Consultant in Emergency Medicine
BASE:
Victoria Hospital, Kirkcaldy
EMERGENCY MEDICINE, VICTORIA HOSPITAL, KIRKCALDY
Acute Care in Fife was centralised in January 2012, with the opening of the new phase of the
Victoria Hospital, Kirkcaldy. A new state of the art Emergency Department opened at this time and
saw over 60,000 patients in the first year of operation.
The new Department includes an 8 bay resuscitation area, a 9 bay majors area, a 6 bay minors
area and 2 paediatric rooms. There is a dedicated Procedures room and a 6 bedded Emergency
Medicine Observation Unit. There is access to electronic pathology and radiology ordering.
Departmental guidelines, protocols and rotas are on the Departmental intranet web pages.
The Emergency Department has 24 hour senior doctor presence, with current shop floor
Consultant presence daily from 0800 to 2300. The advertised Consultant posts will help ensure
maintenance of this direct shop floor presence and leadership.
There is a co-located GP Out of Hours service in the evenings and at weekends, allowing
redirection of appropriate patients.

Psychiatry Interview Room

A secure interview room with access to waiting area and emergency Department
Emergency Medicine Observation Unit (EMOU)

6 bed observation unit solely for admission of patients under the care of the on-call emergency
physician. This includes head injury observation, toxicology and post sedation observation and
other conditions at the discretion of the on-call Consultant. This clinical area is never used for
the accommodation of patients awaiting admission under other specialties and there are strict
admission criteria.
In addition, there is Consultant office space, senior nursing and doctors’ offices, secretarial
accommodation and a relatives’ room.
In-Patient Specialty Support On site
On site support includes Anaesthetics and ICU, General Medicine (with Medical HDU), Cardiology
and CCU, General Surgery (with Surgical HDU), Trauma and Orthopaedics, Maxillofacial Surgery,
Paediatrics and Obstetrics & Gynaecology.
ENT is on site Monday 0800 to Friday 1700.
Emergency radiology is provided in a dedicated emergency diagnostic suite directly adjacent to the
Department. There is 24 hour access to CT scanning. MRI is available on site within office hours.
Emergency laboratory investigations are available 24 hours per day.
There is a stroke thrombolysis service which operates 24/7 either through on-site stroke or
emergency physicians or telemedicine support from the regional hub.
Minor Injuries Unit, Queen Margaret Hospital (QMH), Dunfermline
A 24/7 Emergency Nurse Practitioner minor injuries/illness unit runs in the former Emergency
Department at the Queen Margaret Hospital, Dunfermline. Telephone support is provided by the
Consultant and middle grade staff at VHK.
POST DESCRIPTION
Posts of Consultant in Emergency Medicine, Fife Acute Services Division
The post holders will assist in the provision of a high quality emergency care in Fife. The posts
currently attract 10 Programmed Activities. Special interests or part-time working will be
considered and there is scope for candidates to pursue other areas of interest.
Definition
Whole time Consultants in Emergency Medicine based at Victoria Hospital, Kirkcaldy. These posts
entail responsibility for the running of the Emergency Medicine service within Fife in conjunction
with the current Consultants.
Staffing
Funding is available for 13 WTE Consultants. The Department has recently undergone an
extensive recruitment drive resulting in a number of middle grade appointments, with appointments
to Junior and Senior Clinical Fellow grades. A third round of Junior Clinical Fellow posts has been
advertised with the successful appointment of a further excellent candidate, in addition to the 4.7
listed below. 4 GPs have also been appointed for a number of sessions per week to help cover
evenings and weekends, as detailed below.

Sessions in A&E. Each works 1 shift per week (1400-2200) and 1
weekend per month (1000-2200). All intend to work here long term,
and all but 1 are ex-trainees from NHS Fife ED.

