The doctors and staff at Radnor House Surgery in Ascot are proud to offer the highest standard
of patient-centred healthcare. We aim to work in partnership with you. We will involve you in all
aspects of your care, ensuring that you are aware of and part of what happens.

We are a Training practice training Doctors to become General Practitioners as part of the
Windsor Vocational Training Scheme,
CAR PARKING
There is on-road parking. If that is not available, there is a FREE car park available a short
distance away, just before the Land Rover garage, towards Ascot High Street. Please avoid
parking on double yellow lines. There are 2 disabled parking bays adjacent to the surgery.

When We Are Closed
Outside Surgery hours, the telephone number of the Duty Doctor is on the answer machine. At
the moment all calls are taken by EBPC-East Berkshire Primary Care - a co-op of General
Practitioners call-handled through Royal Berkshire Ambulance Trust. Telephone number 01189

NHS Direct
In the case of urgent need when the practice is closed you can call NHS Direct
(www.nhsdirect.nhs.uk/) (0845 4647) to speak to a triage nurse. Your needs will be assessed and
advice offered or arrangements made for you to see a doctor.
Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery
re-opens.
Call 999 in an emergency. Chest pains and / or shortness of breath constitute an
emergency.
Appointments
Surgeries with the member of the Team are by appointment up to two weeks in advance. You
may request a particular doctor. Please telephone 01344 874011.
We aim to see all emergencies on the day of request. There are a certain number of appointments
kept for on the urgent day appointments.
To access these please telephone at 8.30am or 2.00pm on the day. Please make every effort to
keep your appointment.
If you are unable to keep it, let the Surgery know as soon as possible so that the appointment
may be re-allocated
Telephone Advice
The Receptionists will be able to advise you of the best time to telephone for advice from the
Doctor or Nurse.
Home Visits
If possible please try to telephone reception before 10am if you require a home visit.
A doctor or nurse may phone you back as it may be that your problem can be dealt with by
telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a
hospital attendance.
House visits are only available for patients who are housebound because of illness or disability.
Please remember that several patients can be seen in the practice in the time that it takes to make
one home visit. There are also better facilities for examining and treating patients at the Health
Centre.
Repeat Prescriptions
The Practice offers the facility to obtain regular prescribed medication by repeat prescription.
Please allow 2 working days for us to process your request.
Please use the computer printed repeat slip, or if this is not available, please put your request in
writing.
We do not take requests over the telephone.
If you leave us a stamped addressed envelope we will gladly post you your repeat prescription.
Prescriptions Charges and Exemptions
Extensive exemption and remission arrangements protect those likely to have difficulty in paying
charges (NHS prescription and dental charges, optical and hospital travel costs).
The NHS prescription charge is a flat-rate amount which successive Governments have thought
it reasonable to charge for those who can afford to pay for their medicines. Prescription
prepayment certificates (PPCs) offer real savings for people who need extensive medication.
NHS charges
From 1 April 2009, the charges are:



Prescription (per item): £7.20
12-month prepayment certificate (PPC): £104.00
3-month PPC: £28.25
If you will have to pay for four or more prescription items in three months, or more than 14 items
in 12 months, you may find it cheaper to buy a PPC. The charge for a single prescription item is
£7.20, whereas a three month PPC will cost you £28.25 and a 12 month PPC £104.00.



Clinics & Services
SERVICES AND CLINICS
We offer the full range of General Medical Services. Although we are small, we pride ourselves
on being able to offer all the usual facilities including Maternity Services, Baby Immunisations
Clinic, Minor Surgery, Child Health Surveillance, Chronic Heart Disease, Asthma and Diabetic
Clinics, Well Person advice and general advice on maintaining a healthy body.
TRAVEL CLINIC
The Nursing Service is fully trained in providing you with all the advice and injections you
might need for foreign travel. We are also a registered Yellow Fever Centre and can offer this
service. Please contact our Practice Nurse to discuss your requirements; you will need to
complete a travel form as well.
CERVICAL SMEARS
We feel that prevention of cancer by regular screening is part of good medicine. We recommend
that every woman between 25-65 years of age should have a regular cervical smear every 3-5
years. Please make an appointment with the Nurse for this and mention that it is for a cervical
smear, as a double appointment is needed.
MINOR ILLNESSES
There is a leaflet available at the Surgery, which offers easy to follow advice on how to deal with
minor illnesses.
EXTRA SERVICES
As well as providing the full range of General Medical Services, we also provide services such
as medical examinations, fitness reports, passport signing, private certificates etc. There will
usually be a fee charged for this payable on application. Please enquire at Reception for details.
Test Results
Test Results
Please call after 3:00pm to enquire about your test results as our reception staff will have more
time to deal with your request between these times.
Note that the practice has a strict policy regarding confidentiality and data protection and we will
only release test results to the person to whom they relate unless that person has given prior
permission for the release of this data or they are not capable of understanding the results.
When you take your test you will be told how long it will be before the results are returned to the
practice.
It is your responsibility to check your results and to make an appointment to discuss them with
your doctor if your are advised to do so.
Registration
New Patient Registration
If you would like to register as a patient, please contact the Surgery on the usual number, or call
in person to speak to a receptionist.
We ask all new patients to complete the Practice Questionnaire and require photographic
identification when registering.
If you wish to register your child under 5, please bring red book with details of immunisations.
Practice Area
The Practice area extends from Ascot to the edge of Bracknell, Sunninghill and Sunningdale. A
map is available to view in Reception.
Confidentiality
Confidentiality
The practice complies with Data Protection and Access to Medical Records legislation.
Identifiable information about you will be shared with others in the following circumstances:



