CONSULTANT IN GASTROENTEROLOGY

Applications are invited for an enthusiastic individual to join a dynamic GI service in Forth Valley.
The postholder will replace a full time colleague within a team of 5 Consultant Gastroenterologists
and 1 Associate Specialist.

The successful candidate will be required to contribute to the general Gastroenterology service and
specifically to support the existing luminal and endoscopy services however in addition the
development of a subspecialty interest would be welcome. Existing sub-specialty roles within the
department include inflammatory bowel disease, hepatology, viral hepatitis, nutrition and upper GI
cancer. A contribution to the ERCP service would be welcomed but is not essential.
The GI service functions as a fully integrated unit with all members contributing to a team-based
approach to patient care and this appointment will play an important role in the continuing
development and redesign of Gastroenterology services. The Gastroenterology senior staff
comprises five consultants, one associate specialist and five specialist nurses. We have permanent
funding for 1.6 wte IBD nurses in addition to three established viral hepatitis nurses. We have a fully
equipped endoscopy department with 4 rooms (including one screening room) and an endoscopy
team consisting of colorectal and upper GI surgical colleagues as well as five nurse endoscopists.
We currently have a joint GI Medical / Surgical ward for complex inpatient care (hepato-pancreatobiliary disease, IBD, acute GI bleeds) and weekly upper GI / HPB / colorectal / IBD MDT.
It is expected that there will be a contribution to acute general medical receiving (1 in 15) in the first
instance although with anticipated recruitment into acute physician posts this commitment from
gastroenterology is likely to diminish.
NHS Forth Valley provides acute services for a population of 310,000 people in Central Scotland.
Acute Services, emergency and complex care, and planned services are provided for at Forth Valley
Royal Hospital (FVRH). Relocation of our acute services into this new hospital site at FVRH was
completed in July 2011.

BACKGROUND TO NHS LOCALLY
NHS Forth Valley has a new acute hospital, Forth Valley Royal Hospital in Larbert, which is
supported by a network of four community hospitals in Stirling, Falkirk, Clackmannanshire and
Bo'ness. We also have a number of day and inpatient hospitals which provide care for older people
and patients with mental illness.

Forth Valley Royal Hospital in Larbert is a state-of-the-art acute hospital which provides a wide
range of inpatient, outpatient and day services. These include emergency, maternity, children and
acute mental health services for the whole of Forth Valley.
Stirling Community Hospital has a Minor Injury Unit and also provides a wide range of outpatient
services, whilst Falkirk Community Hospital provides a range of outpatient services and also has two
inpatient wards for patients who require rehabilitation, palliative and continuing care. Bo'ness
Community Hospital provides a range of inpatient and outpatients services and is also the base for 2
GP practices. Clackmannanshire Community Healthcare Centre provides a range of inpatient,
outpatient and day services. It is also the base for 3 GP practices.
The NHS is the largest employer in Forth Valley and is committed to ensuring that its Human
Resources policies and procedures are best practice. Amongst the areas currently being developed
by Human Resources are Work Life Balance initiatives taking account new of new legislation e.g.

THE FORTH VALLEY AREA
The working environment at both hospitals is excellent, but of crucial consideration to any
prospective applicants must be the quality of life that Forth Valley has to offer them and their
families. The area has stunning scenery, such as the ‘Trossachs’ and Loch Lomond and is steeped
in history with Callendar House at Falkirk, Stirling Castle, and the Wallace Monument to name a few
of the sites in the area. It also has many cultural and leisure facilities such as the Falkirk Wheel (the
largest canal barge wheel in the world), the MacRobert Arts Centre at Stirling and National Sports
Academy at Stirling University.
Forth Valley lies equidistant between Scotland’s two main cities Edinburgh, and Glasgow. Edinburgh
and Glasgow with their respective airports, are little over half an hour from most parts of Forth
Valley, and can be reached by the fast and reliable trains, from stations in the Falkirk, Larbert and
Stirling.
There are long established undergraduate and postgraduate links between Forth Valley and the
medical schools in Dundee, Edinburgh and Glasgow. The Royal Colleges of Physicians and
Surgeons in both Edinburgh and Glasgow are also conveniently nearby. Stirling University provides
nursing education for Forth Valley and Highland Health Board areas and also supports several other
health-related academic disciplines.
House hunters in Forth Valley are spoiled for choice. The area, offers a large array of established
and new properties in a variety of attractive settings.
Primary and Secondary Education in the area is first class with a large number of schools having
been built or modernised over the past few years. There is also long-established provision for
private education in the area.
The area also is well served for further education. Stirling University attracts a high proportion of
students Scotland and outside the United Kingdom due to its beautiful campus, flexible study
options and its high ratings for teaching and research. There are also colleges based in Alloa,
Falkirk, and Stirling offering a wide range of courses and resources.
In addition to its scenery, history, culture and shops, residents in Forth Valley have convenient
access to a very wide range of leisure and sporting facilities. There are excellent golf courses, there
is easy access to nearby hills and mountains, and outdoor activities including sailing, hang-gliding,
windsurfing and skiing are all on the doorstep.

