OPMC 16.200.030E Failure to provide/maintain washout area Are washout facilities (e.g. concrete, paint, stucco) available, clearly marked and maintained? Are non-stormwater discharges (e.g., wash water, dewatering) properly controlled? OPMC 16.200.030E Improper storage/disposal of materials Are vehicle and equipment fueling, cleaning, and maintenance areas free of spills, leaks, or any other deleterious material? Are materials that are potential stormwater contaminants stored inside or under cover? OPMC 16.200.030D Failure to stabilize site Are all slopes and disturbed areas not actively being worked properly stabilized? OPMC 16.200.040A Sediment leaving site Are discharge points and receiving waters free of any sediment deposits? OPMC 16.200.040B Tracking sediment off-site Is there evidence of sediment in the street or being tracked off-site? OPMC 16.200.040B Failure to install/maintain temporary sediment controls Are perimeter controls and sediment barriers adequately installed (keyed into substrate) and maintained? Are natural resource areas (e.g., streams, wetlands, mature trees, etc.) protected with barriers or similar BMPs? OPMC 16.200.040B Failure to install/maintain construction entrance Is the construction exit preventing sediment from being tracked into the street? OPMC 16.200.060E Failure to remove temporary sediment controls. Is the site permanently stabilized and all temporary BMPs removed? Implemented? Yes No Yes No Yes No Yes No Yes No Maintenance Required? Yes No Corrective Action Needed and Notes will need stabilized Non-Compliance Describe any incidents of non-compliance not described above: All temporary control measures need to be inspected and maintained on a regular basis until site is permanently stabilized and temporary controls are no longer needed. Permanent stabilization will be by sodding or seeding all disturbed areas, with the resulting perennial vegetation having a minimum of 70% density, at which time all temporary measures will need to be removed. This inspection is not a replacement for regular contractor inspections as required by state and federal regulations. Contractors are responsible for all inspections and documentation as required by their coverage under the Kansas NPDES Stormwater Runoff from Construction Activities General Permit. 3
