Requirements for Enduring Materials
All enduring materials must be reviewed and approved by the Division of CME prior to distribution. Please
complete and submit this page with the first draft of your enduring material.
Required Content This content must be transmitted to the learner before the enduring material begins.

Begin with the phrase, “After participating in this CME activity, participants should be able to:” and then
list the objectives in a bulleted format using action verbs. The objectives must match those on the
application for CME credit.

This information must match that on the application for CME credit
□ Faculty Disclosure Statement (required)
It is the policy of the ACCME and Vanderbilt University School of Medicine to require disclosure of
financial relationships from individuals in a position to control the content of a CME activity; to identify
and resolve conflicts of interest related to those relationships; and to make disclosure information
available to the audience prior to the CME activity. Presenters are required to disclose discussions of
unlabeled/unapproved uses of drugs or devices during their presentations.

Post Enduring Material Requirements
□ Learner assessment (required)
Examples include, but are not limited to patient-management case studies and post-tests. Post-tests will
require a minimum of three questions that address diagnosis, management, and anticipated outcomes.
A minimum performance level must be established and expressed to learners before the enduring material begins.
e.g.: In order to be awarded credit for participating in this activity, a minimum score of 80% is required on the post-test.
□ Bibliographic sources (required)
At least one evidence-based reference (i.e., peer-reviewed article or guideline) is required.
□ Evaluation (required)
All individuals requesting credit must be given the opportunity to evaluate their learning experience.

Additional Requirements for Online CME
□ Activity location
Online CME activities may not be hosted on a website owned or controlled by a commercial entity.
□ Links to product websites
Embedding links to healthcare-related company websites within the educational content of the online
activity is prohibited.
□ Advertising
Advertising of any type is prohibited within the educational content of online CME activities including, but
not limited to, banner ads, subliminal ads, and pop-up windows.
□ Hardware/software requirements
Hardware and software requirements must be transmitted to the learner prior to the start of the online
activity.
□ Contact information
Contact information must be made available to the learner.
□ Policy on privacy and confidentiality
The direct/joint sponsor must adhere to and inform the learner of the following policy:
