JOB DESCRIPTION Job Title: Senior Pharmacist, Parkside, Haemato-oncology Liaison Responsible and Accountable to: Clinical Services Manager, Cancer Centre London Hours: Full time Base: Cancer Centre London ( CCL) Job Summary To support the expansion and development of medical oncology and haematology services and across Parkside Hospital (PH) and CCL. To ensure the same level of clinical input at both sites ensuring optimal care of oncology patients, and meet the standards outlined in the NHS Manual of Cancer Standards and Healthcare Commission Standards. To ensure all Parkside pharmacy staff have a minimum oncology skill set. Plan and coordinate the education and training of oncology pharmacy staff. Support the day to day running of the Oncology Pharmacy at CCL and maintain a professional input into all clinical Pharmacy activities concerned with oncology. Key working relationships Clinical services manager, CCL – for professional and managerial support, planning of workload and expansion of services Pharmacy manager, PH – for training and development of PH clinical pharmacy oncology services to meet specific needs of oncology patients, to be responsible for the clinical pharmacy services to oncology in-patients Senior Oncology Nurses, PH and CCL, to develop a partnership with senior oncology nurses at Parkside and develop minimum shared standards for all oncology clinical practices. 1 Consultant haematologists/oncologists – support all prescribing and medicines management aspects of oncology patient care Main Duties – Medical Oncology/Haematology Development To work within the clinical services team to ensure the initiation, implementation and development of a clinical pharmacy service to Oncology and haemato-oncology inpatients To support the Clinical Services Manager in the development of a seamless oncology service between the two sites ensuring standardization of staff, expertise, documentation and services. Coordinate the development of clinical pharmacy standards for cancer care across the two sites. Attend daily weekday ward rounds and ensure effective communication links between the oncology ward and chemotherapy unit, PH Pharmacy and CCL Pharmacy Ensure harmonization of cancer and palliative care clinical pharmacy services, which are based on research, knowledge and experience, in order to raise and maintain standards of care. Represent CCL at Infection Control and other key clinical meetings . Coordinate improvement of clinical oncology pharmacy services through shared experience and joint management and project progression. Undertake regular meeting with the Parkside Hospital Pharmacy Manager to ensure cohesive development of haemato-oncology In-patient services. Provide pharmaceutical input to the development of pharmaceutical care plans, across site clinical information systems, policies and procedures. Education and Training Develop a framework for a minimum level of expected clinical pharmacy service to cancer patients liaising with the Pharmacy Manager, PH. Train all PH Pharmacy staff to be able to perform at this level. Develop oncology IP service and develop ownership and management of these by PH staff Co-ordinate teaching/educational sessions for Oncology nursing and pharmacy staff at PH and CCL. Train CCL Pharmacy staff in the liaison haemato-oncology role Assist and evaluate the cancer related training of inpatient clinical staff at Parkside Hospital. 2 Support the development of Mosaiq, the electronic prescribing system across both sites. Clinical Governance In conjunction with the CCL Clinical Services Manager and PH Pharmacy Manager, support the co-ordination of the pharmaceutical contribution to peer review visits. Ensure that the resulting action plans are agreed, the necessary actions are undertaken and progress regularly monitored. In conjunction with the CCL Clinical Services Manager and PH Pharmacy Manager , coordinate standardization of chemotherapy protocols and other drug policies across the two sites. Support the CCL Clinical Services Manager and PH Pharmacy Manager , develop safe and effective medicines management policies for oncology patients Ensure the implementation and maintenance of the relevant sections of the Manual of Cancer and Health Care commission Standards. Initiate and co-ordinate appropriate audits relating to best prescribing practice and pharmacy cancer services. Medicines Management Support the Clinical Services Manager and PH Pharmacy Manager, in planning and strategic processes. Help identify cost pressures and service developments. Identify prescribing anomalies cross site, resolve prescribing problems and support the standardization of medicines usage Assist in the monitoring of high cost drugs for cost capture, horizon scanning and managing drug budget. Assist in providing financial reports on drug usage/expenditure across the two sites. Provide appropriate drug forecasting information and new drug evaluation.
