combined effects on the risk of several chronic diseases, including type 2 diabetes,
hypertension, coronary heart disease, heart failure, stroke, and cancer;
 Insulin resistance, glucose tolerance, the metabolic syndrome and the risks of type 2 diabetes,
cardiovascular disease and cancer;
 The independent and joint effects of diabetes, myocardial infraction, stroke, hypertension and
other cardiovascular risk factors on the risk of cardiovascular mortality;
 Coffee and alcohol consumption and the risk of several chronic diseases, including type 2
diabetes, hypertension, coronary heart disease, heart failure, stroke, and cancer;
