One Murray Campus, Tishomingo, Oklahoma 73460 Proctor Information Procedures for Proctoring an Exam off Campus If a student is unable to come to campus for an exam they need to contact their instructor early in the semester to confirm that an exam can be proctored off campus. The Testing Centers will be notified by the instructor that an exam will need to be proctored off campus. The student can obtain through the instructor or Testing Centers the Proctor Agreement Form for off Campus Testing. The student is responsible to seek out a qualified proctor. If help is needed in obtaining a testing site the student may contact the Testing Center for assistance. The proctor must be pre-approved by the instructor and Testing Centers. The proctor selection is based on his/her availability, reliability, and impartiality. The proctor may not be neighbor, friend, family member, or workplace supervisor. The proposed proctor must complete the proctor agreement request and return it along with a business card, stationary with letterhead, or verification of a supervisor. A copy of the form must be returned to the instructor and Testing Center. The instructor completes his/her part of an Instructors Exam Request Form. The instructor will either approve or decline the proposed proctor. If the proctor is approved the student will need to contact the proctor and make an appointment to take the exam. The instructions from the instructor to the proctor on the Exam Request Form will include what materials can or cannot be used. The instructions will contact the instructor with passwords or any other instructions if the exam is on-line. In this manner the proctor receives the E-mail address of the instructor, student login if required and password. The proctor will utilize this information at the time of the exam. The student does not receive this form or any of the above information. Instructor lets the student know directly what materials can or cannot be used and what to expect at the time of the proctored exam. Student sits for the exam, the proctor retrieves the exam, if paper/pencil, and then follows the instructors request for delivery of the exam. If the exam is on-line the proctor is to let the instructor know when the exam was completed. The student is responsible for envelop and postage if test is to be mailed to the instructor.
