We hope to use this site as a way of presenting all our patients with an up to date resource for all
information relating to our practice. We will keep it current with any news, developments &
details that are relevant to the practice & our patients.
As well as a source of information about our Practice, the staff & the services we provide, we
hope you use the website as a useful resource with links to other reliable websites for health
related information. If there is topical health information we will also add this to the website.

Disabled Access
Wheelchair and pram access is via the Grove Road entrance. Please ring the bell and one of our
staff will assist you.
Surgery Opening Hours
Mondays to Fridays 8.30am-6.pm.
Routine appointments are usually available within one working day.
Extended Opening Hour
Patients who genuinely are unable to attend surgery during the normal opening hours ,can prebook an appointment with Dr. Wood on a Tuesday evening between 6.30 and 7.30pm and on a
Thursday morning between 7.30 and 8.00am. These appointmentss must be pre-booked.

Please call only for acute problems that cannot wait until opening hours - 8.30 to 6pm. You may
be asked to attend the out of hours facility at Warwick Hospital.

NHS Direct
In the case of urgent need when the practice is closed you can call NHS Direct
(www.nhsdirect.nhs.uk/) (111) to speak to a triage nurse. Your needs will be assessed and advice
offered or arrangements made for you to see a doctor.
Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery
re-opens.
Call 999 in an emergency. Chest pains and / or shortness of breath constitute an
emergency.
Consultations
Patients are registered with the Practice and may see the doctor of their choice, subject to
availability.
Consultations are by appointment and can be made in person or by telephoning 01789 414942
during the centre’s opening hours.
If you require treatment that day and there is no free appointment, you will be seen at the end of
surgery. You may have to wait.
Home Visits
If you are too ill to come to the surgery and require a home visit, please telephone the surgery
as early as possible to help us plan our Home Visits.
Home visits are at the discretion of the doctor.
Telephone Consultations
Telephone consultations with the doctors are available by request. Please let the receptionist have
an indication of your reason.
Urgent phone calls are taken at anytime by the doctor but please do not abuse this facility.
Interruption during consultation is distracting for both patient and doctor.
The receptionist or nurse can often help you with information, advice and the results of tests
which the doctor will have already seen.
Please ensure that you are available to contact on the telephone number you provide.
Repeat Prescriptions - Allow 2 working days for this process.
These are for medicines taken on a regular basis and authorised by the doctor. All prescriptions
are now computerised.
When ordering always try and use the counterfoil issued with your previous prescription. This
shows your unique prescription number and full details of your prescribed medicines. It will also
tell you when to make a review appointment.
Repeats can be obtained:




NEW Patient Access: Register for ordering repeat medication on line as well as booking
and cancelling appointments.
Personally: tick the medicines required on your counterfoil and drop into post box in
reception or out of hours, place through front door post box.
Via our website: This is our preferred method for ordering prescriptions. (Phasing out
this option due to Patient Access, please register for this service, ask at reception for
details or click on !!Patient Access!! to the right of this screen)

If you are ordering on line and would like your pharmacy to collect from the surgery, you
will need to telephone them directly to avoid delay in courier services.
Prescriptions ordered will be ready for collection in 48 hours.
Prescriptions Charges and Exemptions
Extensive exemption and remission arrangements protect those likely to have difficulty in paying
charges (NHS prescription and dental charges, optical and hospital travel costs).
The NHS prescription charge is a flat-rate amount which successive Governments have thought
it reasonable to charge for those who can afford to pay for their medicines. Prescription
prepayment certificates (PPCs) offer real savings for people who need extensive medication.
NHS charges
From 1 April 2009, the charges are:



Prescription (per item): £7.65
12-month prepayment certificate (PPC): £104.00
3-month PPC: £29.10
If you will have to pay for four or more prescription items in three months, or more than 14 items
in 12 months, you may find it cheaper to buy a PPC. The charge for a single prescription item is
£7.65, whereas a three month PPC will cost you £29.10 and a 12 month PPC £104.00.



Your request for a prescription has now been acknowledged. Please allow 48 hours, excluding
weekends and Bank Holidays, for your request to be processed. Any problems please telephone
the surgery.

We run the following clinics at the medical centre;
Diabetic Clinic
Nurse clinic on Wednesdays 9.00am – 11.00am
- By appointment only
Antenatal Clinic
Wednesday afternoons by appointment run by the Practice Midwife.
Family Planning & Cervical Smears
Both doctors are fully qualified and trained in all aspects of family planning. Contraceptive
services are available during all surgery hours. Cervical smears are organised on a 3 year recall
system by computer for women aged 20 – 65. These are carried out by the practice nurse or
doctor, by appointment. Patients outside this age group can have a smear after discussion with a
doctor or nurse.
Teenage Health Clinic
Dr Crowfoot and the Practice Nurse run a Teenage Health Clinic on Mondays (except Bank
Holidays) from 12.00 – 2.00pm. and Wednesdays 1.00 – 4.00pm No appointment is necessary.