ENPs
An Emergency Nurse Practitioner (ENP) service started 6.1.14 for 12
hours per day, 5 days a week. The short term aim is to expand this to
2 periods of 12 hours per day, covering the 0800-0000 period.
DUTIES AND RESPONSIBILITIES
The main duties and responsibilities for the posts include the following:
Clinical Duties
The Consultants will be responsible for the reception, resuscitation, diagnosis and initial treatment
of all emergencies. They will make arrangements for patients to be admitted into hospital beds,
special units or other hospitals, to outpatients or back to the community.
The appointees will be expected to participate actively in the clinical workload of the Department,
undertaking direct management and assessment of patients. During these Departmental sessions,
the post holders will need to be aware of the junior doctors’ workload and to take action to ensure
that excess hours are not worked.
During these Departmental sessions, the post holders will be expected to maintain an overview of
Departmental activity so that any threat to achieving the 4 hour target can be identified early and
dealt with. Duty Consultants hold the responsibility for liaising with other teams so that the process
of care is safe, smooth and does not involve unnecessary delays. Consultant emergency
physicians hold ‘admission rights’ to both acute medical and surgical admission units.
The Emergency Medicine Observation Unit is a Consultant delivered service, caring mainly for
head injuries and overdoses.
There is a Consultants’ meeting held on the a monthly basis, and a senior staff meeting also
involving senior nurses, ENPs and a middle grade representative (usually the most senior trainee).
The post holders will be expected to attend and contribute to these meetings whenever possible.

Lead for Emergency Care annually.

The job plan based on a standard 10 PA contract would have a DCC:SPA split of 9:1. However,
as the role of Consultant in Emergency Medicine involves considerable time for workforce planning
and service development, in addition to all aspects of clinical and educational supervision and
governance, it is expected that the final DCC:SPA split will be 8:2.
The on call frequency will vary depending upon the number of appointees, but will be a maximum
of a 1:8. The frequency of weekends worked will be a maximum of 1:5, although this will fall to 1:6
from January 2014. See sample 12 week rota on page 10.
In the event of any significant changes in the circumstances affecting this job plan, it is agreed that
it will be renegotiated jointly in collaboration with Consultant colleagues in Emergency Medicine.
Cover for Consultant Colleagues
Annual / Study Leave – You will be required to provide emergency cover for Consultant colleague
during his absence on annual or study leave. If for any reason such deputising is not practicable,
Fife Acute Services Division undertakes to authorise immediate Consultant locum cover.
Teaching
Clinical supervision of trainees has a high profile in the Department and is an important part of the
Consultant role.
There is an ongoing programme of daily and weekly Departmental teaching sessions for the junior
doctors (FY2 & GPST) in which the post holders will be expected to participate. STs attend weekly
afternoon teaching sessions at their base hospital (Edinburgh Royal Infirmary) and Fife
Consultants are expected to contribute to this programme. One to one teaching for middle grades,
including assessment of their management and non-technical skills, has commenced.
Medical students from St Andrews University attend afternoon sessions on a Thursday for 2
semesters per year. The Department also takes students from other Medical Schools for elective
placements.
NHS Fife has an adult simulator which can be utilised for scenario based teaching and this is kept
within the Emergency Department.
Continuing Medical Education
The Board supports and will require the successful candidate to participate in continuing medical
education (CME). You are entitled to 30 days paid study leave within any 3-year period, with
expenses, for the purposes of CME.
Clinical Research
Interest in research is welcomed and would be supported by Consultant colleagues. The
Department has been involved in recent multi-centre trials.
Clinical Governance
NHS Fife is committed to maintaining a high quality of services to patients, with particular regard to
patient safety, by continual development of practice in the light of research evidence and by audit
based against relevant standards. There is a designated Audit Lead and active audit programme
which includes participation in CEM audits.

FIFE REGION
The Region of Fife is bounded in the north by the Firth of Tay, in the east by the North Sea and in
the south by the Firth of Forth. The Region spans an area of 130,700 hectares and has a
population of 360,000. The population served by NHS Fife is currently around 280,000. There is a
highly developed agricultural area in east and north-east Fife, and in the west there is an extensive
cross-section of highly skilled and scientifically orientated industry. The largest towns are
Dunfermline, Kirkcaldy and Glenrothes. The cathedral city of St Andrews is the seat of Scotland’s
oldest and the UK’s second oldest university.
Fife is an area of considerable scenic and historical interest. The usual range of sporting facilities

motorway links to the rest of the UK. Edinburgh & Glasgow Airports are within easy reach by road.
