Patient Education Grant Application Instructions Thank you for your interest in submitting a request for funding to Tibotec Therapeutics. This document provides information on the required documentation for Patient Education grant requests and step-by-step directions for submitting requests through our online application system. IMPORTANT NOTE: As of January 1, 2009, Tibotec Therapeutics will only accept grant applications during certain time periods during the year. These grant application windows are outlined on the www.ttgrants.com web site. Please review these timeframes prior to submitting your grant request. General Considerations for All Patient Education Grant Requests Requests to support live events or printed materials (fact sheets, binders, etc) produced for patients should be submitted as a Patient Education grant requests. Please be sure to review the Eligibility page of the www.ttgrants.com web site to see if your grant request aligns with the Areas of Interest we will consider for funding. Eligibility: Organizations eligible to submit PATIENT EDUCATION grant requests include: Academic Institutions Medical Societies CME Providers Medical Education Companies Hospitals Patient Advocacy Organizations Community-Based Organizations NOTE: Tibotec Therapeutics does not provide grants to individuals or private physician practices. Exclusions: Educational grants cannot be used to support: Programs that have already occurred. Food and beverage expenses at non-accredited programs Promotional activities related to Tibotec Therapeutics products. Development of treatment guidelines. Normal organizational overhead, staff training, or provision of medical care. Gifts and/or incentives for attendees of non-accredited educational programs. Travel, lodging, or honorarium for a healthcare professional to present a paper or poster. Educational activities for which attendee (patient or healthcare professional) travel and/or lodging is paid by the meeting organizers. This includes attendee scholarships and travel vouchers. Marketing sponsorships or exhibit opportunities. Educational grant requests that include marketing sponsorship or exhibit opportunities will be denied. Other Considerations: For non-accredited healthcare professional education and patient education programs, content related to Tibotec Therapeutics products must be within FDA-approved labeling. General Submission Guidelines Grant applications are only accepted during the months of January, March, May, July, September and November. The submission deadline and notification period will depend on the date of your program. Incomplete submissions will result in a processing delay and/or denial of the request. Submissions must be made via the online application system. Faxed and/or mailed submissions will not be accepted. You will receive confirmation of your submission by email. Receipt of a grant application by Tibotec Therapeutics does not guarantee approval of your request, nor financial support for the entire amount being requested. If your grant request is approved, a copy of the Letter of Agreement will be sent to the requesting organization for signature. Disclosure of financial support through an educational grant from Tibotec Therapeutics must be made to program participants. If your grant is accepted, the Company reserves the right to post the information regarding funding under this agreement on a publicly accessible web site. This includes the identity of the recipient, the monetary value of the funding, the purposes for such funding, and other information as the Company determines appropriate. Step-by-Step Instructions for Submitting a Patient Education Grant Request Step 1: Prepare a signed Letter of Request on organizational letterhead. You will be uploading an electronic version of this document as part of your online grant application. Your letter of request will not be considered unless it includes the following information: Title and date of the event, and the amount of funding being requested. Organization's 9-digit tax ID number. A timeframe for the educational program to be developed and executed. A statement as to whether Tibotec Therapeutics will receive the right to use the materials produced for the educational programs for patients. Statement verifying that decisions involving the selection of faculty will not include Tibotec Therapeutics. Statement verifying that any representations of Tibotec Therapeutics products during the educational activity will be within FDA-approved labeling. Assurance that there has been no condition of purchase, use, or recommendation of Tibotec Therapeutics products associated with this request for funding. Name and address to which the check should be made payable and mailed if your request is approved. Step 2: Prepare a detailed Budget for the entire program (not just the portion for which you are requesting funding). You may use your own budget template or the template provided on the Forms page at www.ttgrants.com. The budget must include the following breakdowns (where applicable): Meals - provide breakdown of cost per meal, per attendee/faculty/speaker. Faculty travel costs – provide breakdown of airfare, transportation and lodging per speaker, per day/night Honoraria – provide breakdown per speaker Step 3: Complete the Patient Education Grant Application Supplement Form (available on the Forms page of the www.ttgrants.com web site). All sections must be completed in order for the application to be considered complete. This form will be uploaded as part of your online grant application. Step 4: Complete an IRS Form W-9 (Request for Taxpayer Identification Number and Certification). The information provided on the IRS Form W-9 must match that of the organization that would receive funds if the grant were approved. Step 5: Confirm you have all of the required documents ready in electronic format for your grant submission. All documents should be in Word, pdf, Excel or jpg format. Signed Letter of Request on Organizational Letterhead Detailed Program Budget Patient Education Grant Application Supplement Form IRS Form W-9 Any other attachments you would like to submit as part of your request. (Program brochures, agendas, etc) Step 6: Log onto www.ttgrants.com. Click the “Submit Request” tab on the upper right hand side of the home page. Click on the “Start Application” link to begin the online application process. Upload the Letter of Request and Budget documents to their corresponding fields. The Patient Education Grant Application Supplement form should be uploaded to the field marked “Program Information/ Agenda/Objectives”. The IRS form W-9 and any other documents you would like to submit should be uploaded on the lines designated “Other”. Step 7: Submit your request. You will receive an automated email reply with your Request ID number. Step 8: You will be notified of the status of your grant request during the applicable “Notification Period” posted on the www.ttgrants.com web site.
