Please save your completed author checklist as ac.topic.your intials.date in MMDDYY format (example: ac.hypertension.sb.040612.doc). Upon submission of the extension request form www.fpin.org/extension each author will be permitted one six-week extension. In the event of extenuating circumstances and a second extension is necessary, the faculty co-author or program champion must submit a second extension form through the website for editor in chief approval. Only after editor in chief approval is received will the extension be granted. This policy will apply throughout the editorial process with all versions. Should an author be unable to meet the due date requirements, a one week notice will be given and the question/topic will be made available to another author team. If your question/topic is taken away due to missed deadlines, you will have the opportunity to sign-up for a different question/topic, but extensions for the new topic/question may not be approved.
