Why East Lancs?
The East Lancashire Hospitals NHS Trust and Primary Care Trusts provide services to the
population of East Lancashire of about 510,000 people. Geographically it covers the Blackburn,
Burnley, Hyndburn, Pendle, Ribble Valley and Rossendale areas.
The posts are based in the Royal Blackburn Hospital and Burnley General Hospital. The Trust
offers a comprehensive range of “in” and “out-patient” services, including psychiatry and
outpatient clinics from a range of visiting specialities by tertiary referral. There are full diagnostic
and scanning services available. Both sites are also fully supported by Postgraduate Education
facilities, fully equipped libraries and subsidised staff gyms.
We have also found that 88% of East Lancashire Foundation Trainees have achieved their first
choice of Specialty Training posts!!!
The Trust has undergone a major new review involving relocation of services between the two
hospitals and now includes The Lancashire Women and Newborn Centre which is the culmination
of more than five years’ work and brings together all of East Lancashire’s Women’s and Newborn
services, enabling the delivery of the best possible care. The state-of-the-art building located on
the Burnley General Hospital site includes:
• A Central Birth Suite providing hi-tech facilities for complex births
• An adjoining midwife-led Birth Centre which offers a relaxed environment where healthy women
can go through labour naturally and at their own pace in comfortable rooms with en-suite facilities
• A Neonatal Intensive Care Unit caring for premature newborns from across North and East
Lancashire and providing high level care including whole-body cooling treatments for the most
poorly babies
• A purpose-built Gynaecology Unit incorporating separate areas for services such as oncology,
urogynaecology, fertility treatment and termination of pregnancy
Royal Blackburn Hospital
This new build hospital completed in 2006 is located
approximately 1 mile outside of the town centre. There is a
regular bus service which operates past the main entrance.
Acute services are based at this site including emergency
medicine, surgical emergencies, high risk surgery and a helipad. There is an extensive dining room
serving breakfast, lunch and evening meal where specialist diets and out of hours are catered for.
There is also a newsagents, café and subsidised staff gym. In the Learning Centre there is a fully
equipped library, seminar rooms and a large auditorium all fitted with visualising equipment.
There is also a large clinical skills lab with kitchen and a 3G simulation room, with a new state of
the art doll which can be programmed to simulate clinical emergencies.
Burnley General Hospital
This large district hospital is situated 2 miles from the town
centre and a regular bus service operates past the main
entrance. Since the mid 1960’s there has been extensive
development and refurbishment – including the new main
entrance ‘Phase Five.’ Current developments are now complete and Elective Surgery, Maternity,
Gynaecology and the Neonatal Unit is now on this site. There is also a Psychiatry Department which
specialises in Child & Adolescent Psychiatry and an Urgent Care Centre for minor injuries. As a
Foundation Trainee you will be expected to work on both sites as your rotation requires you to.

Teaching
Teaching is held weekly, FY1 – Tuesdays 12.30–1.30 pm (Blackburn & Burnley sites) & Thursdays 25 pm (Blackburn site) and FY2 – Tuesdays 2-5 pm (Blackburn site). Most teaching is in small groups
and is structured to be as interactive as possible. The topics are split between patient safety,
careers and specialities. The teaching evaluation is monitored closely and feedback is expected
each week. There are also allocated Foundation Forums in the teaching programmes, bi-monthly,
to encourage feedback.

Opportunities Available
As part of your Foundation Year 1 you will be expected to have completed Immediate Life Support
a one-day course accredited by the Resuscitation Council (UK) and ALERT (acute life threatening
events recognition & treatment).
As part of Foundation Year Two you will be expected to complete Advanced Life Support a oneday course accredited by the Resuscitation Council (UK).
Shadowing
Before new Foundation Trainees start with the Trust we hold a shadowing period which you will
have a chance to be involved with and have your own trainee shadow you in your work.
Taster Sessions
If you have not had chance during your rotations to gain experience in a department you are
interested in, a week’s taster session can be arranged so you gain this valuable experience and
broaden your knowledge.

