SCOPE OF WORK
1.
GENERAL SCOPE
Contractor must be a California State licensed pharmacist.
Contractor will provide Drug Regimen Reviews (DRR) for assigned clients living
at Porterville Developmental Center (PDC). Contractor will provide DRR’s that
meet licensing and certification standards for the State of California. The
completed Reviews shall meet the requirements of the DRR as outlined in the
State of California Code of Regulations, title 22 for GAC, NF, and ICF levels of
care. Inspection of drug storage may be assigned. Contractor will provide other
pharmacy duties as assigned as needed.

DESCRIPTION OF DUTIES


The review of the drug regimen of each client shall include all drugs
currently ordered, information concerning the client’s condition relating to
drug therapy, medication administration records, and where appropriate
physician’s progress, nurse’s notes, Behavior Management Committee
approval and laboratory results.
The forms utilized for the reviews shall be the forms of PDC.

The number of client reviews assigned by the Pharmacy Services
Manager (PSM) will be based on complete living residences generally
between 16 and 44 clients each.
The average time per record review shall not exceed 30 minutes.
DRR’s will be completed within the assigned month.
Inspection of assigned drug storage areas.
A written DRR report and the clinic room inspection report will be provided
to the Pharmacy Office Technician no later than one week after the end of
the month.
Completed therapeutic/clinical recommendations will be entered into the
pharmacy data base for review by the Residence Physician through the
Pharmacy Office Technician. Nursing recommendations will be entered
onto the pharmacy data base for review by the Residence Manager
through the Pharmacy Office Technician.
Contractor agrees to provide evidence of and maintain adequate
professional liability insurance.
Review physicians’ medication orders for potential allergy, drug-drug/food
interactions, over guidelines, or other therapeutic issues.
Validates computer entry of medication orders and maintains
computerized patient profiles on the PDC computer system.
Checks Pharmacy Technicians’ filling of medication orders (includes first
doses, stat doses and canisters).
Pharmacy Technicians’ work including ATC, controlled drugs and
repackaging.
Compounds intravenous fluids.
Performs clinical reviews of patient medication regimens.
Monitor, investigate and report adverse drug reactions
Participate in Drug Use Evaluations and other performance improvement
activities.
Participate in Interdisciplinary Team reviews up request.
MATERIALS



Contractor will provide all necessary office/technical supplies/equipment to
complete the DRR.
Facility will provide computer access to DRR data base and training for
data entry.
Facility will provide residence access.

LOCATION
Contractor will do record reviews on the assigned residences at PDC.
6.
HOURS AND ASSIGNMENTS
DRR assignments will be issued on a monthly basis. The Pharmacy Services
Manager will compose DRR assignments ten working days prior to the initiations
of the reviews. The Contractor will be given a sufficient amount of time per living
residence per month. Total hours not to exceed 592 for the duration of this
contract.
