Statement of purpose We aim to: Provide equitable, free, care and respite for adults (over 18 years) of Pembrokeshire with life limiting illness Our objectives are: To provide a variety of care to meet the physical, spiritual, emotional and social needs of the whole person To provide support for family and friends especially those in the carer role To provide access to our team of doctor, nurses and occupational therapist. To offer symptom management e.g. pain, shortness of breath, fatigue and others To anticipate problems from an holistic perspective and ensure early recognition of need To supplement and coordinate community resources on behalf of the patients and their families and/or carers To liaise with the multidisciplinary team outside of Shalom House to provide the highest quality and broadest range of care To provide access to a range of complementary therapies to enhance holistic care To encourage patients to be independent and fully involved with their care To provide a safe, secure welcoming environment where patients feel safe and valued To provide an evidence based approach to practice and to be involved in education training and research 1 The staff: Medical Director (HIW Responsible person)Dr Robert Jones MB ChB (Birmingham 1982) Dip Pall Med (Cardiff 2010) Worked as an anaesthetist from 1984. From 1992 was responsible for in hospital pain management including palliative care pain management in a district general hospital (Redditch). In 2000 became lead clinician for chronic pain running a pain clinic and gained certificate of basic competence in acupuncture (British Medical Acupuncture Society). 2003, moved to Pembrokeshire continuing as an anaesthetist with an interest in pain management specialising in palliative care pain management. Started as a volunteer at Shalom House in 2007, employed as the Medical Director, Shalom House from 2009 and achieved the Diploma in Palliative Medicine (Cardiff) in 2010. Manager and Palliative Care Nurse Mrs Helen Price RGN level 1 1983 Introduction to supervisory care 1997, Community Health Diploma 1991, Welsh National Board Diploma in Palliative Care and Terminal Illness 1997, D32 & D33 NVQ Assessor in Nursing Care 1997, Postgraduate Certificate in Palliative Care (Cardiff) 2010. Worked in community nursing including end of life care from 1990 onwards. Started at Shalom House 2007 and appointed as Manager, Shalom House 2009. Her nursing experience in the Pembrokeshire community is a great asset. Palliative Care Nurse Mrs Allyson Burrows RGN level 2 1977, level 1 1992. Diploma teaching and learning practice 2000, Nursing care and lymphoedema practice 2003, Management lymphoedema 2003, Diploma Indian Head Massage 2005, BSc Palliative Care 2005. Worked in a number of hospices before moving to Pembrokeshire and joining Shalom House in March 2009. Her expertise in lymphoedema is a great asset to us. 2 Occupational Therapist Mrs Jenny Mathieson Worked in the community for seven years where the majority of the patients had palliative conditions. In 2001, began working with patients receiving chemotherapy (the majority palliative therapy). In 2007, began work at Shalom House and in 2008 transferred hospital work to the palliative care ward. Registered Nurses: Sarah Bennett, Amanda Jones, Helen Harries Health Care Assistants Sharon Barrow, Isobel Bowen, Grace Thomas The following are Registered Nurses on a “bank” system working shifts as requiredLinda Frampton, Louise Wiseman, Jay Moss, Deanna Tennacour, Donna Philips, Jayne MacGregor, Catherine Fortune, Lyn Hunter, Susie McQuin. The following are Health Care Assistants on a “bank” system working shifts as required- Helen Williamson, Catering Manager and cook- Julie Archer Administrator - Sarah Burns Cleaner - Helen Jeremy We have a team of volunteers, Complementary Therapists, General volunteers and volunteer Drivers. All undergo Criminal Records Bureau enhanced disclosures and work within our policies. The organisation structure: Shalom House is a registered charity (number 1117564) and company (number 5437976). Company Secretary: Gwyn Griffiths The Medical Director (Dr Robert Jones) and Manager (Mrs Helen Price) report to the trustees. The Service: At Shalom House care is planned and co-ordinated by all the clinical staff who meet in a weekly Multidisciplinary Team Meeting (MDT). The meeting is documented on the All Wales Palliative Care Record system (CaNISC). Dr Jones attends the weekly Hywel Dda (Pembrokeshire) Palliative Care MDT where all aspects of patient care are discussed and integrated pathways of care planned. This ensures that the care of patients at Shalom House is integrated with the wider Hywel Dda care plan for the patient. Dr Jones is also a member of the Pain Team at Withybush Hospital; he can call on their expertise as required. Jenny Mathieson works part time at Shalom House and Part Time at Withybush Hospital this allows her to co-ordinate Occupational Therapy aspects of care across Hywel Dda (Pembrokeshire). All staff and volunteers work within their training, competence and the facilities provided by Shalom House. Shalom House has adopted All Wales Code of Conduct. Shalom House encourages and supports training of staff so that they can fulfil their role. 4
