This class meets 2.5 hours per week for 8 weeks, follows an evidence-based curriculum, and has been endorsed by the NIH as a model of a mind-body intervention. In the early stages of this program, during my academic time, I personally taught several eight-week MBSR class series for patients with chronic illness, and I now supervise a pool of 4 qualified MBSR teachers. I supervise the process of screening and evaluating patients for new MBSR groups of 25-30 Veterans, which start every 1-2 months. To date, over 1,200 veterans have participated in MBSR, and over 350 have enrolled in research studies. UW GME Wellness Program for Residents In January, 2011 I initiated a program intended to decrease burnout, decrease depression, increase professionalism, and increase empathy among physicians in training. With support from the GME office and UWMC, I organized ongoing courses for health care providers, which are held on Sunday evenings at the GME administration building at the South Lake Union Campus. I personally taught the first 8-week series, and the ongoing series are taught by an experienced, trained mindfulness teacher. The project is ongoing. Budget $139,812 1/10/2013 – 9/30/2014 The goal of this project is to promote dissemination of VA best practices, by creating a complete toolkit facilitate the adoption of a standardized mindfulness intervention (MBSR) within VHA facilities. As part of this process we will perform qualitative research on veteran stakeholders and obtain input from other experts who have experience teaching mindfulness to veterans. As an initial experience in dissemination, we will initiate MBSR at a new site – the Boise VA Medical Center. 2. VA CSR&D Merit Review: A Trial of Loving-Kindness Meditation and Cognitive Processing Therapy for PTSD. Role: Co-PI with Tracy Simpson, PhD 1I01CX000857 Total direct costs: $551,941. This non-inferiority trial will assess whether a novel CAM intervention, Loving-Kindness Meditation (LKM), results in reductions in PTSD symptoms and depression that are not meaningfully worse than an accepted VA PTSD treatment (Cognitive Processing Therapy; CPT-C). In the trial 170 veterans with PTSD will be randomized to LKM or an active control (CPT). Comprehensive assessments will be performed at baseline as well as at the end of the 12-week intervention and 3 and 6 months later. Anticipated start date of 2/24/2014. 3. VA HSR&D Merit Review Award (I01): A Non-Inferiority Trial of MBSR and CPT for PTSD. HX-12-019 Role: Co-PI with Tracy Simpson, PhD Project ID: 1 I01 HX001081-1A2 Total Direct Costs $ 575,476 4/1/14 – 3/31/17 The goals of the study are to evaluate the non-inferiority of Mindfulness-Based Stress Reduction (MBSR) as compared to Cognitive Processing Therapy (CPT-C) for veterans with Posttraumatic Stress Disorder (PTSD) in the context of a randomized controlled trial with three-month post-treatment follow-up. PTSD symptoms and quality of life will be assessed. Potential moderators of change will also be evaluated, and semi-structured qualitative interviews will be conducted. If MBSR is shown to be equivalent to CPT, it would provide evidence to support offering MBSR as an additional treatment modality for people with PTSD. Anticipated start date 4/1/2014. Merit Review Award, in response to VA-ORD RFA for Research on New Treatments for Gulf War Veterans’ Illnesses. “A randomized controlled pilot study of a mindfulnessbased intervention for Gulf War Syndrome”, Co-P.I.’s: David Kearney, MD (medicine) & Stephen Hunt, MD (medicine) Budget $196,408 10/1/10 – 9/30/13 The goals of the study are to establish feasibility, and collect efficacy data for a standardized course in Mindfulness-Based Stress Reduction for Veterans with medically unexplained illnesses (Gulf War Syndrome) in the context of a randomized controlled trial. Recruitment is ongoing. 2. “An Open Pilot Study of Loving-Kindness Meditation for PTSD” P.I.: David Kearney, MD (medicine), co-investigator, Tracy Simpson, PhD (mental health). In a before and after pilot study of 43 Veterans with PTSD, we are evaluating a practice intended to increase self-compassion for the treatment of PTSD. The study measures PTSD symptoms, 24-hour urinary measurement of stress hormones, and objective measures of attention. We enrolled 43 Veterans who took part in 3 groups; each group met once per week for 12 weeks, and we are now analyzing the results. Funded through intramural funding from SIBCR. $31,132. 3/1/10 – 3/1/2011 3. American College of Gastroenterology: Pyrosequencing to Identify Changes in Intestinal Microbiota Following a Stress-Reduction Course. (P.I. Kearney). This ongoing study will pilot the use of broad-range bacterial 16S rRNA gene PCR with 454 pyrosequencing to characterize bowel microbiota, and determine if decreases in stress change the composition of the microbiome. American College of Gastroenterology: The Effect of Mindfulness Training on Eating Behaviors and Food Intake (P.I. Kearney). Aims: To assess feasibility and outcomes of 25 overweight or obese Veterans who take part in a mindful eating intervention (Mindful Eating and Living - MEAL).
