Continued NHS TAYSIDE CONSULTANT IN SPECIAL CARE DENTISTRY DUNDEE DENTAL HOSPITAL INTEGRATED DENTAL SERVICES NHS Tayside has a vacancy for a full time consultant in Special Care Dentistry based at the Dental Hospital. This post is to provide the consultant level leadership in the delivery of the Special Care and Dental Anxiety clinical service and to the support of the clinical training of dental undergraduates. The post is for 10 sessions per week, initially a 9:1 ratio of direct patient care to supporting professional activities. The job plan will be modified at times to support the undergraduate teaching programme. The successful applicant will be able to show evidence of experience in all aspects of special care dentistry and be proficient in the delivery of conscious sedation. Continued Consultant in Special Care Dentistry NHS Tayside Integrated Dental Services Dundee Dental Hospital and School The Post This consultant post is ACTD funded and was created to provide an appropriate clinical lead to develop and deliver the undergraduate teaching and clinical training in the dental treatment of patients who had additional needs due to medical, physical or anxiety issues. At that time there was no recognised Dental Specialty in Special Care Dentistry and so the initial appointment was as a Consultant in Restorative Dentistry with a remit to take forward the development of Special Care Dentistry within the dental curricula, a requirement of the General Dental Council. Once the specialty was recognised and the consultant was entered on the GDC specialist list in Special Care Dentistry, the name of the post was modified to reflect this change. Special Care Dentistry within the Dental Hospital consists of one Consultant, one Specialty Grade Dentist who is a Specialist in Special Care Dentistry and a CT2 trainee. They are supported by Dental Nurses who have training and dental nurse experience in intravenous sedation and a part time clinical psychologist. Some of the students’ clinical experience and teaching in sedation and behavioral management is provided within their attachments in oral surgery and paediatric dentistry and the clinical staff in those disciplines work collaboratively with the consultant in special care dentistry to ensure that all the teaching needs are met. Patients are referred to this service from GDPs, GMPs, Cardiology, Haematology, Oncology, the Renal Unit, Immunology, Dermatology, Social Services, Psychology and a range of other consultant clinical services throughout Tayside. Referrals are also received for the care of patients living in Tayside from drug problem services and the Liver Transplant Unit in Edinburgh. There are opportunities to develop even further the links with clinicians at the main teaching hospital at Ninewells, but lack of facilities as well as personnel limits this at the moment. There is also a steady stream of patients who attend the Dental A&E walk-in service who are referred to Special Care because they require help with overcoming their anxiety of dental treatment. The Consultant and Specialist both provide treatment under intravenous sedation for appropriate patients and are both trained and qualified as sedationists. A consultant anaesthetist also provides sedation once every two weeks at the Dental Hospital for patients with a more complex medical status/dental needs. This avoids the need for these patients to require day-case facilities at Ninewells. For those whose treatment requires a general anaesthetic, there is a theatre list every four weeks but this is not always sufficient to ensure that patients are treated within the 12 week treatment guarantee. Throughout the UK the majority of Special Care Dentistry takes place within a community setting, but there remains a necessity to provide secondary and tertiary care for vulnerable patient groups. Within the Dental Teaching Hospitals, it is recognised that there is a place and a need for consultant appointments, both to provide secondary care service delivery, as well as providing training. Referrals of ASA 3 patients have significantly increased and their complex nature requires care from an appropriately consultant led team. During the clinical semesters the BDS undergraduate students are allocated to all of the clinical sessions that come under the remit of Special Care and this teaching component is an integral part of the consultant job plan. There is a need for some expansion of the teaching to include the BSc Dental Therapy students as there will be an expectation that, in the future, they will need to meet some of the dental treatment demands of an ageing population with complex needs. Appropriate candidates for this post will hold full registration with the General Dental Council and should be on the GDC Specialist List in Special Care Dentistry (unless they are not yet eligible for Continued entry due to stage of their training) and will ideally have completed, or be within 6 months of completing a recognised training pathway and success in the Tri-Collegiate Membership in Special Care Dentistry. Applicants are also welcome from those on the GDC specialist list who have a Membership in Special Care Dentistry or who already hold a relevant consultant appointment. Integrated Dental Service The Hospital Dental Service together with the Public Dental Service makes up the NHS Tayside Integrated Dental Service – IDS – which is part of the Access Directorate of NHS Tayside. It is hoped that the successful candidate will work collaboratively with their PDS colleagues in the development of relevant clinical pathways and the training of future specialists in Special Care Dentistry. The Dental Hospital is situated on the main University of Dundee campus, close to the city centre. The main teaching hospital for NHS Tayside is Ninewells Hospital and Medical School, approximately three miles from the Dental Hospital but with excellent transport links to the University and the City centre. A full range of laboratory services is available throughout NHS Tayside which includes Haematology, Biochemistry, Immunology and Microbiology, Imaging and Pathology. The Dental Radiology services are based on the Dental Hospital site and include ultrasound, a full digital dental radiography system and Cone Beam CT. Access to CT, MRI, PET and radio nucleotide imaging is available at Ninewells. Duties of the Post CLINICAL To provide a Special Care Dentistry consultant clinical service within the NHS Tayside based at Dundee Dental Hospital, although duties could involve delivery of a clinical service in other sites throughout Tayside, particularly Ninewells Hospital Dundee, and to accept referrals from primary care dental and medical practitioners, appropriate secondary referrals from specialists within the hospital service and from other appropriate agencies. This service will include the personal treatment of patients under conscious sedation and general anaesthesia. The consultant will be suitably qualified in order that they can deliver intravenous and inhalation sedation and be up to date in their knowledge of the current guidelines and life support techniques. The appointee will be responsible for the delivery of their clinical service to the Clinical Lead/Director within the Dental Hospital and will work closely with the Clinical Service Manager with regards to the day to day running of the specialist service. TEACHING A significant aspect of this post is to be the lead person in designing and delivering the didactic and clinical teaching in Special Care Dentistry and Dental Anxiety. They will be supported by other staff within the specialty and within psychology, oral surgery and paediatric dentistry. Students will attend many of the consultants’ clinical sessions, as they do in many other clinical disciplines, and the clinical job plan will be modified to allow for the didactic teaching at the times that it occurs within the curricula. There is a need to expand the teaching so that the BSc Oral Health Science students (Dental Therapy) many have some experience within this specialty. MANAGEMENT This post has no formal management duties on appointment, other than, in common with all consultants, being responsible for making arrangements to deliver the clinical service as per their job plan but there are opportunities available for all consultants to undergo further training in management and leadership to equip them to take up such roles in the future. If in the future the consultant is willing to become an appraiser then this will be recognised in the job plan at 0.5 sessions per week. Continued TIMETABLE This appointment will initially be for 10 four hour sessions per week composed of 9 sessions of direct patient care and 1 session for supporting professional activities. The outpatient clinical sessions are from 9 a.m. to 12.30 p.m. and 1:30 p.m. to 5 p.m. A substantive job plan will be discussed and agreed with the Clinical Lead, usually within three months of taking up their post, and can be altered from time to time by mutual agreement.
