This questionnaire provides an opportunity for you to provide feedback to the Learning Exchange. Please take
time to complete the questionnaire and return it to Lin Williams or Lorraine Wright by 9 October 2015 at the
Headteacher meeting. Please only tick boxes which relate to activities with which you have previously
engaged.

The Learning Exchange will be reviewing the offer fully for 2016-17 to ensure it meets your needs. For that
reason your feedback is so important. Additionally, each aspect of the offer for 2016-17 will reflect Learning
without Limits and the priorities locally, regionally and nationally. There is a space at the end of each section
for any additional activities you would like us to consider or suggestions you would like to make for the offer.
