Sex of mice supplied: male / female / both Genotype of mice supplied: WT / heterozygous / homozygous Strain of wild type mouse for mating (if applicable): .................................................................... DECLARATION OF UNDERSTANDING AND COMPLIANCE I, the undersigned agree to and have read the following items of information: Heterozygous/Homozygous females can be supplied by Client at the appropriate age for strain. Age of females is preferably 8-16 weeks. Wild type females can be supplied by TASQ at an additional cost. Males used as studs must be of proven fertility. If no proven males are available, TASQ can test fertility at additional cost to the researcher. As there are strain variations in response to superovulation, more than 20 females may be necessary to produce the required embryo numbers for storage. Therefore more than 20 females may be needed to cryopreserve the line. Additional charges will apply per 20 females used for embryo freezing. TASQ stores embryos in 2 separate tanks. Refer to the TASQ website for details regarding the number of embryos to be stored http://tasq.uq.edu.au/embryo-cryopreservation. TASQ will supply a progress report on each strain’s success regarding the number of embryos collected after the first round of cryopreservation. Once the required numbers are reached, TASQ will notify client and await instructions on the fate of the mice held at TASQ. Health report must be sent to UQBR prior to the shipping of mice. Client is responsible for shipment costs to TASQ. Client has obtained an Animal Ethics approval and expiry date. Certificate must be attached to this registration. Have obtained OGTR approval and quoted this number. A test thaw and rederive is conducted to test the viability of the strain for freezing. The thaw results will be relayed to the client. When UQBR covers the costs of cryopreservation, there is a cost of $260/strain to the researcher for the test thaw. This involves the thaw and culture of the embryos and then embryo transfer into a pseudopregnant mother. The thaw results will be relayed to the client. This fee is compulsory. The generation of this genetically modified mouse strain will not infringe any intellectual property rights. I have checked the TASQ website at http://tasq.uq.edu.au and agree to the charges listed for this service. Signature of investigator: ................................................................
