The doctors and staff at Mildmary Medical Practice in North London are proud to offer the highest standard of patient-centred healthcare.
We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups.

In the case of urgent need when the practice is closed you can call NHS Direct
(www.nhsdirect.nhs.uk/) (0845 4647) to speak to a triage nurse. Your needs will be assessed and advice offered or arrangements made for you to see a doctor.
Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery re-opens.
Call 999 in an emergency. Chest pains and / or shortness of breath constitute an emergency.
You are able to make appointments with the practice by telephone (020 7923 1999) or in person.
You can book appointments 2-3 weeks in advance for non-urgent problems or review. This is
Option 2 on the telephone. Please register your mobile phone number with reception for an automatic appointment reminder one day before your appointment is due.
We try to ensure that you see the doctor of your choice, but this is not always possible and it may be necessary to see someone else.
Reception can be very busy in the morning so we would ask you to wait until after 10:30am for an appointment or query that is not urgent. This will help us to offer a good service to everyone.
For a problem that has just arisen or cannot wait, we are able to offer a triage call-back service.
Please call Reception between 8:30 and 11:00am (Option 1 on the telephone) and a GP will call you back and discuss your symptoms with you. You may be asked to come in for an appointment with a suitable clinician, or your problem may be dealt with over the telephone. Please remember to telephone between 8:30 and 11:00am.
Please tell the receptionist if you have an emergency and they will advise you. When we are closed you should contact HARMONI on 020 7388 5800.

The Practice is fully accessible
Mothers are welcome to feed their babies whilst they are at the practice. Please ask at Reception if you would like a private space.
These are only for people who are not able to get to the surgery either because they are housebound or too unwell. Please phone between 9.00am and 11.00am
A doctor or nurse may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.
House visits are only available for patients who are housebound because of illness or disability.
Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the Health
Centre.

Please book an appointment with our Nurse for the following - NHS Health Check, smear, blood pressure check, travel advice & immunisations, asthma check, stop smoking advice, contraception & well woman advice, diabetic check, flu jabs.
Requests must be in writing. Please allow 48 hours (2 working days) before collecting during surgery opening hours.
If you would like us to post your prescription, please provide a stamped addressed envelope.
We do not accept repeat prescriptions over the phone unless we know you are housebound.
Extensive exemption and remission arrangements protect those likely to have difficulty in paying charges (NHS prescription and dental charges, optical and hospital travel costs).
The NHS prescription charge is a flat-rate amount which successive Governments have thought it reasonable to charge for those who can afford to pay for their medicines. Prescription prepayment certificates (PPCs) offer real savings for people who need extensive medication.
From 1 April 2011, the charges are:
Prescription (per item): £7.65
12-month prepayment certificate (PPC): £104.00
3-month PPC: £29.10
If you will have to pay for four or more prescription items in three months, or more than 14 items in 12 months, you may find it cheaper to buy a PPC.

Shared antenatal care with the Homerton, Whittington, UCH and Royal Free Hospitals is provided
A drop-in clinic for immunisation, weighing and advice from the Health Visitor. New baby checks by appointment with the GP.
Advice, sexual health screening (eg HIV testing, chlamydia screening) smears, emergency contraception and provision of free condoms. Caps (diaphragm) or coils (IUCD or IUS) are fitted by appointment with the doctor. Please see the Nurse for pill checks and Depo injections.
Confidential advice and treatment about contraception and sexual health for young people (under
18) – please ask to see a GP.
We offer full care for the majority of our diabetic patients and regular review of all patients with asthma.
In-house support based on CBT. Please see the GP for referral.

Please telephone after 12:00 midday (Option 3 on the telephone) to enquire about your test results as our reception staff will have more time to deal with your request at this time.
Note that the practice has a strict policy regarding confidentiality and data protection and we will only release test results to the person to whom they relate, unless that person has given prior permission for the release of this data or they are not capable of understanding the results.
When you take your test you will be told how long it will be before the results will be available to the practice.
It is your responsibility to check your results and to make an appointment to discuss them with your doctor if you are advised to do so.
The doctors welcome new patients who live within our practice area. To register you must live in our practice area. A map of the area is available at reception. All patients (5yrs and over) are offered a health check.
You may choose to see any of our doctors and we will try to offer appointments with the doctor of your choice, timetable and holidays permitting. All doctors have access to your computerised medical record.
As it often takes some time for records to be forwarded from your former practice, all newly registered patients will be asked to complete a health questionnaire ( below ) and attend an appointment with the practice nurse.
Registration is on Tuesday, Wednesday and Friday between 5:30pm and 6:00pm. You will need:
Your NHS number
One recent proof of address (eg a utility bill dated within the past 3 months)
One proof of residency (eg passport, Home Office papers, a full UK drivers' licence)
You may print off the registration form and health questionnaire. Please fill them out and bring with you when you come to register.

Some of the information you have supplied to us will be held on computer or on paper-based files. They are your confidential medical records.
All staff members at the surgery are bound by law to keep your records confidential.
Sometimes we may share this information with other health professionals involved in your care to help them meet your needs.
You can ask to see your own records, but we need a few days to arrange this.
We are always interested to receive your comments and suggestions. If you experience any problems with the services we provide, please ask to speak with the practice manager. An information leaflet about our complaints procedure is available from reception.
