LOCUM CONSULTANT PHYSICIAN WITH A SPECIAL INTEREST IN
Diabetes and Endocrinology (6 months fixed term)
Part-time 7 programmed Activities

NHS Forth Valley seeks to appoint an enthusiastic and innovative individual to join the
dynamic Diabetes and Endocrine service in Forth Valley. The Diabetes and Endocrine
service functions as a fully integrated unit with all members contributing to a team-based
approach to patient care. There will also be a commitment to general medical receiving
at an approximate frequency of 1 in 12 with overnight on call 1 in 24. This is a new post,
therefore we are hoping to appoint as soon as possible.
NHS Forth Valley provides acute services for a population of 310,000 people in Central
Scotland at the new Forth Valley Royal Hospital at Larbert which became fully
operational in July 2011.
The diabetes team is an integrated multidisciplinary group including a team manager, 5
full-time Consultant Physicians, 5.5 WTE Diabetic Specialist Nurses, 1.3 WTE Diabetes
dieticians (who also have a substantial education role), 0.4 WTE podiatrists, and 0.2
psychological input (both Clinical psychologist and Health psychologist). In joining this
team the successful candidate will play an important role in the continuing development
and redesign of Diabetes and Endocrine services at the new hospital.
Candidates will be expected to have a wide range of experience Diabetes and
Endocrinology and possess the MRCP (UK) or equivalent with full GMC registration and
a licence to practise.

INTRODUCTION TO NHS FORTH VALLEY
NHS Forth Valley is a single, integrated healthcare system comprising acute care facilities and
community based services, which are delivered through Forth Valley Community Health
Partnerships.
The Board’s vision of improving health and healthcare for the people of Forth Valley is
underpinned by its Integrated Healthcare Strategy. The principles upon which this strategy is

NHS Forth Valley is the largest employer in the area, with approximately 8000 staff from a wide
range of professional and support occupations. Currently, services are provided in one acute
hospital, four community hospitals and fifty-six health centres. Acute Services are based in
purpose built 860 bedded Forth Valley Royal Hospital, which opened in 2011 and provides
inpatient and outpatient services for a population of approximately 310,000.
THE FORTH VALLEY AREA
The working environment is excellent, as is the quality of life that Forth Valley has to offer them
and their families. The area has stunning scenery, such as the Trossachs and Loch Lomond. It
is steeped in history with Callander House in Falkirk, Stirling Castle, Bannockburn and the
Wallace Monument to name but a few of the sites in the area. It also has many cultural and
leisure facilities such as the Falkirk Wheel (the largest canal barge wheel in the world), the
MacRobert Arts Centre at Stirling and the National Sports Academy at Stirling University.
Forth Valley also lies equidistant between Scotland’s two largest cities; Edinburgh and Glasgow.
With their respective airports, both are little over half an hour from most parts of Forth Valley, on
the Falkirk, Larbert and Stirling area lines.
House hunters in the Forth Valley area are spoiled for choice. On offer are a large array of
established and new properties in a variety of attractive settings.
Education in the area is first class, with excellent provision of pre-school, primary and secondary
education. State schools in the area have strong reputations and there is also easy access to
the area’s private schools in Bridge of Allan, Dollar and Crieff.
On the further education front, Stirling boasts an excellent University and access to the
University cities of Glasgow and Edinburgh is easy. There are also further education Colleges in
Falkirk, Stirling and Alloa.
In addition to its scenery, history, culture and shopping facilities, residents in Forth Valley have

