Procedures for Changing Service Provider
1. Communication Plan
a. Current service provider will inform parents,
LEA/AEA staff and student of the change in service
provider (no matter what time of year).
b. New service provider will communicate with parents,
LEA/AEA staff and student prior to the transition
taking place.
2. Review/Update student notebook
a. Summary page
b. Progress monitoring updated
c. Technology/Materials list updated
3. Coordinate with new TVI/OMS
a. Identify service hours required per week
b. Identify/Discuss transition date
c. Identify student strengths
d. Identify student concerns/needs
e. Identify/discuss IEP goals
f. Review progress monitoring
g. Review assessment due dates
h. Review technology/materials provided
