People who should have a flu jab

The injected flu vaccine is offered free of charge on the NHS to people who are at risk to ensure that they are protected against catching flu and developing serious complications.

You are eligible to receive a free flu jab if you:

 are 65 years of age or over

 are pregnant have certain medical conditions

 are living in a long-stay residential care home or other long-stay care facility

 receive a carer's allowance, or you are the main carer for an elderly or disabled person

 whose welfare may be at risk if you fall ill are a healthcare worker with direct patient contact or a social care worker

Pregnant women and the flu jab

If you're pregnant, you're advised to have the injectable flu vaccine, regardless of the stage of pregnancy you've reached.

That's because there's strong evidence to suggest that pregnant women have an increased risk of developing complications if they get flu.

If you're pregnant, you will benefit from the flu vaccine because it:

 reduces your chance of getting serious complications of flu, such as pneumonia, particularly in the later stages of pregnancy

 reduces your risk of having a miscarriage

 prematurely or with a low birthweight, due to flu will help protect your baby because they will continue to have some immunity to flu for the first few months of their life

It's safe to have the flu vaccine at any stage of pregnancy, from conception onwards. The vaccine doesn't carry any risks for you or your baby. Talk to your GP or midwife if you are unsure about the vaccination.

If you call with a non urgent problem and are unable to speak to your doctor, please leave a message and a member of the medical team will return your call.

Home Visits

If possible, please try to telephone reception on 01732 883161 between 8.00 and 10.00am if you require a home visit.

A doctor or nurse may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.

House visits are only available for patients who are housebound because of illness or disability.

Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the Health

Centre.

What is happening?

Please be aware that a new on-line service for patients will be operating from 1 st

March 2014. This service will need to be used for all future ordering of online prescriptions (the old system will no longer work) and additionally will enable patients to book normal appointments with their usual doctor on-line. This is a new facility that the practice is operating to meet the requirements of patients as expressed in a recent survey.

What do you need to do?

In order to use this, each individual patient, must fill in a form (available from the surgery) providing his or her email address. This will activate a registration process that will require each individual patient to come back into the main surgery when they have been contacted by reception, with formal photo identification, they will then be given initial log-in details, enabling them to register with the system within 30days (it is then disabled and the process needs to restart). Please do not come in to collect your initial log-in information until you have received a telephone call from reception to say that it is ready for collection.

Can I order repeat prescriptions for my children?

A parent can register a child without photo id, but a request for repeat medication or appointments can only be made through each individual child’s log-in.

Can I stay on the old system?

No.

This will all need to be completed to order online repeat medication from 1 st

March 2014, the old service will no longer operate.

Prescriptions

We seem to be receiving an increasing number of telephone calls enquiring whether prescriptions are ready for collection. This is taking up valuable time and slowing down our process of dispensing. Please allow 48 working hours to prepare your prescription (remember we are closed sat and sun). This will avoid unnecessary telephone calls to the dispensary. Thank you.

Prescriptions can be dispensed to those patients on our dispensing list. Patients who live more than a mile from a chemist and are also south of the M20 may apply to be on this list.

These patients may also take their prescriptions to an outside chemist if they wish. Please ask at the dispensary if you would like to be on the surgery dispensing list.

Repeat Prescriptions

Patients on long term treatment may be given computerised repeat prescriptions. We cannot accept requests for repeat prescriptions over the telephone. Your doctor will still want to see you at agreed intervals to review your treatment and health. A printed prescription will be ready for you at the surgery two working days after we receive your request. You may choose to have your prescription sent to the chemist in Borough Green or Vigo village if you wish, but please allow

3/4 days for this service.

For those on our dispensing list the dispensed prescription may be collected at the surgery, or there is a delivery of medicines to the Post Office in Plaxtol or the Newsagents in Wrotham three times a week. Only patients who are exempt from prescription charges may use the Wrotham delivery.

Prescriptions Charges and Exemptions

Extensive exemption and remission arrangements protect those likely to have difficulty in paying charges (NHS prescription and dental charges, optical and hospital travel costs).

The NHS prescription charge is a flat-rate amount which successive Governments have thought it reasonable to charge for those who can afford to pay for their medicines. Prescription prepayment certificates (PPCs) offer real savings for people who need extensive medication.

NHS charges

From 1 April 2011, the charges are:

 Prescription (per item): £7.65

 12-month prepayment certificate (PPC): £104.00

 3-month PPC: £29.10

If you will have to pay for four or more prescription items in three months, or more than 14 items in 12 months, you may find it cheaper to buy a PPC.

Clinics & Services

NHS Services

The following services are available and details may be obtained from your doctor or from reception:

New registrations

All patients are encouraged to have a basic health check and consultation within a month of registration. Please make an appointment with the nurse.

Well-woman and well-man clinics.

These are conducted by the nurse. Anyone from 16 to 75 years of age may attend at 3-yearly intervals for a routine check and appropriate advice. All patients over 75 years of age are eligible for an annual health check.

Cervical smears.

These may be done by the nurse or your own doctor.

Antenatal clinics and Postnatal care.

