Italy is a member of the Schengen agreement.
The country of your longest stay will issue your Schengen visa. Only if your stay in the Schengen
states is the same number of days in each country would you apply through the country of the first
port of entry.
Schengen visas are free of charge to spouses & children (under 18) of EU passport holders. A copy
of the EU passport & marriage certificate (spouse) or full birth certificate (child) is required. A letter
from the EU Spouse/Parent is also required stating that the family member will travel with or be
joining them.
FROM 6 JUNE 2013 ALL APPLICANTS OVER THE AGE OF 12 MUST APPEAR IN PERSON
FOR BIOMETRIC DATA COLLECTION (FINGER PRINTING AND DIGITAL PHOTOGRAPHS).

ITALY - TOURISM - SHORT STAY
For all persons travelling to Italy for Tourism purposes on trips equal to or less than 90 days.
For everyone, please provide:
� Passport
Cannot be older than 10 years. SA Passport must be valid for at least 3 months from date of return. Must
have the applicant's signature on the relevant information page (newer SA passports have the signature
embedded on the hardcover - Older SA passports must sign the information page at the back). Passports
require 3 valid blank pages in sequence (marked VISA). Visa pages are invalid if damaged or stained by
stamps from other pages. Please note, visas cannot be placed back-to-back. For Non-S.A passports, please
provide copy of valid South African permit (permit must also be valid for at least 6 months from the date of
return). Please add copies of any previous Schengen visas that have been issued in last 3 years, even if in
another passport.
� Schengen Application Form
All necessary fields to be neatly and accurately filled in (online form is recommended for better legibility,
please do not forget to print and bring it on the day of appointment). Form must be signed by applicant (3
signatures per form). In case of minors, the form MUST be signed by one of the parents.
� Color ID Photos
Two (2) Colour ID photos as required by Schengen Standards. Photos can be taken in Capago offices (Cape
Town and Johannesburg only). Please see photo specifications below.
� Travel Insurance
Single page travel insurance policy certificate to be provided. Must display name of insured person, policy
number, amount of cover (minimum € 30,000) and duration of cover. An applicant's insurance should cover
the full period of travel. Insurance must cover Worldwide or the entire Schengen area (even if Italy is the sole
destination). Insurance must provision for evacuation, repatriation and emergency transportation as
well as emergency in/out of hospital medical treatment.
� Proof of accommodation
It is essential to have 100% accommodation for the entire period of stay in the Schengen area, therefore all
confirmed bookings of hotels/apartments/rentals/backpacker's are necessary. The applicant's name needs to
appear on the accommodation bookings. If booked by a 3rd party, please have them provide a copy of their
I.D. and a letter of confirmation that you are their guest. Applicants visiting Italian Citizens or temporary
residence holders in Italy must have a Host Declaration form (available on the Capago website) completed
by their host and accompanied by copy of their Italian I.D./passport/permit and a recent utility bill in their
name. If however it is the applicant who owns property in Italy, please confirm this by providing either a proof
of residence such as Title Deeds, telephone accounts or recent utility bills from the last 3 months that are in
the applicant’s name.
� Proof of transport
All confirmed tickets (flight/ferry/train/bus/taxi) showing entry/exit from Italy or other Schengen countries
MUST be provided. Name of applicant must appear on all proof of transport. All confirmed car rental
bookings must equally be provided. If applicant is being collected by EU resident abroad, a copy of the EU’s
driver's licence, car registration details and a short letter of confirmation of pick-up and drop-off details will be
necessary.
� Proof of Residence
Recent utility bill, bank statements or telephone account from the last 3 months that features the name of
applicant and the physical address. Title Deeds, bank letters/statements and telephone accounts can also be
supplied as proof of residence. If residing with a 3rd party, please attach it to the proof of residence, a copy
of their I.D. as well as a signed letter confirming that you reside with them.
� Proof of funds
Latest 3 months bank statements (full name of applicant must appear). If sponsored by 3rd party, please
have them complete a letter of sponsorship and provide their latest 3 months bank statements (their name
must appear on the bank statements). If the statements are in the name of a company or a trust, then the
applicant must provide company registration documents or trust documents showing that they are
members/owners of the company or the trust. Any traveller who is sponsored by an Italian citizen or Italian
permit-holder living in Italy must obtain from their host a ‘Fideiussione Bancaria’ (letter of sponsorship) which
is obtained by from the bank in Italy and must provide a ID copy of the sponsor. Please note that bank
statements are requested for security reasons and are required even travels are fully paid for by company or
3rd party.
� Proof of employment (if applicable)
Signed confirmation on a company letterhead showing that the applicant is currently employed and will
return to resume his/her duties upon return to S.A. If an applicant owns their own company then they must
provide the company registration documents (CC or CK documents) on which applicant's name appears. If
the applicant operates as a sole trader then they must provide a written affidavit declaration to this effect that
is commissioned by the SAPS.
FOR MINORS, PLEASE ADD:
� Unabridged Birth Certificate
Must show names of both parents and child. If no Unabridged Birth Certificate can be submitted, please
provide an Abridged Birth Certificate, the receipt from an application for an Unabridged Birth Certificate and
most importantly, either a letter from the clinic where the child was born or a letter from the school which
must clearly state the identity of the biological parents.
� Letter from School / University / College
A signed & stamped letterhead declaring that applicant is a pupil/student currently registered at the
School/University/College.
� Parental Consent Letter (if child is travelling alone or with one parent)
Parental Authorization letter must be written by hand on a blank sheet of paper (template available on
Capago Website) then signed by the parents and stamped by the SAPS (South African Police Service). If
both parents are present, the consent letter can be signed in person at Capago. Please note that copies of
both parent’s passports must be attached to the consent letter along with latest 3 months bank statements
and employment letter of one of the parents. It is also essential that the parent's passports feature their
respective signatures which are located either on the hardcover information page (new SA passport) or on
the information page at the back of the older SA Passport.
� Marriage Certificate (If Applicable)
Applicants must please provide a copy of their marriage certificate (or that of their parents if they are minors).
If it is not in English or Italian, it must be translated and legalized by the closest Italian Consular Authority
from where the marriage was registered. If the parents of the child are not married and a marriage certificate
is not available, please add an affidavit to this affect that is signed and stamped by the SAPS (South African
Police).
� Letter of Responsible Adult for School groups
Letter from school or organisational body confirming the identity of the adult(s) accompanying the tour that
will be responsible for the school group.
� Declaration Letter (only in Johannesburg)
For minors travelling with parents, the Italian Consulate needs a letter from parents (Declaration Letter
template on Capago website), declaring that they are travelling with minors. This Declaration Letter have to
be signed by both parents and validated in our office in front of our Capago Representative
FOR UNIVERSITY/COLLEGE STUDENTS, PLEASE ADD:
� Letter from School / University / College
A signed & stamped letter on letterhead declaring that applicant is still a pupil/student currently registered at
the School/University/College.
ITALY - BUSINESS - SHORT STAY VISA
This checklist applies to all persons travelling to Italy for Business purposes on trips equal to or less than
90 days.
For all Business visa cases please provide:
� Passport
Cannot be older than 10 years. SA Passport must be valid for at least 3 months from date of return. Must

