Health Guarantee and Contract

We have made every effort to provide you with a healthy puppy. We guarantee this puppy is purebred and
registerable with the AKC. All puppies are sold with limited AKC registration papers unless other arrangements
have been made. Breeding a puppy with Limited registration is not an option. Upon purchasing a puppy with
limited registration from PR Boxers you are agreeing to spay/neuter your puppy by 7 months of age. Failure
to comply will be considered a breech of contract, and will void the Health Warranty. We guarantee that all
vaccinations and worm medications are current as stated on the provided vaccination schedule. Purchaser is to
have this puppy examined by a licensed veterinarian of their choice within 72 hours of taking possession of
this puppy. If the sales transaction takes place on Friday and Monday is a holiday, Purchaser is given an extra 24
hours. Proof of this exam MUST be sent to Breeder within 15 days of taking possession or The Health
Guarantee is Void. If this puppy is found to have a life threatening hereditary congenital defect at this exam, a
replacement puppy of equal value will be given, or the entire purchase price of the puppy will be refunded,
(Purchaser’s choice), provided that the Breeder is notified within 24 hours after the exam and proof and
documentation is sent to seller via conventional or email, and the puppy is also examined by a veterinarian of
breeders’ choice. This puppy must be returned to the Breeder in good condition, other than the stated defect, along
with the AKC registration papers within 24 hours, at no expense to the Breeder, with a signed document from the
licensed veterinarian stating the defect or problem. Breeder will not be responsible for any veterinarian or shipping
bills incurred by the Purchaser. This guarantee does not include injuries, contagious diseases, illnesses or
disorders that are or could be caused by environmental factors such as allergies, immunity levels, auto
immune disorders, bloat, pneumonia, heatstroke, mite related illness, ringworm, heartworm, fleas, ticks,
conjunctivitis, ear or urinary tract infections, coccidia, giardia, or intestinal worms/parasites, as they are
common in dogs and puppies and treatable. Also not covered are improper bites, un-descended testicles,
hernias, cherry eye, elongated palate, or innocent class I or II murmur. Breeder will not be responsible for
Parvo, Distemper or Cornona Virus, or Kennel Cough after 72 hours of the sale, as these are highly
contagious diseases and they can be contacted immediately after leaving the Breeder. Breeder will not be
responsible for illness or health problems, which are diagnosed after this time period, except where stated in
this contract.
Your puppy is guaranteed for one year from the date of birth against life threatening congenital defects only. If
within that time a life threatening congenital defect (not to include parvo virus, distemper virus, heartworms or
any above mentioned or nutrition related disease) is found by a licensed veterinarian and/or by a veterinarian of
breeders choice, the Breeder will replace the puppy with another puppy of same breed, quality and price. The
replacement will be subject to availability (within 12 months). The puppy will be replaced and no moneys will be
refunded. The Purchaser must provide, at Purchaser’s expense, a written statement from a licensed veterinarian. No
exceptions. Breeder will not be responsible for any veterinarian bills incurred by Purchaser. Breeder will not be
responsible for illness or any health problems, which are diagnosed after these time periods. In the unlikely event of
this puppy’s death in the first year, due to congenital defect, a detailed statement and copies of any tests and results
must be received from a veterinarian confirming such an event and cause before the Breeder will take any action. If
cause is unknown, an autopsy is required. Warranty is void in the event of inconclusive or undetermined diagnosis.
The Breeder will not be responsible for veterinary or medical bills incurred by the Purchaser.
Your puppy has been on a vaccination and de-worming schedule here and is up to date and these records will be
provided to you. Please take these records to your veterinarian, as it is very important to continue with these
vaccinations and preventive care after you take your puppy home. Please do not forget the annual booster after that.
You, the Purchaser, agree to insure your puppy’s good health and to keep our guarantee in effect by taking good

care of the puppy and providing quality, premium food, housing and medical care. Purchaser further agrees to
refrain from exposing the puppy to public places until the full preventive puppy vaccinations series has been
completed, and to postpone strenuous, repetitive exercise until after their Boxer is one year of age. Failure to do so
will void this Guarantee. Please remember that puppies are babies and they need lots of tender loving care! This
contract is valid only for the original purchaser of the puppy. If the dog is transferred to another owner this contract
is null and void.
If Purchaser requests delivery, it shall be by airfreight, C.O.D. to a designated location. Purchaser is responsible for
costs of crate, insurance during shipment, and cost of freight charges. If Breeder has to purchase crate prior to
shipping, Purchaser shall reimburse Breeder in full for this cost. All costs including full purchase price must be
received 14 days prior to shipping or pickup date to allow funds to clear bank, unless payment is made in cash or by
wire transfer which are the preferred methods.
The Breeder makes no guarantees, expressed or implied, under this contract except as stated above.
By signature below, Purchaser acknowledges receipt of a copy of this agreement and information package
including vaccine schedule. Purchaser further agrees that they understand this contract and that if conditions are not
fulfilled this contract is null and void. Purchaser acknowledges that all risk of loss pertaining to the mobility,
illness, or injury of the puppy shall remain upon purchaser. Purchaser agrees to indemnify and hold harmless PR
Boxers, the Johnson's and their estate, from any and all loss, damage, and expense whatsoever from the acts of any
kind whatsoever, any and all claims, presentation, or demand from any person whomever, including purchaser,
arising out of or having to do with the prior or present care, breeding, training, mortality, capacity, and health of
said puppy. Any dispute of this agreement will be subject to the jurisdiction of the state of Kansas, county of
Osage; and that the venue shall be in Osage county, Kansas. Purchaser will be responsible for all legal costs and
attorney fees involved in the dispute.
We hope that we have helped you to choose the right puppy for you and your family. If for any reason this puppy
does not work out for you, and you are unable to provide this puppy with a happy home, you are required to
notify us first for assistance in rehoming and to submit new owner contact info for approval or relinquish the
puppy to us (no monies will be refunded). Under no circumstances is the puppy to be relinquished to any pound,
shelter or rescue facility. Our boxers are bred with love and devotion and no matter what their age, their welfare is
our main concern and they are always welcome in our home. We can be reached at (785-220-0590).We always

you for any assistance we may provide. Enjoy the new member to your family!
Purchaser agrees that PR Boxers may continue to use the puppy's name, images, and other information in
advertising for PR Boxers. Ownership of the puppy will be transferred to the Purchaser for the sale price of
$__________ to be paid as follows: $_________ deposit on execution of the Agreement with the balance of
$__________ to be paid in cash on or before the puppy leaves the Breeders premises. If the balance is not paid by
_______________________, this Agreement shall be cancelled and the Purchaser forfeits the deposit.