These are done by all the doctors who share the care with the midwife, and with the local hospital maternity departments. The midwife holds classes for mothers and their partners to attend in the final weeks of their pregnancy.

Postnatal care will be carried out by the midwife for a minimum of ten days.

Thereafter the health visitor will provide home visiting and is available for continuing advice and support at the child health clinic. She arranges small groups of new mums for mutual support and also clinics for helping with babies’ sleep problems. The health visitor can be contacted during office hours on 01732 882754.

Child development.

The health visitors and doctors offer clinics to monitor children’s progress and to carry out the routine immunisations which are of such importance. Parents are reminded about these by post, as well as personally.

Contraceptive advice.

All doctors offer contraceptive advice in routine surgeries.

Diabetic checks.

These are carried out regularly by doctors & nurses for diabetic patients.

The nurse in charge of diabetic care is also available for advice by appointment.

Cardiovascular checks

These are carried out by the appropriate nurse by appointment

Asthma clinics

These are carried out by the nurse in charge of asthma care, to achieve optimum treatment for everyone with asthma.

Travel clinics.

These are run for those going abroad to ensure that they receive all recommended preventative advice and inoculations. You should contact us at least two months before travelling

Anticoagulant clinic.

This is run by a doctor to control the regime of those patients on Warfarin.

Blood Tests

Please note that we now have appointments available at the surgery every day between

8am and 12am. This extended service means that patients are no longer allowed to use the

Sevenoaks hospital service. Please be aware that you may be turned away at the hospital from the beginning of November.

Confidentiality

The practice has a strict policy regarding confidentiality and data protection. We will only release test results to the person to whom they relate unless that person has given prior permission for the release of this data or they are not capable of understanding the results.

Non-NHS Services

Activities Outside the NHS

Some of our work is not included in the Health Service. This includes medical examinations for health insurance, HGV licences, sports, work and school; private certificates; travel drugs and inoculations. Fees for all these are charged in line with those suggested by the British Medical

Association.

From the 1 st November 2103 our registration process is as follows:-

1.

If you would like to register with the practice, you will need to complete a Registration

Form. You can pick one up from Reception at the Surgery or complete an on line preregistration form - YOU NEED TO COME IN TO SURGERY WITH ALL

DOCUMENTS TO COMPLETE REGISTRATION PROCESS

2.

You also need to complete a New Patient Questionnaire, again either from reception or on line.

3.

You need to bring all these completed forms to Reception along with 1 form of photo ID

(or birth certificate for a child) per person –and one proof of address per family (current bill, tenancy etc).

4.

Only when this process has been completed we will be able to register you as a permanent patient if you live within our practice area.

5.

On joining the practice list you will be allocated a named doctor. We would encourage you to see this Doctor wherever possible, as we believe that continuity of care is helpful to both you and your Doctor

6.

If you wish to subsequently change to another Doctor, this is normally possible on discussion with the practice manager.

7.

If you live more than a mile from the nearest pharmacy we may be able to dispense your medications to you from our dispensary.

8.

If you need to register a child under 6 a pink child health registration form must be filled out at reception, and a record of the child’s immunisations must be handed in for each

child, which will be recorded in the red book. Once these records have been handed in the child can be registered with the practice.

9.

A baby/child under 6 who has come from abroad, should be booked in for a nurse appt to go through their immunisation record, before they can be registered permanently.

10.

If a patient needs an appt straight away, they must come down to the surgery at least 30 mins prior to the appt so there forms etc can be checked and enough time must be left so the registration process can be completed. If under exceptional circumstances there is no one available to register, the documentation should be checked, and the patient put on as a

Temporary Resident.

Confidentiality

Confidentiality

The practice complies with Data Protection and Access to Medical Records legislation.

Identifiable information about you will be shared with others in the following circumstances:

 To provide further medical treatment for you e.g. from district nurses and hospital services.

 To help you get other services e.g. from the social work department. This requires your

 consent.

When we have a duty to others e.g. in child protection cases Anonymised patient information will also be used at local and national level to help the Health Board and

Government plan services e.g. for diabetic care.

If you do not wish anonymous information about you to be used in such a way, please let us know.

Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.

The practice considers aggressive behaviour to be any personal, abusive and/or aggressive comments, cursing and/or swearing, physical contact and/or aggressive gestures.

The practice will request the removal of any patient from the practice list who is aggressive or abusive towards a doctor, member of staff, other patient, or who damages property.

All instances of actual physical abuse on any doctor or member of staff, by a patient or their relatives will be reported to the police as an assault.

Medical Records

How we use your information - Protecting Privacy

The health information you share with us is kept in your medical record, a life long history of your health, which helps us with your future care. Your relationship with your doctor and other health workers is based on mutual trust and confidence and we do all we can to protect that trust.

Patient confidentiality underpins the structure of the NHS and is a priority for us. Dedicated teams of professionals make up the NHS and we will normally share your information only with others directly involved in your medical care.

We aim to keep your health record confidential and we will continue to improve this. We hope you feel confident that your information is safe. The NHS uses new technology to help deliver better patient care - health records can now be securely stored and shared electronically. Some

NHS organisations have developed an Electronic Health Record - a way of storing your medical information electronically.

