GENERAL TERMS AND CONDITIONS OF TENDER
1.
SUBJECT OF TENDER: Tender for conducting medical examination of
Workmen of Hindustan Insecticides Ltd. (hereinafter referred to as “COMPANY”).
Udyogamandal, Kerala.
2.
SCOPE OF WORK:
i) To conduct the following Diagnostic test/ Examination of Workmen of the COMPANY
for three years ie. 2015-16, 2016-17 and 2017-18 on specified duration.

ii)
The samples of blood and urine of the Workmen of the COMPANY shall be
collected from the COMPANY premises on the prescribed working days between
8.30am to 12.00 Noon.
iii)
The Audiometry Test (Hearing evaluation) and Pulmonary Function Test should
be conducted at the premises of the COMPANY on the prescribed working days
between 8.30am to 12.00 Noon.
iv)
The diagnostic test / examination reports shall be submitted to the COMPANY
within two weeks time from the date of performance of prescribed tests.

v)
The rates quoted shall be valid for a period of three years ie. 2015-16, 2016-17
and 2017-18
3. MODE OF SUBMITTING TENDER: Tender in the prescribed terms as given in
tender papers should be sent either by Courier Service/Speed Post/Regd.A.D.OR
personally delivered in SEALED Envelopes addressed to the Personnel Manager,
Hindustan Insecticides Ltd., Udyogamandal-683 501 in TWO SEPARATE BIDS
SYSTEM.
(1) TECHNO COMMERCIAL BID(Annexure-I) : This bid along with signed
General Terms & Conditions of Tender in sealed envelope should be superscribed
“PW- 34 dtd 05.08.2015 Techno Commercial Bid, due on 03.09.2015 at 2pm.
(2) PRICE BID (Annexure II) : This bid should be in separate sealed envelope
superscribed “PW – 34 dtd 05.08.2015, Price Bid.”
Both the above sealed envelopes should be enclosed in a third envelope. The third
envelope should also be sealed and addressed to HINDUSTAN INSECTICIDES
LIMITED, UDYOGAMANDAL and superscribed “PW – 34 - “TENDER FOR MEDICAL
EXAMINATION OF WORKMEN OF HIL DUE ON 03.09.2015 at 2PM.”
Only the technical bid will be opened on due date. After verification and evaluation of the
technical bid, successful tenderers will be informed of the date of opening of price bids
and will be opened on the fixed date and time in the presence of available tenderers or
their representatives. In case of variation in figures and words, the words version shall be
taken as final. Overwriting in the tender is not allowed.
Correction made in the tender should be signed by the tenderer and each page of the
terms and conditions, tender forms duly filled in should be signed, sealed and dated by
the tenderer.
TENDERERS MAY ALSO DOWNLOAD THE TENDER DOCUMENT FROM HIL
WEBSITE (www.hil.gov.in) AND SUBMIT THEIR OFFER AS MENTIONED ABOVE.
4. QUALIFICATION OF THE TENDERER
The tenderer who wish to quote should meet the following qualifications.
i) The Pathological Lab for which tender is to be quoted should have been established
atleast 3 years back with impeccable track record.
ii) The pathological Lab should have experience in conducting Group Medical
Examination for Workmen in any two establishments having workforce of 50 Workmen.
iii) The pathological Lab should be equipped to collect blood and urine samples of
workmen of the COMPANY from the premises of the COMPANY.
iv) The pathological Lab should be equipped to conduct Audiometry Test (Hearing
evaluation) and Pulmonary Function Test at the premises of the COMPANY.

5.
DOCUMENTS TO BE SUBMITTED ALONGWITH TENDER
i) Self attested copy of valid license issued by the Panchayath/Municipality/Corporation.
ii) Self attested copy of the valid Registration certificate issued by the Department of
Health, Govt. of Kerala.
iii) Documentary proof in support of experience in conducting Group Medical
Examination.
6.
LAST DATE FOR SUBMITTING TENDER:
The sealed tender should be received on or before 03.09.2015 at 2 PM. Tender received
after this date and time will not be entertained and considered.
7.
DUE DATE FOR OPENING TENDER:
The tender under sealed envelopes will be opened at 2.30 PM on 03.09.2015 at the Office
of the COMPANY in the presence of such tenderers or their duly authorized
representative who are present personally.
8.
TENDER PRICE: The price should be quoted for each Diagnostic test/
Examination.
There would be generally no negotiations hence please submit your most competitive
prices while submitting the price bid. If required, the negotiation will be held with L1
(Lowest quoted) party only.
9.
VALIDITY PERIOD OF THE TENDER: The tender should be valid for a
period of 6 months for acceptance from the date of the opening of the Tender.
10.
ACCEPTANCE OF TENDER: The COMPANY reserves the right to accept any
or reject any or all tenders without assigning any reason. COMPANY may seek
clarification on the tendered prices and the terms and conditions before accepting any
tender.
The COMPANY shall assess the qualifications and competency of the bidder and the
acceptance of the tender is subject to the satisfaction of the same.
11.
INCOMPLETE TENDER OR LATE TENDER: Tenders received late,
unsealed or without superscription or incomplete in any other respect are liable to be
rejected. For such cases the decision of the tender opening committee constituted by the
COMPANY will be final.
12. SUCCESSFUL BIDDER: The successful bidder shall be informed of the intention
of the COMPANY to award the work through Work Order before the date of expiry of the
validity of the tender.
13.
WORK ORDER: The successful bidder shall be issued with a Work Order. Such
successful bidder hereinafter shall be referred to as CONTRACTOR.

14.
PAYMENT TERMS:
i)
The payment shall be released within 30 days on receipt of the bills, after
submission of reports of the Diagnostic tests / Examinations of the entire Workmen.
15.
STATUTORY PAYMENTS / LIABILITY:
i)
Income Tax and other statutory taxes, if any, will be recoverable from the
CONTRACTOR’s bill as per rules.
16.
TERMINATION OF CONTRACT:
Notwithstanding anything contained herein the COMPANY has the right at any
time to terminate this contract either wholly or in part by giving one Month Notice in
writing to the CONTRACTOR by a registered mail. The COMPANY has no further
liability to make any payment and all the obligations under this contract shall cease to
exist after the expiry of the said period of notice.
Provided that no notice is required to be given except a letter of termination in
case the CONTRACTOR fails to comply with the terms and conditions of this Contract.
17.
LICENSE / REGISTRATION
The CONTRACTOR shall obtain all Licenses/Registration and comply with all
obligations under any other relevant statutes in force from time to time for the due
fulfillment of the contract.
18.
DISPUTE SETTLEMENTS:
If any dispute arises between the COMPANY and the CONTRACTOR as to the
terms and conditions of the agreement, of the manner, method of compliance thereof, the
decision of the Chairman & Managing Director of the COMPANY shall be final and
binding on both the parties.
19. JURISDICTION:
The Court of Law in Ernakulam District, Kerala shall have jurisdiction for
disputes arising out of this tender/contract.
