. 5 The actual cost of my support plan is £3,596.09 for the coming year. This will leave me £1,883 available as contingency, should my family not be able to continue supporting me as much. This will be spent Contingency Outgoings SCC Meal preparation/housework/help getting to bed by a carer £12.54 this gives me a contingency of 150 hours per year or 2-3 hours per week for any of these tasks should this be necessary. Totals £1,883 How will my support be managed? My daughter Wendy will support me with making the payments to the agency and Suffolk Independent Living will help me with finding a P.A. Amy will bring the ramp round and Trudy will keep in touch with Wendy so Wendy can let her know if the family need more help. How will I stay in control? I am able to make my own decisions.
