From September 1979 to 1987, the department was headed by Prof. M.B.Rozumenko. From 1988 until 1998 the department was headed by MD, Prof. M. I. Zhyliayev. In 1998, the department was headed by MD, Assoc. Prof. N.M.Oliynyk. Clinical facilities of the department include maternity hospital, women's consultation with 450 visits per day, gynaecological department of Ternopil municipal hospital № 2. Education In 2008 the department introduced training courses for physicians, obstetriciangynaecologists on the following thematic cycles: “Emergency aid in obstetrics and gynaecology" (one-month cycle). In 2009, it was introduced the training cycles "Prenatal diagnosis" (one-month cycle), "Colposcopic diagnosis of the uterine cervix pathology" (one month cycle). In 2010, it was introduced the training cycle "Operative Obstetrics" (one-month cycle), pre-certificate training of the doctorstrainees, courses for professional improvement. Each year Department of Obstetrics and Gynaecology conducts international scientific-practical conferences on "Actual problems of modern obstetrics". Methodological meeting of the Department of Obstetrics and Gynaecology of the Faculty of Postgraduate Education. The department carries out visiting pre-certificate cycles for doctors who visit the courses, in Rivne region, and the Department conducts pre-certificate courses for doctors-obstetricians of Ternopil, Lviv, Volyn, Zhytomyr, Kyiv, Ivano-Frankivsk and Transcarpathian regions. Since 2008, the department has worked out 12 curricula: for pre-certificate cycle for doctors, visiting the courses, doctors-trainees, interns of the I-II years of study, for doctors visiting the courses in "General practice - family medicine", "Surgery", "Emergency aid in medicine". Shadrina conducting a lesson for interns in the room of colposcopic diagnosis of the uterine cervix pathology The department has developed the following courses for thematic improvement "Prenatal diagnosis”, "Physiological pregnancy" for doctors visiting courses in “General practice – family medicine", "Emergency conditions in obstetrics and gynaecology", "Operative Obstetrics", "Gynaecological Endocrinology", "Emergency aid at terminal conditions in obstetrics and gynaecology”. New methods of teaching, such as lectures with the usage of telecommunication, have been introduced into the educational process, test computer programs are widely used, educational computer CDs in "Practical Gynaecology”, “Obstetric phantom” (in Ukrainian and in English) have been created. All lectures for interns and doctors visiting courses are delivered with the usage of multimedia technologies. MD, Professor A. Yu. Franchuk, delivering a lecture Research The department staff investigates the effectiveness of new drugs for treating inflammation of the uterine cervix, prevention and treatment of complications in menopause of women. The department continues to study the effectiveness of new drugs for the treatment of TORCH - infections in gynaecological patients and during pregnancy. Each year 8-10 scientific articles and abstracts are published in various scientific journals in Ukraine and 2-4 articles and abstracts in educational methodical work. Since 2000, the department staff has participated in the integrated interdepartmental work on the following topic: “Develop and implementation of effective methods and means of diagnosis, treatment and prevention of women's reproductive disorders and perinatal health of the foetus”. Department of Obstetrics and Gynaecology is the founder of scientific and practical journal "Actual problems of Paediatrics, Obstetrics and Gynaecology" published by "Ukrmedknyha Publishing House" in I. Horbachevsky Ternopil State Medical University which has been included in the list of scientific professional journals in Ukraine, where the results of scientific works on medicine can be published. Prof. Boychuk A.V. is the editor of the journal; Assoc. Prof. Behosh B.M. is the scientific secretary of the journal. Participants of the conference "Actual problems of modern obstetrics" in educational-recreational complex “Chervona Kalyna” Over the past 5 years at the Department of Obstetrics and Gynaecology according to the scientific topic "Pathogenetic substantiation of ovarian function disorders and their treatment in women with extragenital pathology, the study of severity of disorders of ovarian function in women with chronic diseases of the gastrointestinal tract and urinary system" four candidate theses have been defended. Scientific Interests of the Department • Theoretical basis and new solution of theoretic and practical tasks of gynaecology, which is characterized by improvement of diagnostic algorithm and development of a staged differential pathogenetically reasonable correction of hypothalamic-pituitary disorders in women with thyroid dysfunction. • Improvement of complex therapy and supervision tactics of postovariectomy syndrome on the basis of development, clinical testing and implementation in practice of pathogenetically