OVERVIEW OF THE DEPARTMENT AND POST
General Medicine and Acute Medical Receiving
There will be a commitment to General Medical receiving at an approximate frequency of
1 in 12 with overnight on call approximately 1 in 24.
Forth Valley Royal Hospital is organised as follows;
There is a Clinical Assessment Unit (“CAU”) with 16 spaces and 4 chairs including two
consulting rooms all adjacent to the A&E Department
The Acute Admission Unit provides an integrated assessment service . This unit provides
care for medical, ageing and health, and surgical patients in the first 24-48 hours following
admission. The unit contains 36 beds and there are also 12 short stay surgical beds.
Patients likely to stay less than 48 hours are discharged from the Acute Admissions Unit by
either the Acute Medicine team (Drs. Dan Beckett, Jennifer Gray, David Thetford, Dr
Catherine MacLean and Dr Leah Leitch) or the on call General Medical team responsible for
the admission. Patients likely to stay more than 48 hours are transferred to the appropriate
speciality ward. The Medical Specialities, Cardiology, Respiratory, Gastroenterology and
Diabetes and Endocrinology each provide a Subspecialty “Physician of the Week” Service
Mon-Fri 9-5. Patients are allocated to a Specialty within the Acute Assessment Unit and care
is taken over each weekday morning.
There is a 14 bedded specialty cardiology area and 6 chairs for chest pain assessment.
There are 23 Critical Care beds including HDU and 2 Coronary Care beds.
There are geographical ward bases for the major medical subspecialty teams

Cardiology
There are three consultant physicians with a special interest in Cardiology. These are Dr
Allan Bridges, Dr Stephen Glen and Dr Allister Hargreaves. There are two consultant
Cardiologists with no commitment to Acute Medicine: Dr James Spratt and Dr Catherine
Labinjoh.
Diabetes/Endocrinology
There are five consultants: Dr Chris Kelly, Dr Nick Barwell, Dr John Doig, Dr Linda Buchanan
and Dr Alison MacKenzie.
Gastroenterology
There are eight consultants: Dr Peter Bramley, Dr David Watts, Dr Stuart Paterson Dr
Richard Burnham, Dr Tim Heron, Dr Tim Gordon-Walker, Dr Santosh Salunke, and Dr
Sandeep Siddhi.

Respiratory Medicine
There are six consultants: Dr Dougie Morrison, Dr Billy Newman, Dr Fraser Wood, Dr
Melanie Cross, Dr Euan Cameron & Dr Mark Spears.
Other specialists supporting General Medicine
There is further support from a group of 10 physicians in Geriatric Medicine (who also
provide the Stroke Service), one Specialty Doctor ortho-geriatrician, 3 consultant
Haematologists who look after inpatients with haematological and lymphoproliferative
disorders plus patients with neutropenic sepsis, three consultant Rheumatologists and an
associate specialist in Rheumatology, 4 consultant Neurologists, one consultant in
Rehabilitation Medicine and 3 Consultant Dermatologists. There are visiting consultants in
Nephrology from Glasgow Royal Infirmary with a satellite hospital haemodialysis unit and
visiting consultants in Oncology from BOC Glasgow.
Current Junior Medical Staffing
We are currently supported by 21 FY1 grades, 8 FY2 grades, 30 specialty trainees (including
8 in GPST) and 1 specialty doctor.
There is a team of 19 Advanced Nurse and Nurse Practitioners who also support our hospital
at night (FACT) team.

Current Diabetes Service in Forth Valley
The diabetes team is an integrated multidisciplinary group including a team manager, 5
existing full-time Consultant Physicians, 5.5 WTE DSN, 1.3 WTE Diabetes dietitians (who
also have a substantial education role), 0.4 WTE podiatrists, 0.4 WTE pharmacist and
0.2 psychological input (both Clinical psychologist and Health psychologist)
The diabetes consultants work as the ‘consultant of the week’ one week in five and are
responsible for picking up and reviewing new specialty patients in AAU/CAU each
morning Monday-Friday. They will also see referrals from other wards with a
diabetes/endocrine issue as well as reviewing team patients in Day Medicine. The
inpatient service is covered by the “consultant of the week”. The diabetes consultants
share a 32 bedded ward with the haematology team who look after their own patients
and the neurology and rheumatology teams who operate a shared care model of care.
The majority of patients on the ward are general medical rather than specialty specific
patients. All patients in the ward are reviewed daily by the consultant of the week with
the support of the junior medical staff. This 1:5 commitment to being ‘consultant of the
week’ means all other activities are cancelled for that week.
The Consultant led Outpatient Service is based in the main Outpatient Areas at FVRH
and SCH, while a range of multi-disciplinary clinics, dietetic and DSN clinics are also held
within FVRH and Stirling Community Hospital.
Over the last decade the Diabetes Team in Forth Valley have developed a range of
patient focused services using extended scope practices involving nursing, dietetic,
pharmacy and podiatry staff, in addition to the more traditional Consultant led clinics in
order to comply with standards of care as laid down by the Diabetes Framework and
NHS Quality Improvement (Scotland).

