The Boston Scientific Neuromodulation SCS Pain Fellows Workshop is limited to 40 pain medicine fellows and is offered on a first-come, first-served basis. The workshop will be held from 7:00 AM to 4:00

PM Wednesday, November 20 th . Please note, there is an initial limit of two pain medicine fellows per

Fellowship program. Additional registrations will be placed on a waitlist and accessed as space becomes available. This workshop is specifically designed for pain medicine fellows. All participants must be enrolled in a pain fellowship and meet the Boston Scientific Neuromodulation Clinical Training

Guidelines.

Following registration confirmation you will receive an email with logistics and travel details for the training. The workshop is offered at no charge and is supported by Boston Scientific Neuromodulation.

Financial support associated with attending the workshop includes air fare, Tuesday night, November

19 th lodging and transportation from the airport to the hotel where the workshop is being held. Please remember that if you are attending the ASRA 2013 Annual Pain Medicine Meeting you will need to register for the meeting and reserve the additional lodging required by following the booking instructions for the conference.

Due to compliance guidelines, fellows must use our travel agency and the travel agency will help select flights within our guidelines for this workshop

Your round trip flight, either departing after the workshop or on Sunday after the ASRA conference and your Tuesday night sleeping room must be booked by our BSN travel department

Travel arrival/departure times must coincide to the date & time of the event

All claims of extenuating travel circumstances will be reviewed by Compliance Counsel of BSC prior to approval
