SCOPE: All Company-affiliated facilities including hospitals, ambulatory surgery centers, outpatient imaging centers, medical office buildings, home health agencies, physician practices, and all Corporate Departments, Groups and Divisions and on-site subcontractors.

PURPOSE: To describe the policy framework for environmental compliance and identify Corporate resources available to assist with implementation.

POLICY:

1.

All employees, subcontractors, and vendors must carry out their roles and responsibilities in a manner that is protective of human health and the environment. Concerns about or violations of this policy should be reported to your supervisor or via the Ethics Line at 1-800-455-1996.

2.

The environmental management program demonstrates the Company’s commitment to:

 adhere to applicable federal, state, and local environmental laws, regulations, and requirements;

 operate in a manner that protects human health and the environment and integrating pollution prevention into planning and decision-making;

 objectively and fully communicate environmental protection information to staff, subcontractor personnel, our customers, applicable stakeholders, and the public; and

 continuously improve our processes, systems, and capabilities so that we can increase the value of our services.

3.

The Corporate Engineering and Facility Management Manager is responsible for development and implementation of the environmental management program including:

 development of environmental management program and related policies; and

 establishment of the framework to measure, evaluate, and correct environmental management program performance.

Each Company-affiliated facility must have a designated Responsible Manager appointed by the CEO or Administrator. The Responsible Manager is responsible for facility compliance with federal, state, and local environmental statutes, regulations, codes, and requirements, as well as daily operational responsibility for implementation of the environmental program.

PROCEDURE:

1.

The Corporate Engineering and Facility Management Department will provide environmental program support to Company-affiliated facilities and will coordinate communication between the facilities and environmental program resources.

2.

The Responsible Manager is responsible for developing and implementing facility environmental compliance procedures. The facility environmental procedures must be consistent with the corporate environmental policies and should be attached to the appropriate corporate environmental policy. The facility environmental procedures should include any additional state or local procedures or requirements.

3.

The Responsible Manager must report environmental situations, problems, or incidents to the

Corporate Engineering and Facility Management Department within 24 hours of occurrence. The

Responsible Manager must also report environmental occurrences to the appropriate agencies within the time limits imposed by regulation.

4.

Each facility periodically (or when requested) must conduct an environmental self-audit in accordance with the requirements of the Environmental Self-audit of Facilities Policy, ENV.018.

The Responsible Manager must provide the completed self-audit documents, written corrective action plan and the scheduled completion date for each corrective action to the Corporate

Engineering and Facility Management Manager.