OVERVIEW OF THE POST
General Medicine and Acute Medical Receiving
Since the centralisation of the emergency services, there has been an average of 37
emergency admissions per day (of all ages) with 25 patients admitted under general medicine.
There is a 20-bedded Combined Assessment Unit (CAU) shared with surgical specialties and

adjacent to the A&E Department. There is a 48-bedded Acute Admissions Unit adjacent to
CAU. Patients admitted to hospital from AAU and triaged to the relevant specialty specific
clinical area (critical care, cardiology, gastroenterology, respiratory, endocrine, stroke or general
medicine (general medical patients over 75 years are cared for by colleagues in Ageing and
Health). There is a large Day Medicine unit on the FVRH site which provides a range of options
for ambulatory management of patients.
Patients likely to be discharged within 48 hours of admission are discharged from the Acute
Admissions Unit by either the Acute Medicine team (4 existing consultant acute physicians) or
the on call team responsible for the admission. Patients likely to stay longer will be transferred
to a specialist inpatient ward. Those patients requiring longer periods of rehabilitation are
transferred to the Community Hospital facilities under the care of colleagues in Ageing and
Health medicine.
There are 15 Consultant Physicians on the Acute Medical Receiving Rota, with a 1/15 ‘on-call’
frequency. There is an active recruitment process for further appointments in acute medicine
that will further dilute the sessional contribution of specialty staff to unselected acute medical
care. This process of refining acute medical care will evolve over the coming months.
Medical Staffing
a) Acute Medicine:
The Acute Medicine service has at present 4 substantive consultants in post, Drs. David
Thetford, Andrea Strang, Dan Beckett and Jennifer Gray.
b) Cardiology:
There are 3 Consultant Physicians with a special interest in Cardiology. These are Dr. Allan
Bridges, Dr. Stephen Glen and Dr. Alistair Hargreaves who contribute to the acute medicine on
call rota. Dr. James Spratt and Dr Catherine Labinjoh are Consultant Cardiologists with no
commitment to acute medicine.
c) Diabetes/Endocrinology:
There are 5 Consultant Physicians with a special interest in Diabetes and Endocrinology; Dr.
Chris Kelly, Dr. Nick Barwell, Dr. John Doig, Dr. Linda Buchanan and Dr. Alison MacKenzie.
d) Gastroenterology:
As outlined above, Drs. David Watts (Clinical Lead), Peter Bramley, Richard Burnham, Stuart
Paterson are currently supported by an Associate Specialist, Dr. Patrick Law.
e) Respiratory Medicine:
There are four Physicians with an interest in Respiratory Medicine, Drs. Stan Wright, Dougie
Morrison, Billy Newman, Fraser Wood and Melanie Cross.
Other specialists supporting General Medicine
There is further support from a group of 9 Consultant Physicians in Ageing & Health, 3
Consultant Haematologists who look after inpatients with haematological and lymphoproliferative
disorders, 1 Consultant Rheumatologist and an Associate Specialist in Rheumatology, 2.5
Consultant Neurologists and 1 Consultant in Rehabilitation Medicine. There are visiting
Consultants in Nephrology and Oncology.

Current Junior Medical Staffing
We are supported by 22 FY1 grades, 8 FY2 grades, and 30 trainees in specialty training.
In addition, we have a team of 11 Advanced Nurse Practitioners and Nurse Practitioners.
Current Gastroenterology Service in Forth Valley
Endoscopy Unit
In FVRH site this currently consists of four ‘state of the art’ endoscopy rooms with dedicated
video-endoscopy stacks and provision for two ERCP sessions per week in the screening room.
Emergency and high-risk endoscopy is carried out in one room each morning Monday - Friday.
One further video-endoscopy stack is available for emergency use in main theatres. Emergency
endoscopy out of hours is provided for by surgical colleagues.
We have currently three nurse endoscopists, trained in upper GI endoscopy, flexible
sigmoidoscopy and colonoscopy with one further nurse trained in upper GI endoscopy, and one
trained in flexible sigmoidoscopy.
Outpatient Services
A regional outreach Hepatology Service is functioning for the treatment and monitoring of
patients with Hepatitis B and C with specialist support nurses.
A nurse-led IBD outpatient service is led by two specialist IBD nurses.
Currently nurse-led Haemochromatosis and/or Coeliac services, and iron deficiency anaemia
and jaundice pathways are being set up. The successful applicant could be involved in the
further development of any of these services.
Specialist Nursing
Hepatitis Specialist Nurses –There are 18 nurse led clinics supporting patients with Hepatitis for
initial assessment, counselling and monitoring of treatment. These include three local prison
establishments, Drug Addiction centres and needle exchange sites, as well as work within
traditional hospital outpatient settlings
Inflammatory Bowel Disease Nurses – This has been supported by 1.6 WTE independent nurse
practitioners commitment to the IBD/colitis workload, providing Telephone clinics, nurse led
outpatient clinics and a helpline, with expansion of this service to all IBD patients in Forth Valley
with opportunities for audit and establishing a comprehensive IBD database.
Nutrition & PEG Service
PEG referrals are handled by a dedicated PEG nurse coordinator with support from consultant
gastroenterologists and allied specialty colleagues. This service is run and supervised by Dr
Richard Burnham
Out of Hours Emergency Endoscopy
This is currently undertaken by GI surgical colleagues.
Imaging Service
Excellent support is offered and facilities include CT and MRI scanning as well as a nuclear
medicine service.

