In order to avoid delays in processing your L&I order, please follow the ordering instructions outlined below.
All required fields must be completed.
To ensure accuracy, Fujifilm recommends the L&I order form be completed electronically and then printed and
faxed with your cartridge purchase order. Handwritten forms increase the likelihood of inaccurate order
placement. Fax completed form with cartridge Purchase Order to 1-914-789-7930.

* It is important to correctly identify the ASCII or EBCDIC format. If defining multiple VOLSER ranges (4B Label Sequence) and not all use the same format, you must define the specific format for each range
separately, in section 5-Special Instructions.

Section A - Alpha Characters and Background Colors
o Indicate Alpha characters, if applicable.
o Indicate Background Colors for Alpha characters using Vibrant color chart selections.
o Background colors for Numeric characters may also be specified. If no background
colors are indicated for Numeric character positions, colors will default per Vibrant
color chart. (Example: "0" = Red, 1 = Yellow, etc.)
