Clinical duties will include the management and investigation of patients in the Respiratory Unit with one or two days per week spent in out patient consultation seeing new patients and returning patients at clinics supervised by a Consultant. Training will be provided in, e.g., intercostal drains and pleural biopsies. Full shift pattern is worked with the post-holder working in the Gartnavel Hospital at Night team. Both posts are available until 5th August 2014, one post is available from as soon as possible and the other post is available from 5 th February 2014. These posts hold educational approval from the Postgraduate Dean. If appointed as a Clinical Fellow this post will not be recognised for Training. To practice medicine in the UK you need to hold both GMC registration and a license to practice. This posts will be based within Gartnavel General Hospital, part of the West Glasgow Hospitals campus.The Western Infirmary (545 beds) and Gartnavel General Hospital (564 beds) are separated by a distance of a mile and a half but function as a major teaching hospital complex in the Western District of Glasgow. The complex is one of the main teaching hospitals for the University of Glasgow and houses University Departments of Medicine, Surgery, Dermatology, Gynaecology, Pathology, Biochemistry, Ophthalmology, Orthopaedics, Immunology and the Renal Transplant Service. THE WORK OF EMERGENCY CARE AND MEDICAL SPECIALTIES Medical Unit The Medical Department, present at both Gartnavel Hospital and the Western Infirmary, covers all aspects of investigation and treatment of acute and non-acute medical patients with corresponding outpatient clinics. Acute medical receiving is undertaken on a rotational basis between teams based at the Western Infirmary and Gartnavel General Hospital. However, all acute medicine admissions are initially to a 30 bedded Acute Medical Admissions Unit at the Western Infirmary. Patients are then transferred to a general medical ward. The Medical Department also provides a range of specialist services including Diabetes, Endocrinology, Rheumatology, Infectious Diseases, a Stroke Unit, an Epilepsy Unit and Blood Pressure Clinics. Respiratory Unit The respiratory unit provides a comprehensive service for the diagnosis and management of all types of respiratory disease. The total population served is over 265,000. Patients are also referred for specialised respiratory investigation and treatment from other Health Boards. The inpatient sleep service is located within the respiratory unit on the Gartnavel site. The unit receives all types of respiratory cases continuously. There are approx. 2,000 discharges per year. In addition to all the techniques and skills expected in any teaching hospital respiratory unit, we have special expertise in the following areas: Sleep related disorders of breathing [Dr C Carlin]: A service to investigate and treat patients with disorders of breathing during sleep was started approximately 10 years ago and the Unit has seen a steady rise in referrals. Cystic Fibrosis (West of Scotland Adult Cystic Fibrosis Centre) [Dr Bicknell, Dr Ross & Dr MacGregor]: Numbers currently attending the Centre – 170 patients. Difficult Asthma [Dr N Lee & Dr M Shepherd]: The unit has particular experience in the investigation and management of cases of difficult asthma. Pulmonary Vascular Disease (Scottish Pulmonary Vascular Unit) [Professor Peacock, Dr M Johnson & Dr C Church]: Patients with pulmonary hypertension are referred from all over Scotland. Endobronchial therapy of lung cancer (laser, stenting, TBNA) [Dr Bicknell]. Thoracoscopic Biopsy Techniques [Dr S Bicknell] The unit is responsible for all aspects of the care of acute and non-acute patients referred to the respiratory outpatient department. Over 1,500 new patients and over 9,000 return patients are seen each year. Respiratory Clinics are held at Gartnavel General Hospital. There are specialised clinics dealing with specific areas: Cystic fibrosis, Difficult asthma, Pulmonary vascular disorders (held at Golden Jubilee National Hospital), Sleep related disorders, TB, Lung cancer, ILD. The unit has expertise in the immunotherapy of allergic diseases such as bee or wasp venom reactions. Respiratory Liaison Service: TB nurse specialist (n=1): undertakes TB contact tracing, BCG vaccination, supervision of DOTs and domiciliary assessment. There are between 40 to 45 cases of pulmonary tuberculosis notified each year from the West Glasgow Area. Respiratory liaison nurses (n=3): run the Asthma & COPD service including a supported discharge service and domiciliary service for patients with severe COPD (mainly on LTOT). Lung cancer liaison nurse (n=1): provides a lung cancer support service. Over 200 patients with confirmed lung cancer are treated annually