There is a structured education programme (New DEAL) and all members of staff have
training in teaching and motivational interviewing. We have developed a structured
education programme for patients with type 1 diabetes using the principles of adult
structured education.
There is a well established CSII service with around 65 patients run by Dr Kelly
There are well established Paediatric/Adolescent Diabetes Clinics. Dr. John Schulga is
the Consultant Paediatrician supported by 3 Paediatric Diabetes Nurse Specialists.
There are transition/adolescent clinics involving Dr. John Doig at FVRH, and Dr Chris
Kelly at SCH with the paediatric team.
There are joint Obstetric/Diabetic clinics held weekly in FVRH. The Diabetologists
involved are Dr. Linda Buchanan, Dr Alison Mackenzie and Dr. Chris Kelly, and the
Obstetricians Dr Nicola Miller and Dr. Paul Holmes. All deliveries and the Neo-Natal
Intensive Care Unit are at FVRH.
There is a multidisciplinary Diabetic Foot Clinic held monthly at FVRH involving Dr Nick
Barwell, Mr Michael Yapanis, a Specialist Podiatrist and Wound Management Nurses.
Supporting IM&T
We use the SCI-DC hospital system and will be updating to the new SCI-Diabetes
system soon.
There are good working relationships with local Primary Care teams. Most practice staff
working with patients with diabetes have been through a Warwick or Bradford course.
There is a long-established Diabetes Managed Clinical Network with a dedicated MCN
Manager, David Munro. The Clinical Lead of the MCN is Hilary Whitty, our Lead Diabetes
Nurse.

Endocrinology Service
The 5 Consultants currently in post see patients with a wide range of endocrine
problems. The Endocrine Specialist Nurse role is being slowly developed through the
Medical Day Unit.
There is a local facility for the administration of radio-iodine treatments for thyrotoxicosis
and Dr Chris Kelly is the ARSAC Licence holder.
A full array of endocrine tests is available either in the local NHS Forth Valley laboratory
or from the regional laboratory in North Glasgow. There are excellent links for tertiary
referral with colleagues in both North Glasgow and Edinburgh. Patients with pituitary
tumours requiring Neurosurgical management are referred to Mr. Patrick Statham at the
Western General Hospital in Edinburgh, or Mr. Lawrence Dunn at the Southern General
Hospital in Glasgow, with both of whom there are excellent links and both these Centres
also provide post-operative radiotherapy to relevant patients.

OVERVIEW OF THE DEPARTMENT AND POST
Administrative Support
The postholder will have appropriate secretarial support and a computer with access to the NHS
network and resources.
Training and Education
There is a large educational facility within the main hospital building with a full range of library
services, details of which can be obtained at:

Forth Valley Royal Hospital has developed a comprehensive clinical skills teaching suite for the
teaching of resuscitation techniques and other procedural skills.
The Scottish Centre for Simulation and Clinical Human Factors is also based in the Forth Valley
Royal Hospital. There are weekly postgraduate meetings involving all specialties.
There are regular evening postgraduate medical meetings in the hospital for both hospital and
primary care medical staff.
Glasgow and Edinburgh with their Royal Colleges, University medical schools and postgraduate
facilities are in easy reach.
Teaching, Audit and Research
The post holder will be expected within his/her NHS responsibilities, to undertake audit, and to
partake in undergraduate and postgraduate teaching as required. They will also be expected to
share in teaching programmes for medical students, junior doctors and middle-grade medical
staff in Forth Valley. There is a well developed programme of weekly Medical Unit Clinical
Meetings, monthly hospital wide Clinical Forums and other Educational events. The successful
applicant will be expected to take an active part in the teaching of medical students who are
attached to the department.

