Clarification of TASKS
(vertical columns)
STUDENT CONTACTS
1. Individual Counseling - record number of sessions for students seen individually. An individual counseling
session should consist of active listening, identifying and defining problem(s), discussing alternative
solutions, and formulating a plan of action. No time minimum is required.
2. Group Counseling/Guidance - record number of sessions conducted and the number of participants that were
included. This includes all small and large group work.
3. Classroom Guidance - record all guidance activities by number of sessions lasting 15 minutes or more and the
number of students participating.
PARENT/STAFF/COMMUNITY CONTACTS
1. GCPS Staff Consultation or Education - record number of staff consultations which reflect interaction with
staff concerning student/school needs and the number of participants. (This includes individual sessions
and RTI/SST/IEP meetings, etc.) Record number of staff educational sessions (staff development)
conducted and the number of participants.
2.
Parent Consultation or Education - record number of telephone or personal conferences with parents
and/or counselor-led parent education meetings, and the number of participants. (This category may
include discussion with parents regarding their options for outside referrals, lunch and learns, parent
evening programs, phone and person-to-person conferences, etc.).
3.
Community Consultation or Education – record number of community consultations or community
workshops/educational programs you lead and the number of participants. (This included consultations
with DFACS, private psychologists on behalf of students, KidsNet, GCPS Board attorney, presentations, etc.)
4.
Law Enforcement Consultation – record number of consultations with local school SRO or other police
departments. Whether consultation is with an individual or a group, record number of participants.
(this includes, probation officers, SVU and any other special police)
TASKS
1.
Professional Leadership – record number of minutes spent in leadership activities such as an after school
club sponsorship, athletic coach, local school committee chairperson, district level committee
participation, professional organization leadership, representing counseling department on School
Advisory Council, head and lead counselor activity hours, professional presentations, etc.
2.
Program Planning – record number of minutes spent in planning counseling program initiatives. (planning
classroom guidance and other group activities, planning parent programs, planning 21C3 initiatives within
your counseling department, creating student/parent/faculty evaluations, Red Ribbon Week coordination,
input monthly report, meeting organization, Peer Leadership organization, etc)
3.
Staff Development – record number of minutes you participated in staff development. (Conference
attendance, college coursework, GCPS and other staff development related to counseling )
4.
Fair Share – record number of minutes spent participating in activities such as test administration, bus
duty, hall duty, etc.
CRISIS INTERVENTION REPORT
The initial contact with the student reporting a personal CRISIS is reported by number in this section. Follow up
sessions with the same students would be recorded in the Individual Counseling/Life Skills category under
STUDENT CONTACTS.
Abuse – Physical – report number of students reporting extreme physical abuse resulting in a report from you to
Department of Family and Children’s Services (DFACS) on behalf of this student.
Abuse – Neglect - report number of students reporting serious neglect in their home resulting in their safety
and/or primary needs not being met (i.e. food, shelter). Report only those students for whom you have
made a referral to Department of Family and Children’s Services (DFACS) on behalf of this student.
Abuse – Sexual - report number of students reporting sexual contact/abuse/exploitation in their life for whom
you have made a referral to Department of Family and Children’s Services (DFACS) on behalf of this
student.
Abuse - Emotional – do we want to add this?
Homeless - report number of students who enroll in your school as homeless and those who become homeless
while enrolled in your school(with whom you have contact). Report each referral made to DFACS or
school Social Worker for one student as a separate contact.
Pregnancy – record the number of girls who report to you that they are pregnant. This number will include
DFACS reports that you make for students under the age of 16 years. After this initial documentation,
all further contacts will be recorded in Individual/Group or Parent consultation, etc.
Rape – record the number of students who report to you that they previously or recently were raped. Reporting
ANY rape to your School Resource Officer (SRO) is required. After this initial documentation, all further
contacts will be recorded in Individual/Parent/Law Enforcement/ Faculty consultation, etc.
Self Harm - report number of students who report behavior that is dangerous to their own well-being. (this
includes, but is not limited to, cutting, eating disorders, dangerous sexual activity, substance abuse)
Student Death(s) - report number of students who die as a result of natural, purposeful, or accidental causes.
This does NOT include student who are distressed as a result of a student death.
Suicide Attempted – report number of students who attempt self harm (NOT include threats). Record number of
actual suicide attempts, even if a student makes multiple attempts.
NOTE: Crisis incidents are NOT tied to a student’s name in the portal.
