How to create an Amendment via TierBase Creating an amendment to an AEC protocol via TierBase can be used for a number of different functions. These include; Answering conditional revisions Applying for a minor change Applying for a modification Please find listed below directions for creating an amendment via TierBase, and also the submission requirements. How to create an amendment Step 1: Log onto TierBase Step 2: Click on ‘List my AEC protocols’ Step 3: Select the protocol you wish to create an amendment and click ‘Modify Selected’ Step 4: Select ‘Create Project Amendment’ 1 Step 5: Click ‘Create amendment for AEC xxx/0x’ Step 6: Click ‘Duplicate AEC xxx/0x’ Step 7: Click ‘OK’. A duplicate copy of the original protocol has now been created. 2 Step 8: Please complete the ‘Ethics Amendment’ form to justify your amendment and also details any animal welfare issues introduced by this amendment. Conditional Revisions: Please include details of the revisions required by the committee in the justification section. Minor Change: Please indicate that you are applying for a minor change in the justification section. Please outline what the change is, i.e. swapping one strain of animals for another without increasing the total number of animals required, and why it is required. Modification: Please indicate you are applying for a modification in the justification section. Please give a small report on the progress of the protocol so far, including the number of animals approved and the number used. Please outline what the modification is and why it is required. Step 9: Click ‘Save Amendment’. This will take you back to your listed AEC protocols. Step 10: Return to the TierBase main screen. Step 11: Click on ‘List my AEC protocols’ Step 12: You will now see 2 copies of your selected AEC protocol. Click on the copy listed as ‘Revision 1’ and ‘Amendment’. This is an unlocked copy of the original protocol, which can be modified. Step 13: Select the protocol you wish to modify and click ‘Modify Selected’ 3 Step 14: After you have created all of the necessary changes to the revised protocol (and SAVED it), click ‘Show Project Amendments’. Step 15: Click ‘Print Amendment’. 4 Step 16: Click ‘Yes-Print Summary’. This prints a summary document that outlines the changes between the original protocol and the revised one. Step 17 (Only required for Conditional Revisions and Modifications): Click ‘Yes – Print Application’. This will print of a copy of the revised protocol.
