FLOOD INSURANCE – WAIVER OF AGENT RESPONSIBLITY
I hereby certify that my agent offered me flood insurance coverage in the National Flood Insurance Program
through Wright Flood. I understand that, because I declined this protection, my agent and the agency will be
held harmless and not liable in the event I suffer a flood loss. I have been made aware of the following facts
and statistics:
1. Homeowners insurance does not cover flood damage.
2. Federal disaster assistance is most typically an interest-bearing loan.
3. Flooding can and does occur in low-risk zones nationwide.
I reject PRP Condo Unit Owners building and contents coverage for flood protection.

Add either a $25.00 or a $250.00 surcharge to the premium in accordance with the Homeowner Flood Insurance Affordability Act of
2014 (HFIAA). The HFIAA surcharge is $25.00 for policies covering primary residences, including contents-only policies, if the named
insured’s primary residence is a single-family dwelling, an individual condominium unit, an apartment unit, or a unit in a cooperative
building. For all other policies, the HFIAA surcharge is $250.00.
Add the $50 Probation Surcharge, if applicable.
Premium includes a 10% Reserve Fund Assessment and $22 Federal Policy Fee.
Premium includes ICC Premium of $5 for residential coverage up to $230,000 and $4 for coverage over $230,000. Deduct this amount
if the risk is a condominium unit.
Use with basement or enclosure rates for buildings with crawlspaces or subgrade crawlspaces with an attached garage without proper
openings. Use without basement or enclosure rates for buildings with crawlspaces or subgrade crawlspaces, including those with an
attached garage that have proper openings. Updated to April 1, 2015 rates