Childhood Immunisations & Travel Jabs
Full immunisation courses are recommended for all children, unless there is a specific reason for
not doing so. Please book an appointment with the practice nurse. The nurse will also advise on
health and immunisation for travellers abroad. As soon as your departure dates are known please
make an appointment with her. Ideally 8 weeks notice is required to ensure complete cover, but
even those departing at short notice can be offered some protection. Some vaccinations are not
available on the NHS and may incur a fee
SEASONAL FLU CLINIC
The seasonal flu clinic will commence at the beginning of October. Please telephone the
surgery during the first week in October to make your annual appointment.See Newsletter.

The practice has a strict policy regarding confidentiality and data protection.
We will only release test results to the person to whom they relate unless that person has given
prior permission for the release of this data or they are not capable of understanding the results.

Due to building restraints our list is restricted to a minimum intake at the present time. If you are
registering with the practice please bring with you a utility bill which shows your present address
and if recently from overseas, let us have sight of your passport or identify card.

The practice complies with Data Protection and Access to Medical Records legislation.



To provide further medical treatment for you e.g. from district nurses and hospital
services.
To help you get other services e.g. from the social work department. This requires your
consent.
When we have a duty to others e.g. in child protection cases Anonymised patient
information will also be used at local and national level to help the Health Board and
Government plan services e.g. for diabetic care.
If you do not wish anonymous information about you to be used in such a way, please let us
know.
Reception and administration staff require access to your medical records in order to do their
jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.

We make every effort to give the best service possible to everyone who attends our practice.
However, we are aware that things can go wrong resulting in a patient feeling that they have a
grieveance. If this is so, we would wish for the matter to be settled as quickly, and as amicably,
as possible.
The Practice has a complaints procedure. In the first instance, you should contact the Practice
Manager and she will set all the necessary wheels in motion.
Further written information is available on the complaints procedure from reception. We are
continually striving to improve our service. Any helpful suggestions would be much appreciated
and a suggestion box is located in the waiting area.

The practice considers aggressive behaviour to be any personal, abusive and/or aggressive
comments, cursing and/or swearing, physical contact and/or aggressive gestures.
The practice will request the removal of any patient from the practice list who is aggressive or
abusive towards a doctor, member of staff, other patient, or who damages property.
All instances of actual physical abuse on any doctor or member of staff, by a patient or their
relatives will be reported to the police as an assault.

Allow access to a suitably qualified medical professional within 48 hours of your initial
contact during surgery hours, or in an urgent case, the same day.
Work in partnership with you to achieve the best medical care possible.

The prevention of disease, illness and injury is a primary concern.
The medical team are here to assist and inform you of the steps you can take to promote good

Let us know if you intend to cancel an appointment or are running late.
Treat staff with courtesy and respect. Reception staff may have to ask some personal
questions to assist us in providing you with the best service
Inform the practice staff of any alterations in your circumstances, such as change of
surname, address or telephone number. Please ensure that we have your correct telephone
number, even if it is ex-directory.
As patients, you are responsible for your own health and that of any dependents. It is important
that you adhere to information and advice given to you by health professionals, and co-operate
with the practice in endeavouring to promote good health.

Doctors
Dr Nigel
Wood
MB, BS, MRCP, DRCOG
Qualified in 1978 from St Thomas Hospital, London (MB, BS). He became a
member of the Royal College of Physicians in 1981 (MRCP) and then left
hospital medicine for General practice. He obtained his obstetric exams
(DRCOG) and his Family Planning certificate in 1985. He spent 3 years in a
group practice and left to start the Arden Medical Centre in 1989.
Dr Jill
Crowfoot
MB, ChB, MRCGP, DRCOG Qualified in 1993 from the University of
Birmingham Medical School (MB, ChB). She has worked in many of
the hospitals around the West Midlands and in 1995 obtained her
obstetric exams (DRCOG) and her certificate in Family Planning. In
1997 she entered General Practice and in 1998 became a member of the
Royal College of General Practitioners (MRCGP). In April 1999 she
joined the surgery and works part-time.
Dr Hazel
Blanchard
Works as Assistant GP. She joined Arden Medical Centre in March
2003. Dr Blanchard qualified in 1997 from Leeds Medical School and
since then has acquired her Obstetric and Family Planning certificates,
as well as her MRCGP. She has worked in many of our local hospitals.
Nurses
Our practice nurse is available 5 sessions a week. As well as nursing
duties she is able to discuss and advise on your health concerns. She
offers a medical check to all patients newly registered with the practice.
In addition she gives advice on smoking cessation, alcohol intake and
diet. Our aim is to reduce the incidence of coronary heart disease and
stroke. Nursing appointments are made at reception.

The receptionists are often your first contact with the practice - making
appointments, delivering messages, receiving requests for repeat
prescriptions and generally looking after you. They have a difficult job but are
always trying to help. All our staff are trained in confidentiality and the patient
medical computer system.

Diane Menage Diane is the community midwife attached to Arden Medical Centre.
