social service programs. A recent grant funds Homeless Housing Services (HHS) for adults with a
mental health or other disability. Their Pathways Program is an emergency service clearing-house
for people with housing, food, job, and other needs. They also sponsor the Linkage Program
whose street caseworker connects homeless people with services. Then Conference assists with
medical prescriptions for people without resources to fill them, The Conference also has a
“Repetitive Payee” program to assist those on SSI (including children) who need help to manage

Description of Services: CRR (County-funded, apartment based program) (3-4 consumers per
apartment. 2 Sites: More intensive program site has staff 24 hours a day and serves 10
consumers. Usually a 1 year long program and consumers can go to the moderate level.
Moderate Intensive Program: has staff for 10 hours per day and serves 28 consumers. To qualify
for this program the consumer needs to have more independent living skills. This is a transitional
program to independent living. Rent based on income. Consumers need to participate in some
form of a day programming at least 3 days per week, which includes: work, partial hospitalization,
clubhouse, etc.

Description of Services: Provides hotline for homeless youth ages 12 to 17. Valley Youth House
has offices in Allentown, Bethlehem and Easton. Counseling for families and youth, family
preservation; independent living program for youths on their own (therapy, supervised residence);
runaway and homeless youth shelter for ages 12 through17; Has transitional housing for youth also.
Their Street Outreach Program caseworkers provide food, clothing, referrals and advocacy to
residents in both Lehigh and Northampton counties. Also available are Student Assistance Program
(SAP) for mental health counseling through Northampton County school districts.
