* For all new applicants, half of the site survey and consultative visit fee must be paid by July 1 of
the year prior to your anticipated survey/consultative visit date and should accompany the letter of
intent. (See Policy No. AC-130 Process for Becoming a New Trauma Center)
** A hospital may elect to have additional site surveyors join the consultative team for an
additional cost. PTSF is waiving fees for Critical Access Hospitals undergoing consultative visits
due to grant funding being provided by the Federal Flex Grant Program. Please contact PTSF
staff for details.