This allows doctors and other health professionals to access your up to date information, whether it is from your GP practice or the hospital. This will enable doctors to make more effective decisions about your treatment and care. Some of the information contained in your health record might be used for reasons other than your own personal healthcare:

In the teaching and training of healthcare professionals.

As part of the funding arrangements for local health services.

 To audit processes and practices.

To help plan future health services.

In health research programmes. You will be specifically asked to consent to any research project in which you participate directly.

 To help plan your ongoing care with other agencies (e.g. social services)

The NHS is also required to have processes in place to manage patient information (e.g. keeping and updating records) which will include your medical information. These reasons are vital to the running of the NHS. Wherever possible, the shared information will be anonymous but if you have concerns about this, you can choose not to allow the information to be used.

Who has access?

Doctors, nurses and other health professionals need access to your records. However, this is on a need-to-know basis and includes only those directly involved in your care. If you are referred to hospital, your health/medical information will need to be transferred from your GP to the hospital, and back again. Secretaries, receptionists and other clerical staff will need limited access in order to carry out administrative tasks such as typing letters and booking appointments.

All staff employed by the NHS have a duty of confidence to ensure that your information is not disclosed inappropriately, and they work to a Code of Conduct for handling personal information.

Other uses for your information

The NHS must, by law, notify the government of certain infectious diseases for public health purposes, e.g. measles, mumps, meningitis, etc. but not HIV/AIDS.

Births and deaths must also be notified.

A Court of Law can insist that medical information be disclosed to them.

Limited anonymised information is also shared with Primary Care Trusts to assist with the organisation of national public health programmes, e.g. breast screening, cervical smear tests, and childhood immunisations. Your personal information will not be passed on to any organisation for monetary gain.

Can I see my medical records?

Yes. Requests should be in writing to your GP. The request must responded to within 40 days and a charge may be made. If you feel anything has been added to the record that is factually incorrect, you have the right to apply to have it amended or deleted in accordance with the guidance in the 1998 Data Protection Act.

Can my records be withheld?

We will withhold any information contained in your records likely to cause harm to your mental or physical state, or that of other people. A senior clinician will make this decision. Any third party information would not be revealed without their consent.

Research

As a Royal College of General Practitioners accredited research Practice we may invite you to take part in a research study. We will only use your information with your explicit consent. If you have been identified for possible inclusion in a research project you will be asked to complete a consent form. If you do not wish to take part, your information will be securely held and no identifiable information will leave the Practice. All reports and results will be anonymous.

Friends of the Surgery

This support group was inaugurated by the patients in 1994. Its aim is to support the practice and to raise funds for the purchase of equipment and amenities for the benefit of patients which the practice could not normally provide. It has proved very successful.

All patients are welcome to join.

There are newsletters and general meetings which they are welcome to attend. The Friends’ notice board in Borough Green Surgery shows news and events of interest to the Friends. Please ask for an application form at reception if you would like to join.

Depression Questionnaire

This easy to use patient questionnaire has been validated for use in Primary Care.

It is used by your doctor to monitor the severity of depression and response to treatment.

It can also be used to make a tentative diagnosis of depression.

Sickness Certificates

You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website (www.hmrc.gov.uk/forms/sc2.pdf) .

Evidence that you are sick

If you are sick for more than seven days, your employer can ask you to give them some form of medical evidence to support payment of SSP (statutory sick pay).

It is up to your employer to decide whether you are incapable of work. A medical certificate, now called a 'Statement of Fitness for Work’ (see below) from your doctor is strong evidence that you are sick and would normally be accepted, unless there is evidence to prove otherwise.

You could also provide evidence from someone who is not a medical practitioner, e.g. a dentist.

Your employer will decide whether or not this evidence is acceptable. If your employer has any doubts, they may still ask for a medical certificate from your GP.

Statement of Fitness for Work - ’Fit Note'

The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help you return to work sooner by providing more information about the effects of your illness or injury.

Summary Care Record

There is a new Central NHS Computer System called the Summary Care Record (SCR). The

Summary Care Record is meant to help emergency doctors and nurses help you when you contact them when the surgery is closed. Initially, it will contain just your medications and allergies.

Later on as the central NHS computer system develops, (known as the ‘Summary Care Record’ –

SCR), other staff who work in the NHS will be able to access it along with information from hospitals, out of hours services, and specialists letters that may be added as well.

Your information will be extracted from practices such as ours and held on central NHS databases.

As with all new systems there are pros and cons to think about. When you speak to an emergency doctor you might overlook something that is important and if they have access to your medical record it might avoid mistakes or problems, although even then, you should be asked to give your consent each time a member of NHS Staff wishes to access your record, unless you are medically unable to do so.

On the other hand, you may have strong views about sharing your personal information and wish to keep your information at the level of this practice. Connecting for Health (CfH), the government agency responsible for the Summary Care Record have agreed with doctors’ leaders that new patients registering with this practice should be able to decide whether or not their information is uploaded to the Central NHS Computer System.

For existing patients it is different in that it is assumed that you want your record uploaded to the Central NHS Computer System unless you actively opt out.
