HOW TO COMPLETE THE REPORT OF ADVISORY CONFERENCE
Before completing this form, please read the Graduate College Bulletin found on our website at
http://gradweb.ou.edu/Current/gcBulletin to understand the details of the Advisory Conference,
Committee Membership regulations, and transferable credit information. Meet with your department
advisor and/or committee chair to verify that your proposed program conforms to Graduate College
policy. Once approved by the doctoral committee, graduate liaison, and the graduate dean, the Report of
Advisory Conference (also known as ACR), constitutes the doctoral student’s specific program of study.

The Report of Advisory Conference:
1) To aid the student in developing an overall plan for attaining the doctoral degree, it is
recommended that the ACR be completed within or as close to the 1 st year of student
enrollment in the doctoral program.
2) Any changes to the program of study or doctoral committee require the completion of the
Request for Change in Advisory Conference found at
http://gradweb.ou.edu/Current/Forms/doctoral. Please note that changes must be made at
least 30 days before the date of the General Exam.
3) Must be submitted to the Graduate College before the Application to Take the General
Exam; not simultaneously.
4) Must be typewritten.
5) Must be endorsed and signed by all committee members and the department liaison.
When completing the Report of Advisory Conference, please note the following:
1) Courses with grades of “A, B, or S” are allowed; no grades of “P, CR” are accepted.
2) List courses in chronological order (by semester)
3) List each course individually with each semester course is taken (for example, do not write
“32 hours of ABCD 1234 Independent Study;” you must write each course individually i.e. 3
hours of ABCD 1234 Ind. Study FA05, 3 hours of ABCD 1234 Ind. Study SP06, and so on).
Section 1: Student information
Please type your full name, Sooner ID number, mailing address, phone number, email address, name
of degree (i.e. Doctor of Philosophy, Ph. D., Executive Doctor), major code (i.e. 1501M), major
name (i.e. English).
Section 2: Tools of Research
These are courses required by the Department and/or committee deemed necessary for research
purposes. Know that your committee may require you to complete tools of research that are not
required by the department.
NOTE:
1) Courses listed in the Tools of Research must also be listed on Application to Take the
General Exam
2) Education students: If your tools of research will count toward your doctoral degree hours,
please indicate this by marking an “x” in the box found in the Tools of Research section.
Section 3: Graduate Coursework Forming the Master’s Degree to be applied to Ph. D.
If you have received a master’s degree from another accredited university, please list all course
information in this box. The doctoral time limits do not apply to these courses.
All OU master’s degree students should use the Program of Graduate Work/Admission to
Candidacy form to complete this section. The doctoral time limits do not apply to these courses.
NOTE:
1) Coursework must carry a grade of “A”, “B”, or “S” and
2) All master’s coursework, whether “required for PhD” must be included here and
3) Thesis hours are limited to the maximum number of thesis hours required for the master’s
degree (for example, if a department only requires 6 thesis hours and you have complete 12
hours, you may only apply 6 hours of thesis).
Section 4: Graduate Credit Not Applied to the Master’s Degree to be applied to Ph. D.
All coursework listed in this section must have been completed prior to admission into the OU
doctoral degree program and should include any/all of the following:
1) Graduate-level courses completed after receiving a bachelor’s degree, but not applied to a
master’s degree
2) Graduate-level courses completed after receiving the master’s degree (also referred to as
post-master’s degree work)
3) Graduate-level courses completed during the master’s degree, but not applied to the
master’s degree
4) Transfer credit not applied to the master’s degree
All coursework in this section is regarded as transfer work and is subject to the Graduate College
policies regarding transfer credit.
1) Time limits begin with the oldest course in this section. If there are no courses listed in
Section 4, time limits begin with the oldest course in Sections 5 or 6.
2) Only include courses applicable to the doctoral degree
3) Must carry a grade of “A”, “B”, or “S” and be no more than five years old at time of
admission/readmission to the doctoral program. Grades of “B-” do not transfer.
SECTIONS 3 & 4 combined cannot total more than 44 hours.
Section 5: Required Coursework
Include graduate-level courses completed while enrolled as a doctoral student at OU. Meet with
your department advisor or committee chair to discuss the required courses as specified in the
approved program. Do not list dissertation hours in this section. Indicate the total number of
dissertation hours in the Summary of Credit Hours.
Section 6: Elective Coursework
Include graduate-level courses completed while enrolled as a doctoral student at OU which meet
the specific elective requirements listed in the approved program. Do not list dissertation hours in
this section. Indicate the total number of dissertation hours in the Summary of Credit Hours.
DO NOT LIST DISSERTATION HOURS IN SECTION 5 or 6. Indicate the total number of
dissertation hours in Section 7 Summary of Credit Hours.
Section 7: Summary of Credit Hours
Include the total number of hours for Sections 3-6.
List the total number of dissertation hours in this section; do not list them in any other sections.
Section 8: Signatures
STUDENT: Please read and sign. Include the date of your Advisory Conference and type the first
and last name of each committee member. Indicate which member is serving as the committee chair
and the outside member. Their roles are defined at the website below.
COMMITTEE: Please sign and include your department and Graduate Faculty Appointment
expiration date. If you have questions regarding your Appointment or the duties of the chair or
outside member, please visit http://gradweb.ou.edu/People/GradFaculty or contact your department
staff assistant or the Graduate College.
