PART TIME METASTATIC BREAST CARE NURSE – POSITION DESCRIPTION Reports to Support Services Manager Job Functions Client support Provide advanced breast care management from diagnosis, through treatment, rehabilitation, follow-up and palliative care (where appropriate) in accordance with recognised clinical practice guidelines. Provide up to date specialised breast care information, education and support to clients regarding treatment modalities, supportive care and practical assistance relevant to their needs and in accordance with evidence based clinical practise guidelines. Demonstrate comprehensive knowledge of informational resources for breast cancer clients and facilitate access to resources relevant to client needs and preferences. Ensure that the patient and their family has access to support, education and information across the disease trajectory through facilitation of communication and continuity of care. Assist clients to prepare for surgery and treatment and provide clinical insight and consultancy in medical and technical matters. Facilitate efficient, timely and comprehensive assessment of current and potential emotional, psychological, physical and practical needs of clients with breast cancer, their families and carers. Provide clients with appropriate referrals as required. Identify and support clients at higher risk, or exhibiting high levels of psychological distress and make appropriate onward referrals. Advocate for clients and promote a client’s ability to participate in care decisions and selfmanagement of health needs, according to the client’s preferences and resources, while ensuring the client has access to appropriate information on which to base decisions. Continuously assess the client’s understanding of their clinical circumstances, treatment and care plan, and preference for information, support and resources. Demonstrate comprehensive and advanced knowledge of the pathophysiology and progression of breast cancer, current evidence regarding breast cancer treatments across the care continuum and the short and longer term effects of breast cancer and its treatments. Programs Work collaboratively with the team to identify gaps in breast cancer related services. Identify and recruit peer supporters; manage and monitor one-to-one relationships. Process financial assistance applications (internal and external) and update the budget and client database accurately. Conduct assessments for practical assistance and refer to the Volunteer Coordinator or external service provider/s. Refer clients to Pamper Days and Re-Fresh activities and share hosting duties. Breast Cancer Click (The Click) Provide online support and information to The Click members. Respond to questions and moderate peer advice and information posted on The Click. Provide clinical insight and consultancy in medical and technical matters. Collect statistical data and contribute to reporting requirements for The Click. Awareness and Education Maintain a high level of clinical expertise and knowledge through participation in ongoing education and professional development activities. Recognise boundaries of own knowledge/expertise and ensure clients are referred to additional support and areas of expertise. Regularly update staff on new developments in the area of breast cancer Provide articles for the newsletter and website. Deliver breast cancer training to new staff. Provide support to rural cancer nurses. Networking and Community Involvement Establish a working relationship and work collaboratively with multidisciplinary team members across the continuum of care. Liaise with general practitioners, specialist oncologists, cancer care coordinators, community nurses, palliative care and local hospital staff in order to build an efficient and appropriate referral network and framework of support for breast cancer clients. Maintain professional and courteous relationships with external service providers. Actively promote Breast Cancer Care WA in order to attract new clients and service providers. Represent Breast Cancer Care WA and/or McGrath Foundation at community events and networking functions. Administration and General Maintain accurate case management records in accordance with professional standards. Collect statistics and provide narrative reports for the Board and sponsors. Maintain client database. Attend regular clinical supervision and debrief with the Services Manager as required. Attend Breast Cancer Care Services team meetings. Contribute to the ongoing management of the Resource Register. Investigate and identify potential new resources for inclusion. Have a flexible attitude to working hours, particularly in relation to moderating The Click and attending after hours events and functions. Facilitate the provision of superior customer service to all Breast Cancer Care WA stakeholders. Maintain regular consistent and professional attendance, punctuality, personal appearance and adherence to relevant health and safety procedures. Provide support to other projects of Breast Cancer Care WA as assigned by the Services Manager. Qualifications Post graduate certificate in Breast Care and/or Oncology/Palliative Care Nursing (Diploma or Masters qualification desirable) An Australian Health Practitioner Regulation Authority (AHPRA) Registered Nurse Essential criteria 1. 2. 3. 4. 5. 6. 7. 8. 9. 10. 11. 12. 13. Demonstrated specialist knowledge in the clinical area of patients with advanced breast cancer across the disease trajectory. Comprehensive oncology/palliative knowledge Experienced in oncology, palliative care or breast cancer nursing Demonstrated ability to coordinate and deliver complex patient care for patients and their families. Demonstrated ability to work in and with a multidisciplinary team. Demonstrated project management, organisation and planning skills. Demonstrated commitment to total quality management and improved health care outcomes. High level interpersonal and organisational skills. Demonstrated commitment to developing and improving personal education skills appropriate to the position. Demonstrated knowledge of and commitment to Occupational Health & Safety. High level of computer literacy High level verbal, written and interpersonal communication skills Current WA Drivers Licence and reliable vehicle. Desirable criteria 1.