LOCUM CONSULTANT RESPONSIBILITIES

As agreed with the General Manager and Associate Medical Director to provide with
Consultant colleagues an area wide service in General Medicine and Endocrinology and
Diabetes with responsibility for the prevention, diagnosis and management of illness, and
for the proper functioning of the service;

Out–of-hours responsibilities for Forth Valley Royal Hospital shared with the other
Consultants as part of a Consultant team;

To provide cover for Consultant colleagues during annual and study leave or at such
other times as agreed with the General Manager and Associate Medical Director ;

To offer professional supervision, training and appraisal of junior medical, nursing and
technical staff as appropriate to enhance role extension and job skills within the service
in order to improve the service provided to patients;

To motivate staff within the service, through leading by example, and fostering good
working relationships at all levels in line with the principles of the local Partnership
Agreements;

To contribute to developments and initiatives within the organisation as appropriate and
as requested;

To undertake teaching, accreditation and examination duties, and contribute to
postgraduate and continuing medical education activity across the area service;

To agree and implement an annual medical audit and clinical governance programme
with the Associate Medical Director in line with the Board’s Clinical Effectiveness
Strategy;

To participate fully in Consultant appraisal and personal development planning activities;

To ensure the efficient and effective use of the organisation’s resources;

To work with colleagues to ensure the effective and efficient development and
implementation of support systems.
The requirements of this post will change through time in line with clinical advances and
developments in services provided by the NHS in Forth Valley.

CONTRACTUAL DETAILS
Contracted activities for this appointment will be defined and agreed with the post holder and the
General Manager, Medical Directorate.
The post holder will be professionally responsible to the Associate Medical Director, Dr Allan
Bridges and managerially responsible to the General Manager, Ian Aitken, Medical Directorate
The Consultants in the department will agree arrangements for cover of Endocrinology &
Diabetes duties.
The Locum Consultant post is available on a full-time basis but the Board welcomes applications
from individuals who are unable to undertake a full time commitment and who may wish to work
on either a part-time or job share basis.
This post is offered on a part time fixed term basis for six months.
The terms and conditions of service applicable to this appointment are the Consultant grade
terms and conditions of service as amended from time to time.
NHS Forth Valley will require each Consultant to agree with the General Manager and Associate
Medical Director, a job plan of fixed and other commitments, which will be reviewed on an
annual basis and revised as necessary to meet the requirements of the local and national
service issues.
Consultants will be required to participate in an annual appraisal.
The post holder will normally be expected to reside within 30 minutes travelling distance of Forth
Valley Royal Hospital.
Formal appointment to this post will be conditional upon satisfactory completion of all preemployment checks, including satisfactory medical screening.
This appointment will be superannuable under the NHS Superannuation Scheme, which is
contracted out of the State Earnings Related Pension Scheme (SERPS). New employees will
automatically be enrolled in the scheme unless they opt out. For further information, please

The employment is subject to one calendar months notice on either side.
Annual leave entitlement is six weeks per year (pro rata for part time post holders), with

NHS Forth Valley does not negotiate salary placements.
On commencement of
employment, salary will be determined in accordance with Section 5 of the terms and
conditions of service.

JOB PLAN
The post holder is required to agree a detailed job plan with the General Manager or his / her
deputy and the Associate Medical Director. Efforts will be made to complete job planning prior to
the date of commencement in post, or, where this is not possible, as soon as possible following
this date.
The detail of fixed sessions, including any special interest, will be finalised following appointment
of the successful applicant. It is expected initially that 6 programmed activities per week will be
devoted to direct clinical care, with 1 programmed activity allocated for supporting professional
activities (including audit, professional development, teaching and administration). Attendance at
multi-disciplinary team meetings, pre-procedure consultations, is included within the 6
programmed activities of direct clinical care and will be appropriately timetabled.

Criminal Convictions - Protecting Vulnerable Groups (PVG) Scheme
The Protecting Vulnerable Groups (PVG) scheme has replaced previously disclosure
arrangements for people who work with vulnerable groups. The duties of this post require you to
undertake regulated work with children and young people (individuals aged under 18 years) and /
or protected adults (individuals aged 16 or over who is provided with (and thus receives) a type of
care, support or welfare service).

Health Screening
This post involves the performance of exposure prone procedures and therefore the successful
applicant will be required to produce evidence of their Hepatitis B and C Immunity Status. The
successful candidate will be required to produce evidence of Hepatitis B and C status prior to
appointment and to undergo satisfactory health screening. Any offer of employment will be
