Bridge of Earn Student Welcome Pack This induction pack has been devised to give you some useful information about your Primary Care Placement and to help you settle in to the environment quickly and safely. Welcome Welcome to Bridge of Earn / Abernethy placement: Address: Phone numbers: Nurses House, 1 Station Road, Bridge of Earn, Perthshire PH2 9EA. 01738 812314- District Nurse 01738 813722- Health Visitor On arrival in the base you will be welcomed by whichever staff member is available and introduced to the other members of staff. You will be shown around the building paying particular attention to fire exits and fire extinguishers, policies and procedure manuals, toilets and changing facilities as well as kitchen facilities. After initial orientation to the building you will be reminded about confidentiality and conduct and identification badge checked, before being included in the discussion about the daily work. There will be a discussion about the student’s expectation and learning needs from this placement. If your named mentor is not available, you will be allocated to another appropriate member of staff, after discussion with you. During the first few days you will be introduced to the other members of the Primary Health Care Team in Bridge of Earn. You will also have the roles and responsibilities of each member explained to you and an orientation to the areas covered by the team. Discussion about practice based and geographical working will be explained. During your placement if you feel things have not been explained adequately, or you have misunderstood, please feel free to ask any member of the team where we will endeavour to answer your questions. This placement works on mutual respect and open communication so feel free to make suggestions out with our listings of placements if you feel it will enhance your learning in the placement. Practice Information Bridge of Earn Practice Under 5’s 5-18 18-64 65-75yrs 75+ Practice Population: 160 465 1837 336 297 Total number of patients: 3095 The Surgery is situated at corner of Main Street (opposite Pillar Box); it is staffed by two Doctors, some locum doctors, one Practice Nurse, one health care assistant as well as several reception staff. The District Nursing Team and Health Visitor visit daily to communicate any concerns or to collect any instruction from the other members of the team. There is also a monthly Primary Health Care Team meeting on the second Tuesday of each month. This is attended by the multidisciplinary team and patients’ care is discussed, packages of care are planned and any other business can be discussed. The surgery is open 8.30am- 6.pm every weekday. When staffing permits, the surgery will be open till 7pm on alternate Tuesdays and Wednesdays. Abernethy Surgery Practice Population: 0-4yrs 5-24’s 25-64 65-74yrs 75+ Total number of patients: 194 541 2,238 451 355 3,779 The surgery in Abernethy is open three mornings a week, staffing levels permitting. It is a satellite clinic of Newburgh Surgery. The Child Health Clinic and immunisation clinic is now held in Newburgh. Students are allowed to sit in with GPs during consultations after the GPs and the patient’s permission has been obtained. We encourage students to ask questions and will always try to answer them. However, we will expect the students to assist with their own learning by developing a questioning approach. We will treat students as individuals acknowledging their needs and encouraging them to feel part of the care team by involving them in assisting and directing healthcare professionals in providing patient care. We will recognise and value the contribution of all students in the process of planning and carrying out patient care and assist them to achieve/maintain knowledge and skills in order to provide safe and effective care. WHAT YOU CAN EXPECT FROM US You will receive an induction into your work area to ensure you are familiar with the environment and are able to practice safely. You will discuss your learning needs and outcomes at the beginning of the placement. We will provide a safe and healthy environment which is conducive to meeting identified individual student learning needs. During your placement you will be allocated a mentor to work alongside. The mentor will be a qualified practitioner who will assist and support you during your clinical work. Your mentor will assess your performance against your course learning outcomes and provide feedback to help you develop your skills. You can expect to have an initial, interim and final assessment/interview with your mentor. You will receive supervision during your clinical practice. You will be a valued member of the multidisciplinary team during your placement and can expect support from all your colleagues. We will listen to your feedback about your placement and will respond to any issues raised sensitively. WHAT WE EXPECT FROM YOU We expect you to arrive on time for planned shifts and any other activity identified by the Mentor or delegated supervisor. We expect you to be courteous, respectful and show motivation to learn. We expect you to ensure your Mentor is aware of your learning outcomes for the placement and specific learning needs. We expect you to act in a professional manner. We expect you to dress in accordance with the University/NHS Tayside uniform policy. You should inform your Mentor or delegated person if you are unwell, not able to attend your placement or have to leave unexpectedly. If you are unable to contact someone directly please leave a message on the answer machine. We expect you to maintain and respect confidentiality at all times. This applies to clients, their records and discussions between the student and the Mentor. If you encounter a difficult situation whilst visiting patients you should call for help via mobile phone or house phone if possible. Any incidents should be reported to your mentor and the relevant incident form completed as soon as possible. We would like you to raise any issues regarding your placement with your mentor. If this is not possible you should contact your Learning Team Facilitator. Learning Opportunities/Programme After the first few days you will be encouraged to discuss your learning plan with your mentor and arrange with them a suitable programme with other relevant disciplines keeping your mentor aware of any visits you may have arranged along with times and dates. The district nurses work as part of a wider multi-disciplinary team including specialist nurses. We encourage students to spend some time during their placements with the Practice Nurses, Health Visitors, and School Nurses etc. to gain insight into the varied roles of members of the Primary Care Team.