To provide further medical treatment for you e.g. from district nurses and hospital
services.
To help you get other services e.g. from the social work department. This requires your
consent.
When we have a duty to others e.g. in child protection cases anonymised patient
information will also be used at local and national level to help the Health Board and
Government plan services e.g. for diabetic care.
If you do not wish anonymous information about you to be used in such a way, please let us
know.
Reception and administration staff require access to your medical records in order to do their
jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.

Comments & Complaints
Comments & Complaints
We make every effort to give the best service possible to everyone who attends our practice.
However, we are aware that things can go wrong resulting in a patient feeling that they have a
genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly,
and as amicably, as possible.
To pursue a complaint please contact the practice manager and she will deal with your concerns
appropriately. Further written information is available on the complaints procedure from
reception.
We are continually striving to improve our service. Any helpful suggestions would be much
appreciated and a suggestion box is located in the waiting area.

Staff Details
Doctors
Dr JOHN
RAWLINSON
Married with three children, qualified at Oxford University in
1979. He completed his General Practice training in Cirencester in
Gloucestershire where he was fortunate to have worked for a year
in a mini General Hospital, gaining wide experience in major
branches of Medicine. He also gained experience in Psychiatry,
Paediatrics and Obstetrics and Gynaecology. He has also
undertaken specialist training in Family Planning and Child Care.
His other interests include Medical Politics, ensuring that the views
of General Practice are fairly represented at the Primary Care
Organisation, Health Authority and NHS Executive. He has
recently obtained the MRCGP qualification. He is also a GP trainer
and Radnor House is now a training practice.
Dr KATIE
SIMPSON
Married with three children, qualified at Leeds in 1991. She
completed her General Practitioner training in Windsor and Ascot.
Awarded GP Registrar of the Year prize in 1995 (East Berkshire
GP Training Scheme). She has worked in Accident and
Emergency, Paediatrics, Psychiatry, Medicine and Obstetrics and
Gynaecology. She has specialist qualifications in Family Planning
and Obstetrics and Gynaecology. She holds the Post Graduate
Degree of Membership of the Royal College of General
Practitioners (MRCGP). In addition she is the lead GP for mental
health in the Berkshire East PCT.
Nurse Practitioners
Zena Linda Our Nurse Practitioners are able to offer the full range Primary Care.
Davison
Nursing activities and are available by appointment. Nurse Zena works
Tuesday and Thursday 8.30-12.30 and runs her INR clinic Thursday
afternoon 2-3.

Practice Management
Mrs Ann Higgins is our Practice Administrator. She is available to deal with any
administration matters about the Practice and to offer you any help and guidance
that will enable you to utilise our services fully to your benefit and to best effect.
Reception
The Reception team is experienced and will be able to deal with your appointments,
home visit request, repeat prescriptions and general enquiries about services
available at the Practice. Any information given to any member of the Surgery staff
will be treated CONFIDENTIALLY. You may speak to them any time during
surgery opening hours.
Community Midwives
Our Midwife is part of the Diamond team based at Heatherwood Hospital, our local
hospital based in Ascot, and visits the Practice every fortnight
Community Nurses
We have a highly experienced District Nursing Team which is based at Green
Meadows Surgery. They can be contacted on 01344 873275. Staff Nurse Clare
Broad is attached to Radnor House and visits the surgery daily.
Health Visitors
Health Visitor, Cate Ingram can be contacted on 01753 636523/4 Baby Clinic is on
Tue 9.30-11.15 St Michaels Church Hall Sunninghill.
