Oversees the development, implementation and evaluation of assessments and care plans in consultation with the care team, residents and their families. Assist and educate staff in the implementation of care plans to ensure that resident care needs are actively put into place and improve the outcomes for residents. Support the Service Manager to ensure this service is managed and developed in line with the Mercy Health overall objectives. Position Purpose: Mercy Health None Care Support Coordinator Qualifications: Resource management: Essential: Registered Nurse. Three years experience in post graduate nursing. A working knowledge of the needs of elderly persons residing with a residential care facility Strong written and verbal communication skills. Excellent problem solving, assessment and care planning skills Positive attitude that promotes the Mercy Health care first concepts. A current valid police record check. Desirable: Relevant Post Graduate Qualifications in gerontology or working towards.