Postgraduate and Developmental Facilities
There is a purpose-built Education Centre on the FVRH site. It houses the Postgraduate Library
and audio-visual facilities and offers opportunities for a wide range of development activities for
all staff groups. This is a modern, high quality facility which is widely used by groups throughout
the country.
A wide range of full-text and e-journals are freely available. Internet access is available in both
centres, and also in consultant workspace accommodation.
There are regular postgraduate meetings in both hospitals. Glasgow and Edinburgh with their
Royal Colleges, university medical schools and postgraduate facilities are within easy reach.
Forth Valley Royal is the site of the Scottish Clinical Simulation centre.
CONSULTANT RESPONSIBILITIES

As agreed with the Associate Medical Director (Medicine), to provide with consultant colleagues
an area service in GI Medicine with responsibility for the prevention, diagnosis and management
of illness, and for the proper functioning of the service;

To provide cover for consultant colleagues during annual and study leave or at such other times
as agreed with the Associate Medical Director (Medicine);

To offer professional supervision, training and appraisal of junior medical, nursing and technical
staff as appropriate so as to enhance role extension and job skills within the Gastroenterology
service in order to improve the service provided to patients;

To motivate staff within the service, through leading by example, and fostering good working
relationships at all levels in line with the principles of the local Partnership Agreements;

To contribute to developments and initiatives within Acute Services as appropriate and as
requested;

To undertake teaching, accreditation and examination duties, and contribute to postgraduate
and continuing medical education activity across the Area service;

To agree and implement an annual medical audit and clinical governance programme with the
Associate Medical Director (Medicine), in line with NHS Forth Valley’s Clinical Effectiveness
Strategy;

To participate fully in consultant appraisal and personal development planning activities;

To ensure the efficient and effective use of Acute Services resources;

To work with Consultant colleagues to ensure the effective and efficient development and
implementation of support systems.
The posts’ requirements will change through time in line with clinical advances and developments in
services provided by the NHS in Forth Valley.
CONTRACTUAL DETAILS

Contracted activities for this appointment will be defined and agreed with the postholder and
Medical Director.

The postholder will be professionally responsible to the Medical Director and managerially
responsible to the General Manager of the Emergency & Inpatient Services Unit.

The terms and conditions of service for the appointments are those determined by the Terms

NHS Forth Valley will require each consultant in to agree with the Medical Director a job plan of
fixed and other commitments, which will be reviewed on an annual basis and revised as
necessary to meet the requirements of the local and national service issues.


This proposal will alternate in rotation (2 weeks out of 9 weeks) with existing colleagues as
‘Gastroenterologist of the week’ (emergency endoscopy cover 0900-1700, inpatient and outpatient
referrals and urgent outpatient activity).

 Criminal Convictions – Protecting Vulnerable Groups (PVG) Scheme
In February 2011 the Scottish Government introduced a new membership scheme to replace and
improve upon the current disclosure arrangements for people who work with vulnerable groups. The
duties of this post requires you to undertake regulated work with children and young people
(individuals aged under 18 years) and/or protected adults (individuals aged 16 or over who is
provided with (and thus receives) a type of care, support or welfare service). Candidates who are
successful at interview for this post will therefore be required to be a member of the PVG Scheme
and consent to NHS Forth Valley obtaining a Scheme Record or Scheme Record Update. Any
failure to disclose such convictions could result in the withdrawal of any offer of employment, and
disciplinary action, including dismissal, may be taken.
 Health Screening
The successful candidate will be required to produce evidence of Hepatitis B and C status prior to
appointment and to undergo the health screening required for the post. Any offer of employment will
be subject to satisfactory Occupational Health assessment.