Job Descriptions
Integrated Medicine
The department of Medicine has a full team of on-site Consultant Physicians covering General
Medicine, Endocrinology, Gastroenterology, Elderly Medicine, Cardiology, Respiratory Medicine,
Rheumatology, Dermatology, and Genito-urinary Medicine.
You will gain experience in both an in-patient and out-patient setting with appropriate exposure to
emergencies and will be part of a team including Consultants and Specialty Trainees. The FY2
posts are currently attached to General, Rehabilitation and Stroke Medicine. You will be expected
to attend, and participate in, educational activities including grand rounds and audit
presentations. The FY2 rotas are full shift and all are compliant with the EWTD.
The competencies attained in Integrated Medicine will include:
~ History taking
~ Examination and selection of appropriate investigations
~ Clinical skills e.g, lumbar puncture, chest and peritoneal drains and central lines
~ Acute assessment of medical problems
~ Co-ordinating treatment and investigations
~ On-going management of patients including liaison with other teams and discharge planning
Genito-Urinary Medicine / Histopathology / Clinical Microbiology / Infection Control
This innovative FY1 post will involve work both in the GU Medicine Department, Infection Control,
Clinical Microbiology and the Pathology Department. This post will be un-banded.
The learning objectives for these departments will include:
Microbiology:






Learn about the role of the microbiologist in dealing with patient referrals, e.g. the weekly
blood culture results, by shadowing the microbiologist on call, reviewing patient notes and
then discussing cases with consultants in form of mini-CEX/CbD
Understand the treatment and prevention of C.Dificile and MRSA by attending C.Dificille
ward rounds
Extending personal knowledge of infection control by spending a day with the infection
control nurses/team
Understand the role of the microbiologists in various settings within the hospital by
attending ICU ward rounds to review complex patients and the appropriateness of
antibiotic treatment
Review the role of various health agencies such as HPA and their role in communicable
diseases by arranging a day with the local HPA team
Develop the skills needed to analyse samples within the lab by spending a day in the
laboratory with staff shadowing and reviewing culture plates with staff

Pathology/Histopathy:



Learn basic skills in analysing histological samples and become competent in using a
microscope by attending sessions with the histopathology team
Understand the role of pathologists/histopathologists within an MDT setting by attending
any MDT meetings with the histopathology staff and complete a reflective piece of work
Attend a post mortem / autopsy, and understand the role of the pathologist regarding post
mortem examinations
Blood Sciences:


Understand the techniques used to analyse and authorise blood science results by
attending lab sessions with technicians and review blood results with blood science
consultants
Understand how the transfusion system operates by attending the transfusion department
and review notes obtained following transfusion assessments
GU Medicine:






Learn how to take a focused sexual health clinic and perform appropriate examinations by
observing and performing observed history taking and examinations during the weekly
GUM clinics
Develop own knowledge of the treatment of common GUM conditions by producing a
short presentation and quiz to fellow foundation year colleagues
Understand the importance of confidentiality and the measures taken in the GUM setting
by discussing with staff members confidentiality and methods used, and any circumstances
where it can be broken
Understand the role of health practitioners in contract tracing, and the various methods
used by observing contract tracing being completed, and produce a reflective piece on the
advantages and disadvantages of each
Develop skills in HIV pre-test counselling by observing pre-counselling in action, and
complete an assessment with staff
Understanding the role of the community teams within the GUM speciality by attending
family planning clinics at Larkhill, and conducting a questionnaire to help develop services
available for patients
Learning Objectives

Complete an audit, and seek out presentation opportunities where possible. When working
in the various departments inquire as to possible audits already started, or being planned
with a view to complete/help
Psychiatry (Lancashire Care Trust)
The Psychiatry element of the FY1 and FY2 tracks will be fully integrated in the duties and working
patterns of colleagues in the Lancashire Care NHS Trust. All duties are worked on a full shift
system which includes out of hours. The Trust covers a number of sites and provides psychiatric
services to in and out patients, and in addition to working in specialist units you will carry out
assessments in acute settings and in the community, working with General Practitioners. There is a

full supporting Consultant and Middle Grade team, together with hospital and community based
specialist nurses and support workers.
The competencies attained in Psychiatry will include:
 Assessment and management of psychiatric illness
 Management of acute illness (including acute confusional state and acute psychosis)
 Prioritisation of work-load
 Discharge planning
 Risk assessment
 Understanding the life style effects on health from alcohol and drug usage
Paediatrics and Neonates
There are Foundation Year posts available in Paediatrics (FY1) and Neonatology (FY2). The
department offers a wide experience over a range of Paediatric interests and includes exposure to
NICU, paediatric medicine and paediatric sub specialties, including out patients.
There is full Consultant, Specialty Trainee and Specialist Nurse support at all times to all trainees.
The FY1 posts are based in Paediatrics and are un-banded. The FY2 rotas are full shift and
compliant with the EWTD. With the opening of the new Women’s and Newborn unit FY2 posts will
be based in the Neonatal Unit in Burnley.
The competencies attained in Paediatrics and Neonates will include:






Management and care of the acutely unwell child
Resuscitation of the new-born and children
Drug-dosing
Cannulation and airway management of the new-born and children
Vaccination policies
Communicating with children and parents
Obstetrics and Gynaecology
The department delivers care to over 6,000 women per year. There are 13 Consultants working
within the department whose special interests cover maternal medicine, foetal medicine, minimal
access surgery, infertility, gynaecological oncology, ultrasound, uro-gynaecology, colposcopy, early
pregnancy problems and medical education. You will be part of a team, including Consultants
Specialty Trainees and you will be based at the new unit at Burnley. The posts are available in FY1
(un-banded) and FY2 which are full shift and compliant with the EWTD.
The competencies attained in Obstetrics & Gynaecology will include:





Communication skills
Breaking bad news
Care of the pregnant women
Management of acute emergencies, especially haemorrhagic shock

Surgery
The department covers general surgery, vascular surgery and urology. There are a full range of
sub-specialities within the department. You will be part of a team, including Consultants and
Specialty Trainees. The posts are available in FY1 and FY2. The FY1 posts include a one-week
placement in Critical Care. The FY2 rotas are full shift and are compliant with the EWTD.

Emergency Department
The posts are available in FY1 and FY2. The FY1 posts are un-banded. The FY2 rotas are full shift
and involve day, twilight and night duties. They are also compliant with the EWTD. The work will
involve being in the Emergency Department and the Blackburn and Burnley Urgent Care Centres.

The work will involve seeing, diagnosing and managing all types of emergency patients, trauma
and non-trauma in both adults and children, under the direct supervision of a senior colleague,
arranging any necessary investigations, instituting appropriate treatment and the disposal of the
patients safely and appropriately, whether by discharge or by referral to other specialities. The
experience involves trivial minor injuries to major life threatening illnesses and trauma and there
may be opportunities for day secondments to the ambulance service, the plaster room etc. There
is Consultant presence in the Emergency Department from 9.00am – 11:30pm 7 days a week, and
in the Urgent Care Centres from 9.00am until 5.00pm 5 days a week.
The competencies attained in Emergency Medicine will include:









Safe assessment and discharge of all patient groups, especially the acutely ill
Assessment and management of minor injuries
Practical skills (e.g., cannulation, CPR, insertion of chest drains, suturing, ABG,
venepuncture.)
Radiological interpretation skills (e.g., CXR, AXR, fractures)
Critical care skills
Understanding of ethical and legal issues – consent, breaking bad news, dealing with

Trauma and Orthopaedic Surgery

The unit undertakes major joint and revision surgery, specialist techniques in shoulder, knees and
foot & ankle. We have a very busy trauma service. FY1 will form part of the on call team. There
are daily trauma lists. You will be part of a team, including Consultants and Speciality Trainees.
The post holder will have Consultant led education and supervision and will be supported in their
professional development. You will be expected to undertake an audit project whilst in the unit
and will have the opportunity for exposure to all of the work of the department.

Ophthalmology
The department comprises of several sub specialities which include Cataracts, Glaucoma,
Oculoplastics, Paediatric Ophthalmology, Medical and Surgical Retina, External Eye Disease,
Botulinum Toxin Injections, some Refractive Surgery and General Ophthalmology.
This is a team of eight Ophthalmic Consultant Surgeons supported by two Associate Specialist, five
Staff Grades, two Specialty Trainees and four Clinical Fellows/Trust doctors. This team provides a
comprehensive ophthalmic service including a daily dedicated Accident & Emergency clinic.
The FY1 will have a fixed timetable taking into consideration the teaching sessions. They have
clinic and theatre sessions and will also have opportunity to shadow the doctor in eye emergency
clinics. Duties include clerking in and seeing ophthalmic cases for theatre, emergency cases and
ward patients. There would also be the opportunity of helping out in clinic.
There is a half day audit session held on a monthly basis. There is quite a lot of opportunity for
informal teaching during theatre and emergency clinic sessions.
Otolaryngology (ENT)
You will work in a team with the support of Consultants and Specialty Trainees. Your duties will
include emergency work both in the ENT casualty clinic, out-of-hours and routine care of pre and
post-operative patients on the ward. The timetable also includes attendance at operating theatre
lists and general and specialised out-patient clinics. The on-call is full shift.
The competencies attained in ENT will include:









Use of otoscope for the diagnosis of ear disease
Undertaking clinical tests of hearing, including tuning fork tests and voice testing
Interpretation of audiograms
Micro suction of ears
Nasal packing by various methods for epistaxis
Removal of foreign bodies from the ear, nose and throat
Pre and post-operative care of surgery patients
Use of flexible nasendoscopy for evaluation of pharynx and larynx
Drainage peritonsillar abscess (Quinsy)
Critical Care/Anaesthetics
There will be opportunities to work in theatre on both hospital sites. The posts are available in FY1
(un-banded) and are compliant with the EWTD.
In the Critical Care Unit you will be directly supervised by the Consultant for that week. You will
be expected to attend ward rounds and carry out clinical examinations, recording and collating the
information on the individual patients. You will have no direct responsibility for clinical decisions,
but will be expected to take part in a regular teaching programme and to make use of what
educational opportunities are available.
In Anaesthesia, you will be expected to accompany the Consultant whose list you are allocated to
on pre-operative visits, and to assist in theatre with general anaesthesia, and procedures

associated with this. You will be required to follow up patients, where appropriate. At no time
will you be expected to be responsible for anaesthetising a patient. You are similarly required to
take part in educational opportunities available.
You will have a learning contract and regular appraisal, with this feeding into your overall appraisal
for the year.
As well as the 4 month post the department also rotates all foundation doctors through a 1 week
placement in Critical Care to facilitate junior doctors obtaining skills in care of the acutely unwell
patient.
Radiology
During your working week you will have the opportunity to work alongside different members of
the imaging team including both Radiologists and Radiographers. You will gain experience in the
role of medical staff within the Department of Radiology setting and their role in the management
of patients and insight into the technical work carried out in the department. You will be expected
to participate in clinical meetings including cancer MDT’s and attend teaching sessions with
radiology trainees. You will also be expected to participate in clinical audit and will gain insight
into the role of clinical governance within a radiology department.
The competencies attained in Radiology will include:








Interpretation of plain film, CXR and abdomen
Awareness of appropriate modalities for investigation and the limitations of these
modalities.
Basic ultrasound technique and interpretation
Basic CT and MRI interpretation
Risk to patient associated with different imaging modalities
Opportunity to undertake basic screening procedures under supervision, eg. Barium
enemas, HSG
Gain exposure to interventional radiology and assist in procedures such as angiography and
image guided drainages
Administration of IV drugs/ contrast where applicable
The trainee will gain experience in the role of medical staff within the Department of Radiology
setting and their role in the management of patients and insight into the technical work carried
out in the department.
General Practice
During your placement in General Practice you will develop key skills including communication
skills, team work, triage and problem solving. Your understanding of clinical governance and risk
management in primary care and the development of critical appraisal and management skills will
be developed. You will also develop an understanding of the primary and secondary care roles,
responsibilities and understand how they interact.
The competencies attained in General Practice will include:

~ Being able to consult, visit and prescribe alone with surgeries of 6 - 8 patients at intervals of no
less than 15 minutes per patient
~ Start to manage both acute and chronic illness in the community

F1 year.

The post is subject to the Terms and Conditions of Service of
Hospital Medical and Dental Staff as modified from time to
time. Current copies of these Terms and conditions may be
seen on the Postgraduate Website.
All staff appointments are made subject to the receipt of
satisfactory references and all initial appointments to the
National Health Service are made subject to satisfactory
medical and police clearance being produced. A medical
examination may be necessary on initial appointment and
periodically thereafter.
Full information about the rotation, location and content of
placements is given in an accompanying document.
All Medical School Graduates are required to undertake a
mandatory four-day period of shadowing (the four days prior
to commencing as an FY1 trainee). However a two-week
period of shadowing is also available prior to commencing as
an FY1 trainee.
The salary scale is in accordance with the current national
rate and placement within the scale will be in accordance
with previous reckonable service.
FY1 doctors also are entitled to 27 days annual leave in the
year. The 27 days entitlement does not include the 8
statutory holidays. If a foundation doctor works any part of a
bank holiday they are entitled to an additional days leave in
lieu. In some specialties there is some restriction on when a
foundation doctor can take their leave during the 4 months
(e.g. not on their ‘hot week’ for surgery). The individual
directorates will inform the foundation doctor of any specific
restrictions on when they can take leave. Leave must be
booked at least 6 weeks in advance.
The Trust will consider British/EEA nationals for appointment
in the first instance. If the applicant is a non-resident of the
United Kingdom or European Economic Union, they are
required to have a valid work visa and leave to remain in the
UK, which is renewed as required. The Trust is unable to
employ or continue to employ the applicant/post holder if
they require but do not have a valid work visa and/or leave
to remain in the UK.
From August 2008 F1 doctors have been required to pay for