� Travel Insurance *
Single page summary of international travel insurance policy certificate to be provided. Must display name of
insured person, policy number, amount of cover (minimum €30,000) and duration of cover. The travel
insurance coverage must be a minimum of double the days of travel in the Schengen area. Travellers
requesting validity greater than 6 months must ensure that the insurance covers the entire period of travel
and not just the total days. Insurance must cover the entire Schengen territory, therefore the term
"Schengen" or "Worldwide" should appear on the policy certificate. Insurance should cover evacuation,
repatriation and emergency transportation as well as emergency in/out of hospital medical treatment.
� Proof of funds

� Proof of Business Invitation
Signed professional business letter of invitation from company in Italy. It must be on a letterhead and
indicate an outline of the business to be conducted in Italy. Dates and locations of all meetings should also
be stated. Please note that all business letters should be accompanied by the Italian Chamber of Commerce
documents (Visura Camerale) to qualify for a business visa.
� Proof of accommodation
Confirmed bookings of all hotels/apartments/rentals/backpacker's etc. It is essential to have 100%
accommodation for the entire period of stay in the Schengen area. Applicant's name needs to appear on the
accommodation bookings. If booked by a 3rd party, please have them provide a copy of their I.D. and a letter
as confirmation that you are their guest. Applicants visiting Italian Citizens or Temporary Residence holders
in Italy must have a Host Declaration form completed by their host and supported by copy of their Italian
Permit/I.D. If an applicant owns property in Italy then they must please provide either the Title Deeds or
Lease Agreement of that property supported by a recent utility bill from the last 3 months.
� Chamber of Commerce / Visura Camerale
These official documents from the Italian Chamber of Commerce, known as a Visura Camerale, must
accompany all business visa applications.
� Proof of Residence
Recent utility bill, bank statements or telephone account from the last 3 months that features the name of
applicant and the physical address. Title Deeds, bank letters/statements and telephone accounts can also be
supplied as proof of residence. If residing with a 3rd party, please attach it to the proof of residence, a copy
of their I.D. as well as a signed letter confirming that you reside with them.
PHOTO SPECIFICATIONS

2 colour photos must be submitted.

The photos must be less than six month old.

The photos must be of a 3.5 cm X 4.5 cm size.

The size of the head in the picture, measured from the top of the hair to the bottom of the chin
should be approximately 3 cm. That is, the face should make about 80% of the total photo area.

If possible, the ears should be exposed.

Photos must be in colour showing natural skin tones.

The background must be either white or a very light colour. Anything else is not acceptable.

Glasses or other eyewear of any kind that affects the appearance of the person is not allowed. To be
on the safe side, it is best to remove your glasses before having your photo taken even if you
normally wear them.

Face masks and veils, even if traditional are not permitted.

The photos should not have any flash reflection on the skin.

Their eyes should not appear red in colour as often happens with flash photography.

Hair must not obscure, even partly, a clear view of the person’s eyes.
PROCESSING TIME

7-10 working days from appointment date.
SPECIAL NOTES :
Non-South African Passports must have a permanent residence permit sticker in their passport and must
provide the original Permit of Permanent residency issued from the Dept of Home Affairs. The Consulate
reserves the right to request further documentation.
For minors who do not have an unabridged birth certificate please provide :

Copy of receipt from Home Affairs showing that the unabridged certificate has been applied for.
Letter from the child’s school stating BOTH biological parent’s names
Affidavit from each parent stating they are the biological parent’s of the child and that they have
applied for the certificate and it will not be ready by travel date.
5. If the child does not attend a school, any other evidence the parent’s can provide will be considered
(eg. Medical aid card with all family members names on it).
