Details of the current fee can be obtained from the Explosives Inspectorate www.hse.gsi.gov.uk/explosives. The Competent Authority Document will not be issued, or the explosives which are the subject of the application added to the List of Classified Explosives and Fireworks (LOCEF) (http://webcommunities.hse.gov.uk/inovem/inovem.ti/explosives/grouphome), until the total fee, which will be advised on completion of work on the application, has been paid. This form is to be used for all explosives and desensitised explosives with the exception of: o military explosives as defined; o fireworks conforming to the UN default table; that are to be carried by road in Great Britain, where recognition of explosives classified by the Competent Authority of a country that is NOT a Contracting Party to ADR is required. Separate forms are available for fireworks conforming to the UN default table from whatever source and for explosives, other than fireworks, that do not have a competent authority document. Forms may be obtained from the Explosives Inspectorate at the address shown above or from the HSE website (www.hse.gov.uk). In this context, fireworks are articles designed for entertainment purposes as defined in the UN Recommendations for the Transport of Dangerous Goods / ADR and expected to be assigned on classification to UN No’s 0333 – 0337. If the explosives for which recognition of assignment of classification is required are military explosives as defined under the CDG, application should be made to the Secretary ESTC, Abbeywood, Bristol, BS34 8JH from whom application forms are available. Please ensure that all the relevant sections of this form are fully completed. Where insufficient space is provided on the form additional information may be appended on a separate sheet which should clearly indicate to which section of the form it refers. Applications for more than one explosive may be made on the same form. All applications will be acknowledged and given an HSE file reference. Please quote this reference on any future correspondence relating to the application. Companies House Number If you are a limited company, give your Companies House number, which can be found at www.companieshouse.gov.uk. 3. Address The address which should appear on the CAD is to be given. For a company or other body corporate, the registered address should be given. The address for correspondence, if different, should be given at Section 5 below. Telephone No Fax No Email 4. Declaration (i) The declaration should be signed by the applicant, or an official of the company named in Section 1. This applies even where an application is being handled by an agent or consultant. Unsigned applications will not be processed but returned to sender. Applications without the supporting evidence below will not be processed, but returned to sender You should ensure that you enclose the supporting evidence, for every article / substance whose classification you want to be recognised by HSE. We need to be able to identify that the explosives on your application form are the same as the explosives on your supporting evidence, so please mark each document to identify what the evidence you have supplied is in relation to the check list below e.g. US DoT. If a number of explosives are listed on the same document, please highlight which item relates to the numbered item on the application form e.g. write Item 1 etc next to the correct explosive item, thus clearly identifying your supporting evidence. If the names or part numbers are different to those shown on the classification document to be recognised you should provide alternative auditable evidence to prove that the explosives on your application are those referred to on the National CAD issued by the non Contracting Party to ADR. The explosives should be consecutively numbered to provide a cross-table reference. The name given should be the name under which the explosives will be placed on to the market or otherwise supplied for transport. The name should be the one that appears on the competent authority document (CAD) to be recognised and on the exterior of any outer packaging. Please list any alternative names on a separate sheet in order to provide an auditable trail. 3. The stock, part number or other identifying reference under which the explosives will be placed on to the market or otherwise supplied for transport should be given. Together with the name this will uniquely identify the explosive. Please list any alternative part numbers on a separate sheet in order to provide an auditable trail. 4. The name and address of the manufacturer of the explosive should be given. 5. & 6. Give the UN number and classification code to be recognised. This should be that assigned by the national competent authority of a non Contracting Party to ADR and recorded in the official classification document (CAD) which should be attached to this application together with an English translation where necessary. 7. The national competent authority issuing the document claimed as the basis for classification should be indicated by use of the ISO international code for countries unstats.un.org/unsd/methods/m49/m49alpha.htm. 8. The official classification document reference number should be given and a copy included with the application. Classification will not be recognised unless a copy of the document is received. Part C Packaging Information Information on the packaging of an article or substance is essential since classification is packaging dependent and often specific and limited to the packaging described. You should ensure that these descriptions correspond to those Inner, Intermediate and Outer packaging’s permitted under the relevant Packing Instruction 3. Maximum number of articles/assemblies in each package, as well as any restrictions imposed on the quantity for example if the Competent Authority have said ‘not more than 25 articles per box’. 4. Where safety to transport or the hazard division has been assessed as being dependant upon a particular outer packaging the UN packaging mark for that packaging should be provided. The packaging mark is the UN mark referred to in Chapter 6 in ADR. A copy of the packaging certificate should be attached to this application together with an English translation where necessary. 5. Total mass in kilograms of the whole package and contents, including mass of packaging. 6. Total net explosive mass or content in kilograms in the outer package. 7. Explosives classification is often packaging dependant. Where safety to transport or the hazard division has been assessed as being dependent upon how articles are packed within the inner intermediate and outer packaging’s, the unique reference or specification for that packing method should be listed.
