Application Form
I wish to forego _________ of my annual basic salary in lieu of the provision of an
annual travel pass by Dublin City University.
I realise that this arrangement will operate for a period of one year and that the
reduction in salary will be reflected in my pay slips over that period.
I undertake to reimburse Dublin City University in full for any amount outstanding on
my annual travel pass should I leave the University from 1st February 2014 to 31st
January 2015.
I have read and I agree to the conditions attached to the Dublin City University
Travel Pass Scheme.
