and ___________County School District 21st Century Community Learning Center
I. Background and Introduction
This document defines the relationship of Communities In Schools of _________ County, Inc.
(CIS) and the ____________ County School District 21st Century Community Learning Center
(21st CCLC).
Communities In Schools establishes partnerships between service agencies, corporations,
foundations and the community with schools to effectively address the vital non-academic
resources and services that youth need to succeed in obtaining an education.
Through this program, partnerships are built, bringing comprehensive resources and services
such as food, clothing, school supplies, parent involvement activities, health and dental care, job
shadowing, workforce preparation and college access onto the school campus to help meet the
non-academic needs of children. CIS uses existing resources efficiently and leverages funding to
effectively provide students with the CIS Five Basics:
 A one-on-one relationship with a caring adult;
 A safe place to learn and grow;
 A healthy start and a healthy future;
 A marketable skill to use upon graduation; and
 A chance to give back to peers and community.
The _____________ County School District 21st Century Community Learning Center seeks to
improve high school completion rates by offering a range of academic supports and enrichments.
As part of this mission, 21st CCLC provides non-traditional credit opportunities, tutoring, credit
recovery, after-school activities, healthy snacks, a safe environment and transportation to enrolled
students.
II. In support of their mutual goals, Communities In Schools of ___________ County and
the ______________ County School District 21st Century Community Learning Center
agree to cooperate as follows:

The 21st Century CLC will:
1. Regard CIS as a stakeholder in all activities related to drop-out prevention, workplace skill
development, college access, service learning and other non-academic student achievement
issues.
2. Co-apply and co-implement the 21st Century CLC grant.
3. Employ staff to serve as CIS Site Coordinators at designated 21st CCLC sites.
4. Provide access to necessary student data
5. Cooperate in the meeting of all standards required for the CIS Total Quality System.
6. Serve on the CIS Board of Directors.
Communities In Schools will:
1. Recognize 21st CCLC as a stakeholder in all CIS activities
2. Co-apply and co-implement the 21st CCLC grant
3. Train, support and supervise CIS Site Coordinators
4. Connect community resources with students enrolled in the 21st CCLC to address basic needs
5. Implement workforce preparation activities such as business etiquette, job shadowing, career
coaches, internships and other activities as needed
6. Implement college access activities
7. Provide service learning opportunities
8. Cooperate in the meeting of all requirements of the 21st CCLC grant
9. Provide access to necessary student data and program evaluation
10. Provide matching resources and fund raising to support grant
11. Serve on the 21st CCLC Advisory Committee
III. Duration
This agreement is effective (insert date) for the duration of 12 months, and will automatically
renew for subsequent 12-month terms in the absence of notice by either party 30 days prior to the
renewal date.
IV. Modifications
This agreement may be amended at any time by an agreement in writing executed by authorized
representative of Communities In Schools of ___________ County and the _____________
County School District 21st Century Community Learning Center.