and histological diagnosis obtained in 89% of patients. Cystic fibrosis nurse specialists (n=3): provide a service to the West of Scotland Adult Cystic Fibrosis Centre. Pulmonary Vascular nurse specialists (n=2): provide a service to the Scottish Pulmonary Vascular Unit. Four routine bronchoscopy sessions are undertaken each week with emergencies being accommodated at other times. The unit also performs medical thoracoscopy with facilities for up to two cases per week. The unit provides a specialised lung function service with laboratories situated at the Western Infirmary and Gartnavel General Hospital. One-two days per week will be spent in out patient consultation seeing new patients and returning patients at clinics supervised by a Consultant. Training will be provided in intercostal drains and pleural biopsies. All junior staff are encouraged to perform an audit during their time in the department. Full shift pattern is worked with the post-holder working in the Gartnavel Hospital at Night team. In addition to the above duties the appointee will be liable for duty in occasional emergencies and unforeseen circumstances which might occur. Teaching All units provide weekly postgraduate medical clinical meetings for their staff. In addition, there is a weekly hospital meeting – held in the postgraduate centre. In addition there is a training programme of bedside clinical teaching geared towards the needs of trainees aiming to sit the PACES examination. The postholder will participate in bedside, outpatient and small group teaching for 3rd and final year undergraduates and postgraduates. There will also be lectures to undergraduates and presentations at clinical meetings. The postholder shares the responsibility of supervising FY1 doctors with his/her colleagues. Research and audit The postholder will be expected to actively contribute to audit and contribution to the unit’s research interests will be encouraged. Administration The postholder will assist in the planning of student teaching and examinations, and organising duty rotas, and may also be assigned duties by consultant staff. Location of Duties The post is based at Gartnavel General Hospital, but also requires duties to be undertaken in the Western Infirmary. In addition to the above duties, the appointee will be liable for duty in occasional emergencies and unforeseen circumstances that might occur. Study & Training The post is educationally approved for medicine. Study leave within the United Kingdom will be in accordance with the terms and conditions of service, and applications should be made to the Clinical Director. Library Facilities There are good Library facilities available in Gartnavel General Hospital and the Western Infirmary. Facilities are available for photocopying and texts of journals can be obtained from other libraries. The main University Library is within easy walking distance of the Western Infirmary. Postgraduate Medical Education Courses available in the locality Numerous postgraduate courses are organised jointly by the University of Glasgow and the Royal College of Physicians and Surgeons of Glasgow, many of them on a half-day release basis. Details of these courses are available in the main Medical Library, from the College, or from the Medical Faculty Office at Glasgow University. During the academic year there are weekly clinical meetings for the entire hospital within the Western Infirmary and Gartnavel General Hospital. Medical Negligence In terms of NHS Circular 1989 (PCS) 32 dealing with Medical Negligence, the Division does not require you to subscribe to a Medical Defence Organisation. Division indemnity will cover only Division responsibilities. It may, however, be in your interests to subscribe to a defence body in order to ensure you are covered for any work which does not fall within the scope of the indemnity scheme. Other Facilities (clubs, car-parking, shops, transport, etc) Car Parking for those on call is available within the hospital grounds at both Gartnavel Hospital and the Western Infirmary. The Western Infirmary is immediately adjacent to excellent shopping facilities in Byres Road and to bus and underground transport. TERMS AND CONDITIONS OF SERVICE The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council. TYPE OF CONTRACT Fixed Term GRADE AND SALARY - If appointed as an StR (Locum Appointment for Training) the salary range will be: £30,002 to £47,175 (pro rota) - If appointed as a Clinical Fellow the salary range will be: £30.002 to £39,693 (pro rota) New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer. HOURS OF DUTY SUPERANNUATION Full Time 40.00 New entrants to NHS Greater Glasgow and Clyde who are aged sixteen but under seventy five will be enrolled automatically into membership of the NHS Pension Scheme.