grounded phased use of hormonal and herbal medicine due to its positive effects on hormonal, lipid, calcium-phosphorus metabolism, homeostasis and bone mineral density. • Improvement of methods of diagnosis and prevention of growth retardation in pregnant women engaged in chemical production by enhancing adaptive mechanisms in order to reduce the obstetric and perinatal complications. • On the basis of studies of neuroendocrine condition and structural-functional state of bone tissue pathogenetically foregrounded use of hormone therapy and therapy by calcium containing drugs has been developed, clinically tested and introduced into practice due to its positive effects on hormonal and calciumphosphorus metabolism and bone mineral density. Clinical work One of the most important indicators of the work of maternity units is maternal mortality. One of the main reasons for maternal mortality in Ukraine is obstetric bleedings. The department staff suggests teaching emergency aid in puerperal bleeding and use of advanced technologies in organ- preserving postnatal operations on the uterus – dressings of the major vessels. Owing to the developed measures no cases of maternal death from obstetric bleeding have been registered. Five organpreserving operations at massive postoperative haemorrhage have been performed during the period of 2010-2011. Assoc. Prof. Koptiuh V.I. is consulting a pregnant woman in the delivery room of the family type in Ternopil municipal hospital № 2 A great contribution of the department staff concerning treatment is the introduction of laparoscopic surgical techniques on the uterus, fallopian tubes and ovaries in 2008. The positive effect of the interventions on the fallopian tubes and ovaries was estimated by the number of pregnancies. During this period 24% of women became pregnant (i.e. every 2-3 woman). The following scientific achievements have been implemented in medical practice in Ternopil municipal hospital № 2 A method of ovulation restoration in women with endocrine infertility accompanied by subclinical hypothyroidism, prevention of bone loss in women after histerectomy and ovariectomy, prevention of intrauterine growth retardation in pregnant women at risk by means of "Artichoke extract - Health". A method of treatment of hyperprolactinemia and galactorrhea in women with different content of biologically active prolactin with subclinical hypothyroidism; screening method for assessing the degree of estrogen deficiency; a method of prevention of atherogenic changes and thromboembolic complications in women after histerectomy and ovariectomy, a method of diagnosis of osteopenic changes in women after ovariectomy; a method of pregravid preparation of women with prenatal losses in case history. A method of preventing growth retardation syndrome in pregnant women at term 12 - 16, 24-28, 32-36 weeks of pregnancy if early signs of placental dysfunction are present; a method to reduce the frequency of obstetric and perinatal complications in women with anomalies of labor activity with placental dysfunction; a method of preventing placental dysfunction in pregnant women at high risk for the development of obstetric and perinatal pathologies. Diagnosis of placental failure in pregnant women with diffuse toxic goiter by determining placental lactogen and content of α-fetoprotein in serum of venous blood; monitoring algorithm for pregnant women with diffuse toxic goiter. Perspective areas of the department are: - Implementation of new perinatal technologies to reduce maternal and perinatal losses. - Implementation of reproductive technologies in women with infertility using laparoscopic techniques on the fallopian tubes and ovaries. - Implementation of video and computer processing of colposcopic images of pathology. Creation of educational video library of colposcopic pictures of precancerous processes of uterine cervix. Since 2010 a new research area has been worked out: "Study of extragenital diseases influence on menstrual function, study of the impact of medical complexes that regulate metabolic disorders on the state of the foetus, pregnancy and childbirth in women with metabolic syndrome and miscarriage". Method of uterine arteries embolization has been implemented into practice. The principle of uterine arteries embolization aims at reaching vessels involved in myoma blood supply, using a special catheter tube controlled by X-ray technology, and at blocking, "sealing" these vessels by small balls, emboli. Emboli are inserted into the vessel through a catheter. Deprived of nutrition and blood, myoma ceases to grow and decreasing in size and eventually dies, turning into a scar on the uterus. The principle sphere of scientific research Rehabilitation of patients with tubal infertility after laparoscopic intervention, treatment of background diseases of the uterine cervix, gynaecological endocrinology accompanied by the diseases of thyroid, complex therapy and supervision tactics of patients of reproductive age with uterine leiomyoma.
